Acoustical and Perceptual Analysis of Noise Reduction Strategies in Individuals With Auditory Neuropathy Spectrum Disorders

Purpose The conventional amplification devices render minimal or no benefit at abating the speech perception problems of individuals with auditory neuropathy spectrum disorder (ANSD). This study was undertaken to evaluate the effect of noise reduction strategies (multiband spectral subtraction, Wiener-as, Karhunen–Loeve transform [Subspace], and ideal binary mask [IdBM] algorithm) on speech using speech perception measures and acoustic measure among individuals with ANSD. Method Two groups of participants (age: 17–43 years) were recruited in the study. Group I comprised 12 individuals with a confirmed diagnosis of ANSD and not exceeding moderate degree of hearing loss and Group II of 10 individuals with normal hearing in both ears. The signal-to-noise required for 50% speech recognition (SNR-50) was measured for the participants in five conditions, that is, unprocessed speech and speech processed with four noise reduction strategies. Additionally, an acoustic objective measure Extended Short-Time Objective Intelligibility algorithm was employed to estimate the intelligibility index across the conditions. Results Significant difference was found across conditions in both the groups. Pairwise comparison revealed significantly better speech perception on SNR-50 measure with IdBM strategy, for both the groups. No significant difference in SNR-50 was observed with other noise reduction strategies. IdBM condition also gave the highest intelligibility index ( d ) values using Extended Short-Time Objective Intelligibility algorithm. This finding needs to be verified on a larger group of individuals with ANSD. Conclusions IdBM noise reduction strategy rendered significantly lower SNR-50 compared to other noise reduction strategies for individuals with ANSD in this study. This provides clinical evidence for the same and also recommends trying on a larger group of participants before its implementation in hearing devices. Apart from this, the current strategies used in hearing aids provide no improvement in speech identification in noise for this population. Hence, though the present hearing aids may show benefit in quiet condition, chances of its rejection are high in noisy backgrounds.

The stated results of using outbreeding and inbreeding in a selection of cattle of the Ukrainian Whitehead breed, which belongs to the local population and is preserved only in one breeding herd. It was revealed that animals obtained from related breeding are characterized by a sufficiently high level of productivity and normal reproductive qualities. Although outbred and inbred heifers in the process of growth from birth to 18 months of age differed somewhat from each other in live weight, the difference was not statistically significant. In different periods of growth, both outbred and inbred animals had an advantage, indicating the possibility of improving the trait by purebred breeding methods. The absolute increase in live weight of experimental heifers on rearing was: Group I – 295 kg, II – 289 kg, III – 298 kg, IV – 308 kg and V – 293 kg without a statistically significant difference between the groups, which made it possible to conclude that there was no inbred depression when breeding cattle of a given herd through a related selection of parental pairs. It is recognized that the selection of calves to increase their live weight at birth will not have a positive effect on the trait during the growth of animals, as evidenced by the correlation between them. As a result of the study of reproductive ability, the expediency of obtaining and using inbred cattle in a herd, which was inseminated almost a month earlier than outbred, was proved. Between outbred and inbred cows, a significant differentiation of milk yield for the first – third lactation was found, without a significant advantage of animals obtained by one or another method of purebred breeding. According to the first lactation, the highest milk yield was observed in cows of the close inbreeding group (group V) – 4501 kg, which exceeded the outbred ones by 150 kg and the lower inbred ones (groups II-IV) by 191 – 633 kg. Inbred cattle with distant, moderate and close degrees of inbreeding at the second lactation had from 4629 kg to 4719 kg of milk, with the highest rate in cows of a moderate degree of inbreeding, while outbred cows produced only 4582 kg. Cows of a moderate degree of inbreeding had the highest milk yield in the third lactation – 5204 kg and the lowest – 3897 kg of the group of a close degree of inbreeding. Outbred cows for this trait were superior only to individuals of the group of close inbreeding. The results of a comparative analysis of economically useful traits of outbred and inbred cows and heifers of the Ukrainian Whitehead breed indicate the possibility of using a related selection of parental pairs in a herd to increase the efficiency of interbreeding selection, replicate the hereditary traits of the ancestor and preserve the disappearing domestic breed of cattle.

