Introduction. One of the factors in the pathogenesis of atopic dermatitis is a dysfunction of the skin barrier. Background. To reveal the features of the barrier function of the skin in children with atopic dermatitis by measuring transepidermal water loss (TEWL) and the pH of the skin. Methods. The study included 98 children with atopic dermatitis. All patients measurement was performed by TEWL (Tewameter TM 300, Multi Probe Adapter MPA 5/9, Courage + Khazaka) and the pH of the skin (Skin-pHMeter, Multi Probe Adapter MPA 5/9, Courage + Khazaka). Results. TEWL value was significantly higher in children with severe atopic dermatitis (p=0,00001), in children up to 3 years (p=0,002), in patients with skin colonization of Staphylococcus aureus (p=0,006) and with specific IgE to staphylococcal enterotoxins. Skin pH was significantly higher in children with severe atopic dermatitis (p=0,0001), and in children under the age of 3 years (p=0,04). Conclusion. The determination of the level of TEWL and skin pH to assess the degree of inflammation activity in atopic dermatitis and to justify use of topical therapy, aimed to restoring the epidermal barrier of the skin at children with atopic dermatitis, is substantiated.

Abstract BackgroundSkin barrier functions develop after birth and may be related to skin disorders in infants. ObjectivesWe aimed to assess associations between dynamic trends of four skin barrier functional parameters in early life with infant atopic dermatitis (AD).MethodsBased on the prospective cohort MKNFOAD (NCT02889081), we examined transepidermal water loss (TEWL), stratum corneum hydration (SCH), skin pH, and sebum content at five anatomical sites (cheek, forehead, forearm, abdomen, and lower leg) in 418 term infants at birth, 42 days, and 6 months. Trend differences by sex and association with AD at age 1 year were tested using variance analyses. Associations of the parameters with AD risk were tested using discrete time survival analysis, adjusting extensive covariates including parental history of allergy, infant’s sex, birth weight (kg), and delivery mode. Odds ratios (ORs) and 95% confidence interval (CIs) were reported.ResultsOverall TEWL and SCH appeared trends of increase while skin surface pH and sebum content showed trends of decrease within the first 6 postnatal months. Sex differences were significant for sebum content only (p<0.001). After adjustment for parental and children covariates, cheek TEWL (OR=1.26, 95% CI 1.00–1.57, p=0.045) at birth and 42 days (OR=1.52, 95% CI 1.17–1.97, p=0.002) were significantly associated with increased AD risk. Associations were not observed between SCH, skin pH, and sebum content at birth or 42 days with AD. ConclusionsSkin barrier functions of Chinese term infants varied nonlinearly after birth. Higher postnatal TEWL levels in early life indicate higher risk of early-onset AD.

Abstract Background Atopic dermatitis (AD) is a chronic allergic inflammatory skin disease characterized by complex pathogenesis including skin barrier dysfunction, immune-redox disturbances, and pruritus. Prolonged topical treatment with medications such as corticosteroids, calcineurin inhibitors, and T-cell inhibitors may have some potential side-effects. To this end, many researchers have explored numerous alternative therapies using natural products and mineral compounds with antioxidant or immunomodulatory effects to minimize toxicity and adverse-effects. In the current study, we investigated the effects of mineral complex material (MCM) treatment on 2, 4-dinitrochlorobenzene (DNCB)-induced AD-like skin lesions in SKH-1 hairless mice. Methods Animals were divided into four groups; normal control (NC), negative control treated with DNCB only (DNCB only), positive control treated with DNCB and tacrolimus ointment (PC) and experimental group treated with DNCB and MCM patch (MCM). Skin inflammation and lesion severity were investigated through analyses of skin parameters (barrier score and strength, moisture and trans-epidermal water loss level), histopathology, immunoglobulin E, and cytokines. In addition, reactive oxygen species (ROS), nitric oxide (NO), glutathione peroxidase (GPx), and catalase (CAT) levels were measured in both serum and skin lysate. Results Our results demonstrates that MCM patch improved the progression of AD-like skin lesions by significantly increasing skin barrier strength and decreasing trans-epidermal water loss. Additionally, dermal administration of MCM patch significantly reduced epidermal thickness, ROS, and NO levels in skin lysate. Furthermore, we found that MCM suppressed the levels of AD-involved (Th1 and Th2) cytokines such as IL-2, IFN-γ, and IL-4 in blood. In addition, the levels of other Th1, and Th2 and inflammatory cytokines such as IL-1β, TNF-α, IL-6, IL-12(p70) and IL-10 were found lowest in the MCM group than in the DNCB only and PC groups. Moreover, we found total serum IgE level significantly increased after DNCB treatment, but decreased in the PC and MCM groups. Conclusion Taken together, our findings suggest that MCM application may have beneficial effects either systemic or regional on DNCB-induced AD lesional skin via regulation of the skin barrier function and immune-redox response.
