The hazard of chemical process equipment consists of two parts: the inherent hazard of process equipment and the hazard from domino effect among equipments. The inherent hazard of equipment depends on the properties of the substance present in the equipment and the specific process conditions. The domino effect is responsibility for many most destructive accidents in the chemical process industry. However, domino effect is either not considered at all or is done with much less rigour than is warranted. A method was proposed to evaluate the hazard of chemical process equipment. The inherent hazard and the hazard from domino effect were considered in the method. The procedure for the domino effect analysis among equipments was presented to evaluate the hazard from the domino effect. The method was implemented in a case study. The results show that it can be used to select the process equipment which should be intensive monitored.

In today’s competitive market place, manufacturing companies must apply continuous process improvement in order to maintain a returning customer base. One way of achieving constant process improvement is through value stream mapping. Value stream mapping is used to visualize the current processes for easier understanding and problem identification. A well-defined problem statement will ensure a successful outcome of a project improvement process. This research provides a case study performed on a rope manufacturing process. A current state value stream map is created, and the possible improvements are suggested. The implemented results are shown in the form of future state map. The results show that, after waste elimination and structural revision, a manufacturing process becomes more efficient, enabling the customer to receive an order significantly faster.

Accidents caused by the domino effect are the most destructive accidents in the chemical process industry. These chains of accidents may lead to catastrophic consequences and may affect not only the industrial sites, but also people, environment and economy. However, quantitative risk assessments do not usually take the domino effect into account in a detailed, systematic way, mostly because of its complexity and the difficulties involved in its incorporation. A method for quantitative assessment of domino effects is presented. The consequence and probability of a certain accident can be estimated. The domino sequences from the initial accident to the last accident can be obtained. The method has been implemented in a case study. The results show that it can indeed be used to estimate the impact of the domino effect in quantitative assessment.

This research was conducted in manufacturing industry, so this research is based on case study application. This research serves to reduce waste in the industry when making a product. This study categorizes value-added work and which work has no added value. And it is measurable and has value, so it can be evaluated in the future. Later this will be poured or depicted on a map called Value stream mapping. This is a tool from Lean Manufacturing. Lean manufacturing is useful for analysing and reducing non value-added activities, value stream mapping analysis tools, 5L1H process mapping activities, and 5 why tools. From the results of this study obtained the efficiency of the process cycle and total estimation of the improvement of the lead time. This calculation can be an evaluation material for the company.

Recent accident analyses show that the accident rates in the chemical process industry (CPI) were still not decreasing. In the paper, the issues and challenges of risk reduction strategies were discussed. A case study using accident cases, extracted from the U.S. Chemical Safety and Hazard Investigation Board (CSB) database was conducted. The results of the accident analysis were discussed and then compared to previous analysis of the Failure Knowledge Database (FKD, Japan). The CSB analysis shows that the industry had moved from procedural risk reduction strategies to inherently safer and add-on engineered strategies. After 10years improvement, the industry was approaching towards more balanced safety management to prevent occurrence and recurrence of accidents with the emphasis on both management and engineering risk reduction strategies.