Background and Objectives: Although many studies have evaluated the effect of the digital noise reduction (DNR) algorithm of hearing aids (HAs) on speech recognition, there are few studies on the effect of DNR on music perception. Therefore, we aimed to evaluate the effect of DNR on music, in addition to speech perception, using objective and subjective measurements. Subjects and Methods: Sixteen HA users participated in this study (58.00±10.44 years; 3 males and 13 females). The objective assessment of speech and music perception was based on the Korean version of the Clinical Assessment of Music Perception test and word and sentence recognition scores. Meanwhile, for the subjective assessment, the quality rating of speech and music as well as self-reported HA benefits were evaluated. Results: There was no improvement conferred with DNR of HAs on the objective assessment tests of speech and music perception. The pitch discrimination at 262 Hz in the DNR-off condition was better than that in the unaided condition (<i>p</i>=0.024); however, the unaided condition and the DNR-on conditions did not differ. In the Korean music background questionnaire, responses regarding ease of communication were better in the DNR-on condition than in the DNR-off condition (<i>p</i>=0.029). Conclusions: Speech and music perception or sound quality did not improve with the activation of DNR. However, DNR positively influenced the listener’s subjective listening comfort. The DNR-off condition in HAs may be beneficial for pitch discrimination at some frequencies.

Background: Transient noise can be disruptive for people wearing hearing aids. Ideally, the transient noise should be detected and controlled by the signal processor without disrupting speech and other intended input signals. A technology for detecting and controlling transient noises in hearing aids was evaluated in this study. Purpose: The purpose of this study was to evaluate the effectiveness of a transient noise reduction strategy on various transient noises and to determine whether the strategy has a negative impact on sound quality of intended speech inputs. Research Design: This was a quasi-experimental study. The study involved 24 hearing aid users. Each participant was asked to rate the parameters of speech clarity, transient noise loudness, and overall impression for speech stimuli under the algorithm-on and algorithm-off conditions. During the evaluation, three types of stimuli were used: transient noises, speech, and background noises. The transient noises included “knife on a ceramic board,” “mug on a tabletop,” “office door slamming,” “car door slamming,” and “pen tapping on countertop.” The speech sentences used for the test were presented by a male speaker in Mandarin. The background noises included “party noise” and “traffic noise.” All of these sounds were combined into five listening situations: (1) speech only, (2) transient noise only, (3) speech and transient noise, (4) background noise and transient noise, and (5) speech and background noise and transient noise. Results: There was no significant difference on the ratings of speech clarity between the algorithm-on and algorithm-off (t-test, p = 0.103). Further analysis revealed that speech clarity was significant better at 70 dB SLP than 55 dB SPL (p < 0.001). For transient noise loudness: under the algorithm-off condition, the percentages of subjects rating the transient noise to be somewhat soft, appropriate, somewhat loud, and too loud were 0.2, 47.1, 29.6, and 23.1%, respectively. The corresponding percentages under the algorithm-on were 3.0, 72.6, 22.9, and 1.4%, respectively. A significant difference on the ratings of the transient noise loudness was found between the algorithm-on and algorithm-off (t-test, p < 0.001). For overall impression for speech stimuli: under the algorithm-off condition, the percentage of subjects rating the algorithm to be not helpful at all, somewhat helpful, helpful, and very helpful for speech stimuli were 36.5, 20.8, 33.9, and 8.9%, respectively. Under the algorithm-on condition, the corresponding percentages were 35.0, 19.3, 30.7, and 15.0%, respectively. Statistical analysis revealed there was a significant difference on the ratings of overall impression on speech stimuli. The ratings under the algorithm-on condition were significantly more helpful for speech understanding than the ratings under algorithm-off (t-test, p < 0.001). Conclusions: The transient noise reduction strategy appropriately controlled the loudness for most of the transient noises and did not affect the sound quality, which could be beneficial to hearing aid wearers.

