Comparing Recognition of Distorted Speech Using an Equivalent Signal-to-Noise Ratio Index

An index of equivalent performance in noise was developed to compare recognition in different forms of speech distortion. Speech-recognition performance of young and elderly listeners with and without hearing loss was evaluated for undistorted speech presented in quiet and noise, and for speech distorted by four time-compression ratios and by four reverberation times. The data obtained in noise on young subjects with normal hearing served to generate a normalized regression equation, which was used to convert percent-correct performance in different distortion conditions to equivalent performance for undistorted speech at a particular S/N ratio. Comparisons of the equivalent S/N ratios obtained in the various conditions allowed rank-ordering of speech recognition performance in different types of degradation. The data also show that age and hearing loss affect recognition of speech degraded by reverberation or time compression. However, age effects are evident primarily in the most severe distortion conditions. Recognition of undistorted speech in noise was affected by hearing loss but not by age. These findings support a hypothesis that stipulates that increased age produces a reduction in the functional S/N ratio.

Improving the signal-to-noise ratio (SNR) for individuals with hearing loss who are listening to speech in noise provides an obvious benefit. Although binaural hearing provides the greatest advantage over monaural hearing in noise, some individuals with symmetrical hearing loss choose to wear only one hearing aid. The present study tested the hypothesis that individuals with symmetrical hearing loss fit with one hearing aid would demonstrate improved speech recognition in background noise with increases in head turn. Fourteen individuals were fit monaurally with a Starkey Gemini in-the-ear (ITE) hearing aid with directional and omnidirectional microphone modes. Speech recognition performance in noise was tested using the audiovisual version of the Connected Speech Test (CST v.3). The test was administered in auditory-only conditions as well as with the addition of visual cues for each of three head angles: 0°, 20°, and 40°. Results indicated improvement in speech recognition performance with changes in head angle for the auditory-only presentation mode at the 20° and 40° head angles when compared to 0°. Improvement in speech recognition performance for the auditory + visual mode was noted for the 20° head angle when compared to 0°. Additionally, a decrement in speech recognition performance for the auditory + visual mode was noted for the 40° head angle when compared to 0°. These results support a speech recognition advantage for listeners fit with one ITE hearing aid listening in a close listener-to-speaker distance when they turn their head slightly in order to increase signal intensity.

Objective: This study was aimed at evaluating improvements in speech-in-noise recognition ability as measured by signal-to-noise ratio (SNR) with the use of wireless remote microphone technology. These microphones transmit digital signals via radio frequency directly to hearing aids and may be a valuable assistive listening device for the hearing-impaired population of Mandarin speakers in China.Methods: Twenty-three adults (aged 19–80 years old) and fourteen children (aged 8–17 years old) with bilateral sensorineural hearing loss were recruited. The Mandarin Hearing in Noise Test was used to test speech recognition ability in adult subjects, and the Mandarin Hearing in Noise Test for Children was used for children. The subjects’ perceived SNR was measured using sentence recognition ability at three different listening distances of 1.5, 3, and 6 m. At each distance, SNR was obtained under three device settings: hearing aid microphone alone, wireless remote microphone alone, and hearing aid microphone and wireless remote microphone simultaneously.Results: At each test distance, for both adult and pediatric groups, speech-in-noise recognition thresholds were significantly lower with the use of the wireless remote microphone in comparison with the hearing aid microphones alone (P < 0.05), indicating better SNR performance with the wireless remote microphone. Moreover, when the wireless remote microphone was used, test distance had no effect on speech-in-noise recognition for either adults or children.Conclusion: Wireless remote microphone technology can significantly improve speech recognition performance in challenging listening environments for Mandarin speaking hearing aid users in China.

This study investigated the hypothesis that age effects exert an increased influence on speech recognition performance as the number of acoustic degradations Of the speech signal increases. Four groups participated: young listeners with normal hearing, elderly listeners with normal hearing, young listeners with hearing loss, and elderly listeners with hearing loss. Recognition was assessed for sentence materials degraded by noise, reverberation, or time compression, either in isolation or in binary combinations. Performance scores were converted to an equivalent signal-to-noise ratio index to facilitate direct comparison of the effects of different forms of stimulus degradation. Age effects were observed primarily in multiple degradation conditions featuring time compression of the stimuli. These results are discussed in terms of a postulated change in functional signal-to-noise ratio with increasing age.

