The failure of top-down categorical approaches for generating solutions to many local problems has led to the adoption of alternate approaches. Many scholars believe that a confluence of local and global forces have generated complex problems, which call for new approaches to problem solving. Previously, the top-down approach relied entirely on the knowledgeable elite. Communities were seen as passive study subjects and information flow was one way only- from knowledgeable elites to the less knowledgeable community agents or community-based organization acting on behalf of communities. The objectives of this study are to provide a review of governance as a means of organizing community action to address community problems in the Black Belt Region (BBR) of the Southeastern United States, and an assessment of community problems in the BBR from the perspectives of community-based organizations (CBOs). Data was collected from CBOs via a telephone survey in eleven Southeastern states and via listening sessions conducted with CBOs in 9 Southeastern states. The study provides valuable insight regarding the challenges faced by these organizations and strategies they employ in adapting to serve their communities.

The paper explores the centrality of community-based education and training in addressing constraints and opportunities for wastewater management in Uganda. To be sustainable, wastewater management need to be conceived in terms of socio-economic incentives, community action, group pressure and social capital. It is assumed that communities could be motivated to undertake sustainable wastewater disposal activities if concrete benefits are demonstrated. The benefits include among others; reducing expenditure on health; improvement of the fisheries sector and the use of wastewater for crop farming. The paper is based on a study that analyzed the role of socio-economic incentives and Public-Private-Partnership (PPP) in sustainable management of wastewater. Data was collected through personal interviews, documents analysis and review of recent studies on wastewater reuse in Uganda. Focus was put on districts of Mukono, Buikwe and Kayunga in central Uganda. Findings reveal that in line with the Hydro-Social-Health cycle, physical, social, political, economic, and cultural factors converge to influence wastewater management. The paper duly describes innovative education and training approaches based on Communal Water Protection Units (COWAPU) facilitated by multidisciplinary Water Professionals and Educators (WAPE).It is concluded that it is possible to operate a complete sanitation system without subsidies.

Abstract Background Parents play a pivotal role in adolescent sexual health and Human Papillomavirus (HPV) vaccination. Nurses are on the frontlines of healthcare and play a critical role in promoting HPV vaccination and parent-child sexual health communication. We enhanced the Families Talking Together (FTT) parent-based sexual health curriculum to include adolescent vaccinations herein, FTT + HPV, and trained student nurses to provide a strong HPV vaccination and parent-child sexual health communication endorsement. Methods Using a randomized attention-controlled trial design, we examined the efficacy of FTT + HPV among 519 parents and their 11–14 year old youth recruited from medically underserved communities between 2015 and 2018. Participants were recruited from 22 after-school programs (e.g., Boys and Girls Clubs) and 19 charter schools. For parents, we examined protective factors including parent-child sexual health communication and parental involvement. For youth, we examined sexual health knowledge, parent-child sexual health communication, and parent-child connectedness. To assess HPV vaccination initiation and completion, we searched IMMTRAC immunization registry records for 85% of youth and used parental report for youth without registry records. Group differences were calculated using the estimated mean difference at one- and six months post-intervention with significance set at the p < 0.05 level. Results Baseline rates of HPV vaccination were low at 55.7%. No significant difference between the groups was seen in vaccination initiation or completion rates by one-month post-intervention. However, by six-months post intervention, there was a significant difference between the groups with 70.3% of the intervention group initiating the HPV vaccination series vs. 60.6% for the control group (p = 0.02). No difference between the groups was found for HPV series completion at six-months. There were significant differences in condom knowledge (p = 0.04), parent-child connectedness (p = 0.04), and communication frequency (p = 0.001) with greater improvement in the intervention vs. the control group. Rates of sexual activity remained low in both groups throughout the six-month follow-up period. Conclusion A brief parent-based adolescent sexual health and HPV vaccination intervention delivered by student nurses can improve sexual health outcomes including protective parental factors, adolescent sexual health knowledge, and HPV vaccination initiation rates. This exploratory study examines the role community-based organizations have in bridging low-income students of color to postsecondary institutions. Data came from interviews with organization staff, high school students, and college students associated with three distinct community-based organizations located in a mid-size city. The findings suggest that organization staff are well-positioned in youth, academic, and community social networks. Staff become social brokers across these networks through three steps: cultivating authentic and safe relationships, lessons from students, and becoming advocates. Community-based organization staff strategically advocate for underrepresented student college enrollment and admissions by serving as social brokers between students, schools, and their communities.