Introduction The bone-anchored hearing aid (BAHA) is a bone conduction system that transmits the sound directly to the inner ear by surpassing the skin impedance and the subcutaneous tissue. It is indicated for patients with mixed, conductive and unilateral sensorineural hearing loss who did not benefit from conventional hearing aids (HAs). Although the benefits from BAHA are well demonstrated internationally, this field still lacks studies in Brazil. Objective To assess the auditory rehabilitation process in BAHA users through audiological, speech perception and tinnitus aspects. Methods Individuals with hearing loss were assessed before and after the implantation. The participants were subjected to pure tone audiometry in free field, functional gain audiometry, speech perception tests, tinnitus handicap inventory (THI) in open format, and to the visual analog scale (VAS). Results It was found that the participants benefited from the use of BAHA. The difference in the performance of the participants before and after the BAHA surgery was significant in terms of hearing acuity. There was no statistically significant difference in the speech perception tests. The tinnitus assessment showed that 80% of the participants scored slight tinnitus severity in THI after using a BAHA. Eighty percent of the participants classified their tinnitus as absent to mild in the VAS after the surgery. Conclusion Based on the results of the current study, we can conclude that the participants improved both the auditory perception and the tinnitus handicap.

Background: Before advanced noise-management features can be recommended for use in children with hearing loss, evidence regarding their ability to use these features to optimize speech perception is necessary. Purpose: The purpose of this study was to examine the relation between children's preference for, and performance with, four combinations of noise-management features in noisy listening environments. Research Design: Children with hearing loss were asked to repeat short sentences presented in steady-state noise or in multitalker babble while wearing ear-level hearing aids. The aids were programmed with four memories having an orthogonal arrangement of two noise-management features. The children were also asked to indicate the hearing aid memory that they preferred in each of the listening conditions both initially and after a short period of use. Study Sample: Fifteen children between the ages of 8 and 12 yr with moderate hearing losses, bilaterally. Results: The children's preference for noise management aligned well with their performance for at least three of the four listening conditions. The configuration of noise-management features had little effect on speech perception with the exception of reduced performance for speech originating from behind the child while in a directional hearing aid setting. Additionally, the children's preference appeared to be governed by listening comfort, even under conditions for which a benefit was not expected such as the use of digital noise reduction in the multitalker babble conditions. Conclusions: The results serve as evidence in support of the use of noise-management features in grade-school children as young as 8 yr of age.

Background: Auditory neuropathy spectrum disorder (ANSD) is a form of sensorineural hearing loss, causing severe deficits in speech perception. The perceptual problems of individuals with ANSD were attributed to their temporal processing impairment rather than to reduced audibility. This rendered their rehabilitation difficult using hearing aids. Although hearing aids can restore audibility, compression circuits in a hearing aid might distort the temporal modulations of speech, causing poor aided performance. Therefore, hearing aid settings that preserve the temporal modulations of speech might be an effective way to improve speech perception in ANSD. Purpose: The purpose of the study was to investigate the perception of hearing aid–processed speech in individuals with late-onset ANSD. Research Design: A repeated measures design was used to study the effect of various compression time settings on speech perception and perceived quality. Study Sample: Seventeen individuals with late-onset ANSD within the age range of 20–35 yr participated in the study. Data Collection and Analysis: The word recognition scores (WRSs) and quality judgment of phonemically balanced words, processed using four different compression settings of a hearing aid (slow, medium, fast, and linear), were evaluated. The modulation spectra of hearing aid–processed stimuli were estimated to probe the effect of amplification on the temporal envelope of speech. Repeated measures analysis of variance and post hoc Bonferroni’s pairwise comparisons were used to analyze the word recognition performance and quality judgment. Results: The comparison between unprocessed and all four hearing aid–processed stimuli showed significantly higher perception using the former stimuli. Even though perception of words processed using slow compression time settings of the hearing aids were significantly higher than the fast one, their difference was only 4%. In addition, there were no significant differences in perception between any other hearing aid–processed stimuli. Analysis of the temporal envelope of hearing aid–processed stimuli revealed minimal changes in the temporal envelope across the four hearing aid settings. In terms of quality, the highest number of individuals preferred stimuli processed using slow compression time settings. Individuals who preferred medium ones followed this. However, none of the individuals preferred fast compression time settings. Analysis of quality judgment showed that slow, medium, and linear settings presented significantly higher preference scores than the fast compression setting. Conclusions: Individuals with ANSD showed no marked difference in perception of speech that was processed using the four different hearing aid settings. However, significantly higher preference, in terms of quality, was found for stimuli processed using slow, medium, and linear settings over the fast one. Therefore, whenever hearing aids are recommended for ANSD, those having slow compression time settings or linear amplification may be chosen over the fast (syllabic compression) one. In addition, WRSs obtained using hearing aid–processed stimuli were remarkably poorer than unprocessed stimuli. This shows that processing of speech through hearing aids might have caused a large reduction of performance in individuals with ANSD. However, further evaluation is needed using individually programmed hearing aids rather than hearing aid–processed stimuli.