Background: Recently there has been interest in using subjective ratings as a measure of perceived effort during speech recognition in noise. Perceived effort may be an indicator of cognitive load. Thus, subjective effort ratings during speech recognition in noise may covary both with signal-to-noise ratio (SNR) and individual cognitive capacity. Purpose: The present study investigated the relation between subjective ratings of the effort involved in listening to speech in noise, speech recognition performance, and individual working memory (WM) capacity in hearing impaired hearing aid users. Research Design: In two experiments, participants with hearing loss rated perceived effort during aided speech perception in noise. Noise type and SNR were manipulated in both experiments, and in the second experiment hearing aid compression release settings were also manipulated. Speech recognition performance was measured along with WM capacity. Study Sample: There were 46 participants in all with bilateral mild to moderate sloping hearing loss. In Experiment 1 there were 16 native Danish speakers (eight women and eight men) with a mean age of 63.5 yr (SD = 12.1) and average pure tone (PT) threshold of 47. 6 dB (SD = 9.8). In Experiment 2 there were 30 native Swedish speakers (19 women and 11 men) with a mean age of 70 yr (SD = 7.8) and average PT threshold of 45.8 dB (SD = 6.6). Data Collection and Analysis: A visual analog scale (VAS) was used for effort rating in both experiments. In Experiment 1, effort was rated at individually adapted SNRs while in Experiment 2 it was rated at fixed SNRs. Speech recognition in noise performance was measured using adaptive procedures in both experiments with Dantale II sentences in Experiment 1 and Hagerman sentences in Experiment 2. WM capacity was measured using a letter-monitoring task in Experiment 1 and the reading span task in Experiment 2. Results: In both experiments, there was a strong and significant relation between rated effort and SNR that was independent of individual WM capacity, whereas the relation between rated effort and noise type seemed to be influenced by individual WM capacity. Experiment 2 showed that hearing aid compression setting influenced rated effort. Conclusions: Subjective ratings of the effort involved in speech recognition in noise reflect SNRs, and individual cognitive capacity seems to influence relative rating of noise type.

The results of recent animal studies have suggested that cochlear synaptopathy may be an important factor involved in presbycusis. Therefore, here, we aimed to examine whether cochlear synaptopathy frequently exists in patients with presbycusis and to describe the effect of cochlear synaptopathy on speech recognition in noise. Based on the medical history and an audiological examination, 94 elderly patients with bilateral, symmetrical, sensorineural hearing loss were diagnosed as presbycusis. An electrocochleogram, auditory brainstem responses, auditory cortical evoked potentials, and speech audiometry were recorded to access the function of the auditory pathway. First, 65 ears with hearing levels of 41-50 dB HL were grouped based on the summating potential/action potential (SP/AP) ratio, and the amplitudes of AP and SP were compared between the two resulting groups. Second, 188 ears were divided into two groups: the normal SP/AP and abnormal SP/AP groups. The speech recognition abilities in the two groups were compared. Finally, the relationship between abnormal electrocochleogram and poor speech recognition (signal-to-noise ratio loss ≥7 dB) was analyzed in 188 ears. The results of the present study showed: (1) a remarkable reduction in the action potential amplitude was observed in patients with abnormal SP/AP ratios; this suggests that cochlear synaptopathy was involved in presbycusis. (2) There was a large proportion of patients with poor speech recognition in the abnormal SP/AP group. Furthermore, a larger number of cases with abnormal SP/AP ratios were confirmed among patients with presbycusis and poor speech recognition. We concluded that cochlear synaptopathy is not uncommon among elderly individuals who have hearing ability deficits, and it may have a more pronounced effect on ears with declining auditory performance in noisy environments.

