Some Implications of Listening Level for Speech Reception by Sensorineural Hearing-Impaired Children

The benefits of amplification for sensorineural hearing-impaired persons listening at low levels were investigated. Eleven college students were tested, and the results suggest that the level above threshold at which a person listens to speech may ultimately influence the benefits obtained from amplification. The data indicate the possibility of an additional limitation to the potential for auditory speech communication when these persons cannot listen comfortably at levels above threshold of 20 to 30 dB or more.

The performance of 23 hearing-impaired children on a closed-response, auditory speech discrimination test and on an open-response, auditory speech discrimination test was compared to their performance on auditory tests of sensitivity, teacher-evaluated categories, and other related subject data. A comparison of the results of closed-response, auditory speech discrimination test and the open-response, auditory speech discrimination test indicates that the closed-response set test paradigm appears more productive for use with severely hearing-impaired subjects whose level of performance is low (but not 0%) on the open-response, auditory speech discrimination test. The closed-response test scores for this group are highly positively correlated to data dependent upon hearing function, whereas the open-response scores are not. Analyses of the closed-response set test results indicate that a closed-response set test paradigm can successfully demonstrate auditory speech discrimination error patterns on a subject group basis.

Σκοπός: Σκοπός αυτής της μελέτης ήταν η ανάπτυξη της Δοκιμασίας -Τέστ, το G-SEBSAT τέστ, στην ακουστική ομιλία βασισμένο σε προτάσεις για παιδιά με προβλήματα ακοής.Μέθοδοι: Προσλήφθηκαν εβδομήντα έξι παιδιά κατόπιν έγκρισης από την τοπική επιτροπή δεοντολογίας και κατόπιν ενημέρωσης από τους γονείς τους. Η συλλογή του λεξιλογίου βασίστηκε στην προβολή εικόνων που επιλέχθηκαν από δημοφιλείς αναγνωστικές ύλες στα ελληνικά στα παιδιά με προβλήματα ακοής (Hearing Impaired-ΗΙ). Εκπονήθηκε μια γραμματική ανάλυση περιεχομένου για να προσδιοριστούν οι μέσες συντακτικές και μορφολογικές δομές των φράσεων που χρησιμοποιούνται από τα παιδιά της ΗΙ ομάδας . Δέκα λίστες που σχετίζονται με αντοίστοιχες εικόνες, δημιουργήθηκαν με βάση το λεξιλόγιο και τη γραμματική ανάλυση και καταγράφηκαν από έναν άνδρα ομιλητή με μητρική του γλώσσα την σύγχρονη ελληνική. Οι δέκα λίστες παρουσιάστηκαν σε παιδιά με κανονική ακοή (Normal Hearing-NH) και σε παιδιά με HI, ενώ και στις δύο ομάδες καταγράφηκε το μέσο όριο απόκρισης ομιλίας (Speech Reception Threshold-SRT) καθώς και η κλίση της καμπύλης SRT στο επίπεδο SRT των 50% σωστών αποκρίσεων (S50). Οι κατάλογοι καταγγελιών επικυρώθηκαν σε σχέση με τη μεταβλητότητα της δυσκολίας τους σε κάθε ομάδα, καθώς και με τη μεταβλητότητα δοκιμής-επανεξέτασης των αντίστοιχων βαθμολογιών SRT.Αποτελέσματα: Το μέσο όριο απόκρισης ομιλίας (SRT) σε όλες τις λίστες για παιδιά με ΗΙ ήταν 65,27 dB και η κλίση της καμπύλης SRT στο επίπεδο SRT των 50% των σωστών αποκρίσεων ήταν 3,11% / dB. Τα αντίστοιχα αποτελέσματα σε όλους τους καταλόγους για τα παιδιά με NH ήταν 17,66 dB και 9,7% / dB αντίστοιχα. Τα SRT των παιδιών με ΗΙ συσχετίστηκαν έντονα θετικά, με στατιστικά σημαντικό τρόπο με την μέτρηση ακουστικού τόνου (Pure Tone Audiometry-ΡΤΑ) τόσο στις δοκιμασίες όσο και στις δοκιμαστικές συνεδρίες (δοκιμασία: r = 0.750, Ρ <0.0005, επανέλεγχος: r = 0.753, Ρ <0.0005). Η συσχέτιση Spearman των βαθμολογιών των τιμών SRT και των τιμών κλίσης ήταν 0.998 και 0.997 αντίστοιχα για το ΗΙ και 0.939 και 0.88 για την ομάδα με ΝΗ, υποδεικνύοντας πολύ χαμηλή μεταβλητότητα σε όλες τις συνεδρίες δοκιμής και επανεξέτασης. Επιπλέον, η ανάλυση της διακύμανσης (ANOVA) του μέσου SRT στα παιδιά με NH και τα υπολείμματα SRT στην ομάδα με HI έδειξε ότι οι διαφορετικές προτάσεις ήταν της ίδιας δυσκολίας σε κάθε ομάδα. ((F (9,81) = 0,401, ρ = 0,930 και (F (9,93) = 2,241, ρ = 0,025 αντίστοιχα).Συμπεράσματα: Για πρώτη φορά δημιουργήθηκε στην ελληνική γλώσσα επικυρωμένη δοκιμή ομιλίας ακουστικής φωνής. Οι τιμές SRT και S50 και για τα παιδιά με NH και με HI είναι συγκρίσιμες με παρόμοιες δοκιμές που αναπτύχθηκαν σε άλλες γλώσσες.