Background: Research suggests that for individuals with significant low-frequency hearing, implantation of a short-electrode cochlear implant may provide benefits of improved speech perception abilities. Because this strategy combines acoustic and electrical hearing within the same ear while at the same time preserving low-frequency residual acoustic hearing in both ears, localization abilities may also be improved. However, very little research has focused on the localization and spatial hearing abilities of users with a short-electrode cochlear implant. Purpose: The purpose of this study was to evaluate localization abilities for listeners with a short-electrode cochlear implant who continue to wear hearing aids in both ears. A secondary purpose was to document speech perception abilities using a speech-in-noise test with spatially separate noise sources. Research Design: Eleven subjects that utilized a short-electrode cochlear implant and bilateral hearing aids were tested on localization and speech perception with multiple noise locations using an eight-loudspeaker array. Performance was assessed across four listening conditions using various combinations of cochlear implant and/or hearing aid use. Results: Results for localization showed no significant difference between using bilateral hearing aids and bilateral hearing aids plus the cochlear implant. However, there was a significant difference between the bilateral hearing aid condition and the implant plus use of a contralateral hearing aid for all 11 subjects. Results for speech perception showed a significant benefit when using bilateral hearing aids plus the cochlear implant over use of the implant plus only one hearing aid. Conclusion: Combined use of both hearing aids and the cochlear implant show significant benefits for both localization and speech perception in noise for users with a short-electrode cochlear implant. These results emphasize the importance of low-frequency information in two ears for the purpose of localization and speech perception in noise.

AbstractThe use of probe microphone measures in hearing aid verification is often neglected or not fully used by practitioners. Some practitioners rely on simulated gain and output provided by manufacturer's fitting software to verify hearing aids.This study aims to evaluate the effectiveness of manufacturer’s prefit procedure in matching the prescribed real-ear targets. It also aims to study its correlated impact on the predicted speech perception in children with severe and profound hearing loss.This cross-sectional experiment was carried out by measuring the output of hearing aids based on prefit versus real-ear at low-, moderate-, and high-input levels. The predicted speech perception for different hearing aid fittings was determined based on the Speech Intelligibility Index (SII).Sixteen children (28 ears) aged between 4 and 7 yr, with severe to profound sensorineural hearing loss took part in the study.Two different types of hearing aids (Phonak and Unitron) were programmed based on their respective manufacturers’ Desired Sensation Levels (DSL) v5 Child procedure. The hearing aids were then verified using coupler-based measurements and individual real-ear-to-coupler differences. The prefit outputs were compared with the DSL v5 Child–prescribed outputs at low-, moderate-, and high-input levels. The hearing aids were then adjusted to closely match the prescribed output. The SIIs were calculated for the fittings before and after adjustment.Sixty four percent of fittings that were based on the prefit procedure achieved the optimal fit-to-targets, with less than 5-dB RMS deviations from the DSL v5 Child targets. After adjusting the hearing aids to attempt to meet the DSL v5 Child targets, 75% of the ears tested achieved the optimal fit-to-targets. On average, hearing aid outputs generated by the manufacturer’s prefit procedure had good and reasonable agreement with the DSL v5 Child–prescribed outputs at low- and mid-frequencies. Nonetheless, at 4000 Hz, the hearing aid output mostly fell below the DSL v5 Child–prescribed outputs. This was still the case even after the hearing aid was adjusted to attempt to match with the targets. At low input level, some prefit outputs were found to be higher than the prescribed outputs. The deviations of prefit outputs from the prescribed outputs were dependent on the type of hearing aid and input levels. There was no significant difference between the SII calculated for fittings based on the prefit and adjusted fit.Prefit procedure tends to produce outputs that were below the DSL v5 Child–prescribed outputs, with the largest mean difference at 4000 Hz. Even though the hearing aid gains were adjusted to attempt to match with the targets, the outputs were still below the targets. The limitations of hearing aids to match the DSL v5 Child targets at high-frequency region have resulted in no improvement in the children’s predicted speech perception.