The equalization cancellation model is often used to predict the binaural masking level difference. Previously its application to speech in noise has required separate knowledge about the speech and noise signals to maximize the signal-to-noise ratio (SNR). Here, a novel, blind equalization cancellation model is introduced that can use the mixed signals. This approach does not require any assumptions about particular sound source directions. It uses different strategies for positive and negative SNRs, with the switching between the two steered by a blind decision stage utilizing modulation cues. The output of the model is a single-channel signal with enhanced SNR, which we analyzed using the speech intelligibility index to compare speech intelligibility predictions. In a first experiment, the model was tested on experimental data obtained in a scenario with spatially separated target and masker signals. Predicted speech recognition thresholds were in good agreement with measured speech recognition thresholds with a root mean square error less than 1 dB. A second experiment investigated signals at positive SNRs, which was achieved using time compressed and low-pass filtered speech. The results demonstrated that binaural unmasking of speech occurs at positive SNRs and that the modulation-based switching strategy can predict the experimental results.

The Modified Rhyme Test (MRT), recorded using natural speech and two forms of synthetic speech, DECtalk and Votrax, was used to measure both open-set and closed-set speech-recognition performance. Performance of hearing-impaired elderly listeners was compared to two groups of young normal-hearing adults, one listening in quiet, and the other listening in a background of spectrally shaped noise designed to simulate the peripheral hearing loss of the elderly. Votrax synthetic speech yielded significant decrements in speech recognition compared to either natural or DECtalk synthetic speech for all three subject groups. There were no differences in performance between natural speech and DECtalk speech for the elderly hearing-impaired listeners or the young listeners with simulated hearing loss. The normal-hearing young adults listening in quiet out-performed both of the other groups, but there were no differences in performance between the young listeners with simulated hearing loss and the elderly hearing-impaired listeners. When the closed-set identification of synthetic speech was compared to its open-set recognition, the hearing-impaired elderly gained as much from the reduction in stimulus/response uncertainty as the two younger groups. Finally, among the elderly hearing-impaired listeners, speech-recognition performance was correlated negatively with hearing sensitivity, but scores were correlated positively among the different talker conditions. Those listeners with the greatest hearing loss had the most difficulty understanding speech and those having the most trouble understanding natural speech also had the greatest difficulty with synthetic speech.

An 81-year-old female was referred for cochlear implantation due to difficulty communicating in her daily activities despite the use of appropriate amplification. The poorer ear was unable to tolerate amplification for the past 15 years. The open-set sentence-recognition test score in quiet in her "good" ear was 85 percent correct, indicating that the patient was not a traditional cochlear implant candidate. However, the sentence-recognition score in noise at +10 dB signal-to-noise ratio was 0 percent, demonstrating a significant breakdown in the patient's speech understanding in more difficult listening situations. This speech-in-noise score appeared to correlate with the patient's reported communication difficulties as well as with the communicative breakdowns that were observed clinically. The patient underwent cochlear implantation in the better ear. Cochlear implantation in this nontraditional patient provided objective and subjective benefit over hearing aid use. Una mujer de 81 años de edad fue referida para implante coclear debido a sus dificultades para comunicarse en sus actividades diarias, a pesar del uso de amplificación apropiada. El oído peor había sido incapaz de tolerar la amplificación durante los últimos 15 años. El puntaje de la prueba de reconocimiento de palabras de lista abierta en silencio en su oído "bueno" fue de un 85%, indicando que le paciente no era una candidata tradicional para un implante coclear. Sin embargo, los puntajes de reconocimiento de frases en ruido, a una tasa señal/ruido de ±10 dB fueron de 0%, demostrando un colapso en la comprensión del lenguaje por parte de la paciente en situaciones auditivas más difíciles. Este puntaje de lenguaje en ruido pareció correlacionar con las dificultades reportadas por la paciente, así como con el colapso comunicativo que se observó clínicamente. La paciente fue sometida a una implantación coclear en el mejor oído. El implante coclear en esta paciente no tradicional aportó un beneficio objetivo y subjetivo sobre el uso de auxiliares auditivos.
