Do police experience trauma during routine work? An analysis of work-related potentially traumatic events and expressed trauma in a sample of Ghanaian police officers

Abstract Background We investigated work-related exposure to stressful and traumatic events in police officers, including repeated exposure to traumatic materials, and predicted that ICD-11 complex PTSD (CPTSD) would be more prevalent than posttraumatic stress disorder (PTSD). The effects of demographic variables on exposure and PTSD were examined, along with whether specific types of exposure were uniquely associated with PTSD or CPTSD. Methods An online survey covering issues about trauma management, wellbeing and working conditions was disseminated via social media and official policing channels throughout the UK. In total, 10 401 serving police officers self-identified as having been exposed to traumatic events. Measurement of PTSD and CPTSD utilised the International Trauma Questionnaire. Results The prevalence of PTSD was 8.0% and of CPTSD was 12.6%. All exposures were associated with PTSD and CPTSD in bivariate analyses. Logistic regression indicated that both disorders were more common in male officers, and were associated independently with frequent exposure to traumatic incidents and traumatic visual material, and with exposure to humiliating behaviours and sexual harassment, but not to verbal abuse, threats or physical violence. Compared to PTSD, CPTSD was associated with exposure to humiliating behaviours and sexual harassment, and also with lower rank and more years of service. Conclusions CPTSD was more common than PTSD in police officers, and the data supported a cumulative burden model of CPTSD. The inclusion in DSM-5 Criterion A of work-related exposure to traumatic materials was validated for the first time. Levels of PTSD and CPTSD mandate enhanced occupational mental health services.

Abstract Introduction National reports of work-related injuries found the excess risk of work injury attributed to shift work to be significantly higher among women. The Working Time Society (WTS) concluded that male sex is one of the few factors that is “consistently associated with perceived or actual shift work tolerance”. However, it is unclear if physiological parameters are involved. Laboratory-controlled studies report sex differences in circadian rhythms (body temperature, melatonin). In sleep deprivation protocols, alertness and cognitive performances were affected by sex, menstrual cycle phase and hormonal contraceptives [HC] use. Nevertheless, field studies that compare male and female shift workers are scarce. Methods An observational study including 76 police officers working on patrol: 56 males and 20 females (11 using [HC], 6 not using [non-HC] and 3 with unknown use of hormonal contraception) aged 32.0 ± 5.3 years. Participants were followed throughout a month-long work cycle (1,457 morning, evening, night, or other shifts, plus rest days). They filled out time-stamped questionnaires (Samn-Perelli, KSS, Visual Analogue Scales, ~5/day; sleep and work-related information, ~1–2/day), completed 5-min Psychomotor Vigilance Tasks (PVT, ~2/day), and wore an actigraph to collect activity data. Linear mixed-effects models were used to analyze the effects of group, time awake and time-of-day on fatigue, sleepiness, alertness, mood and PVT measures. Results Self-reported measures and psychomotor performance significantly varied with time awake and time-of-day. Fatigue and sleepiness levels were significantly higher among female compared to male police officers, both with time awake and across the 24-h day. These variations were similar between non-HC females and the other groups. Compared to males, HC females were more fatigued and less alert, both with time awake and across the 24-h day, and sleepier with time awake. Having children at home did not explain these differences. Conclusion The results of this study expand our knowledge on the sex differences in the sleep and circadian physiology and demonstrate a critical effect of HC on women fatigue, sleepiness and alertness when working shifts. Sex and hormonal parameters must be considered in occupational medicine as well as in future laboratory and field studies on shift workers and circadian rhythms. Support (if any) IRSST, FRQS.

Child welfare workers often experience work-related traumatic events and may be at risk of post-traumatic stress disorder (PTSD), which can hinder early interventions for child abuse. This study examined the association between each single work-related traumatic event experienced by child welfare workers and the cumulative number of traumatic event types with PTSD symptoms. A checklist of traumatic events was used to investigate work-related traumatic events. The PTSD checklist for DSM-5 (PCL-5) was used to screen for PTSD symptoms. Two multivariate analyses were performed. A total of 140 workers were included in the analyses. In the first multivariate analysis, the event, “Witnessed a parent violently beating, hitting, kicking, or otherwise injuring a child or the other parent during work” (β = 11.96; 95% CI, 2.11–21.80; p < 0.05) and resilience (β = −0.60; 95% CI, −0.84 to −0.36; p < 0.01) were significantly associated with PTSD symptoms, as was resilience in the second multivariate analysis (β = −0.60; 95%CI, −0.84 to −0.36; p < 0.01). The association between the cumulative number of event types and PTSD symptoms was not significant, but it was stronger when the cumulative number was four or more. The findings suggest the importance of reducing child welfare worker exposure to traumatic events.

Background: Despite the prevalence of obesity and its increases in associated diseases such as diabetes and cardiovascular disease, no studies have addressed the spread of obesity among police officers working in stressful environment. Therefore, the present study conducted to access the prevalence of work related stress in police personnel.  Aims and Objective: To access the prevalence of obesity and stress burden among police workers and to see association of stress burden of police workers with their obesity.   Material and Method: This cross-sectional observational study done on 245 police workers in PAC, Sitapur, India. Subjects having more than ten years of working experience were included in study. Anthropometric measurements of subject recorded by following standard protocol. Standardized stress scale was used to access the degree of stress in police workers. Available data analyze and expressed in percentage, mean with SD and chi square test to see the significance of association.   Results: According to result of body mass index 77.14% of subjects were obese and 9.8% of subject was overweight. According to waist circumference 82.04% of subjects were obese. Result of stress score showed that 31.84% of subjects having mild stress and 16.73% of subjects having moderate stress. 30.35% of subjects having abdominal obesity were suffered from mild stress and there association was highly significant (p=0.02).   Conclusion: The study recommends health education programs to combat obesity and overweight. Regular counseling should be conducted to alleviate work related stress.