This study examined whether the Test of Auditory Comprehension of Language (TACL) (Carrow, 1973) scores were significantly affected by mode of presentation. The TACL was presented to 32 children with moderate sensorineural hearing losses. Two groups of 16 children were matched for age, sex, and hearing loss and were given either an auditory-only or auditory-visual presentation of the TACL. No significant difference was found between mean TACL scores by presentation. Mode of presentation had no effect on the TACL scores for those children examined.

AbstractDue to the close anatomical relationship between the auditory and vestibular end organs, hearing-impaired children have a higher risk for vestibular dysfunction, which can affect their (motor) development. Unfortunately, vestibular dysfunction often goes unnoticed, as vestibular assessment in these children is not standard of care nowadays. To timely detect vestibular dysfunction, the Vestibular Infant Screening–Flanders (VIS–Flanders) project has implemented a basic vestibular screening test for hearing-impaired infants in Flanders (Belgium) with a participation rate of 86.7% during the first year and a half. The cervical Vestibular Evoked Myogenic Potentials (cVEMP) test was applied as vestibular screening tool to map the occurrence of vestibular (mainly saccular) dysfunction in this population. At the age of 6 months, 184 infants were screened. No refers on vestibular screening were observed in infants with permanent conductive hearing loss. In infants with permanent sensorineural hearing loss, a cVEMP refer rate of 9.5% was observed. Failure was significantly more common in infants with severe-profound compared to those with mild-moderate sensorineural hearing loss (risk ratio = 9.8). Since this is the first regional study with a large sample size and successful participation rate, the VIS–Flanders project aims to set an example for other regions worldwide.

A review and critique of the research on acupuncture and transdermal electrostimulation therapy in treating sensorineural hearing loss is presented. Of 111 subjects who had undergone acupuncture treatment only 4% showed improvement. Some of the subjects in these studies were noted to exhibit a decrease in hearing sensitivity. Additional data collected at our clinics on four preschool hearing-impaired children failed to provide evidence of improvement following acupuncture. Similar results are reported with transdermal therapy, suggesting that both procedures are inappropriate for the treatment of sensorineural hearing loss.

Aims: This study aims to assess the academic performance of children with hearing impairment who received early intervention in Lahore. Study Design:  Cross sectional survey design was used. Place and Duration of Study: Data was collected from Special Institute/School; Hamza foundation academy Lahore, Pakistan for the duration of six months from March 2021 to September 2021. Methodology: 97 students with moderate to severe sensorineural hearing loss children (aged in between 4 to 12), using hearing aids (HA’s) and cochlear implant (CI) were included by using purposive sampling technique. Hearing impaired children with other than sensorineural hearing loss and children who didn’t receive early intervention (hearing aids/implants or speech therapy) were excluded from this study. Results: It was found that 97 children with hearing impairment achieved significantly in their test score (80 to 99%) across English, Science, and Mathematics as compared to Urdu and Islamiyat (70 to 79%) after the implementation of intervention strategies. Conclusion: The study conclude that, while children with hearing impairment faced struggle in some areas of academics which includes listening and imitation in subjects like Urdu (structure of words) and Islamiyat (due to Arabic talafuz), their  academic performance in Math, English, and Science is higher with overall achieved percentage between  80 to 99%.
