AbstractObjectivesTo analyse the changes in reported frequency of retinal detachment admissions and vitreo-retinal surgery procedures performed between 2000-2018 in England. To obtain information useful to contribute towards the planning of service delivery.MethodsAnalysis of England’s Hospital Episode Statistics from the Health and Social Care Information Centre and population data from the United Kingdom’s Office for National Statistics.ResultsEpisodes of “retinal detachments with breaks” increased year on year from 3,447 (7.0/100M) in 2000 to 10,971 (19.7/100M) in 2018 (p<0.001), whereas records of “tractional retinal detachment” increased from 290 (0.6/100M) to 910 (1.6/100M) in the same period (p<0.0001). The number of reported pars plana vitrectomies irrespective of indication increased over fourfold from 5,761 to 26,900 (p<0.0001), while the number of scleral buckling records decreased by two thirds from 2,897 to 780 (p<0.0001). During the same period the population of England increased from 49.2 million-55.6 million, proportionally at a slower rate than that for recorded hospital episodes.ConclusionsThe frequency of admissions to hospital for surgically treated retinal detachment seems to have been increasing significantly since 2000. This effect is more marked in cases of rhegmatogenous retinal detachment. This may be partially explained by repeat surgery in cases of recurrent retinal detachment. Other possible explanations may be increased incidence of disease (due to increased rates of cataract surgery, increasing longevity and rates of myopia), improvements in patient access, and increased public awareness. It is possible that this observation is due in part to local changes in methodology of hospital coding.

We report a large subconjunctival-orbital granuloma in a 51-year-old male presenting with a blind painful right eye and marked chemosis 15 months after undergoing vitrectomy and silicone oil retinal tamponade for retinal detachment with no reported intraoperative complications. Gross and histopathologic examination of the enucleated eye and episcleral tumor revealed a bosselated mass measuring 17 × 10 × 5 mm containing prominent vacuoles with surrounding epithelioid histiocytes and foreign body multinucleated giant cells. Such a large silicone-induced orbital granuloma following uncomplicated retinal surgery in a grossly intact eye has not been previously reported to the authors’ knowledge. High intraocular pressure and emulsification of oil may facilitate silicone extravasation through scleral wounds after retinal surgery.

Abstract Aim To evaluate the primary anatomical success and visual outcomes in patients with rhegmatogenous retinal detachment (RRD) undergoing 25-gauge pars plana vitrectomy (25g PPV) surgery in Pakistan. Method This is a five-year retrospective, interventional cohort study of 418 consecutive patients with RRD who underwent 25g PPV. Surgeries were performed by two experienced surgeons at tertiary care hospitals in Pakistan. Consecutive patients, who underwent 25 g PPV surgery as treatment for RRD between October 2013-October 2018 were included. We excluded patients who had a history of previous retinal surgery or who did not complete the 4-8 weeks primary outcome visit. Data was collected from the patient’s files by using a pro forma. SPSS version 23.0 (IBM SPSS Statistics, Armonk, NY) was used. A p-value of &lt; 0.5 was considered significant. Results We identified 452 patients through coding system of hospitals who underwent 25g PPV surgery between October 2013-October 2018. Secondary retinal surgery and incomplete follow up cases were excluded. A total of 441 patients’ files were reviewed for the study, out of which 418 patients met the criteria for final analysis. Males were in a higher number 284 (67.9 %). The average age was 49±15.8. In our study, 186 (44.4%) patients were phakic at the time of presentation. At 2 months follow up, the primary anatomical success rate was 87.1 %. Conclusions Surgical outcomes of RRD with 25g PPV surgery in our study were similar to the outcomes reported in the developed world. We propose a prospective multicenter national study to prospectively evaluate the risk factors for RRD surgical failure in the Pakistani population.
