Effect of Lifestyle Intervention on Cardiometabolic Risk Factors in Overweight and Obese Women with Polycystic Ovary Syndrome: A Systematic Review and Meta-Analysis

Abstract Background: The polycystic ovary syndrome (PCOS) was associated with a higher risk of cardiovascular diseases, related to metabolic dysfunction, due to its peculiar hormonal pattern, dyslipidemia, and inflammatory state. This study was aimed to investigate the effects of thylakoid-rich spinach extract supplementation combined with a hypocaloric diet on cardiometabolic risk factors in obese women with PCOS. Methods: In a double-blind, placebo-controlled design, forty-eight participants (age 20–45 years) consumed in random order either a thylakoid-rich spinach extract powder (5g) or a matched placebo (5g starch), given daily combined with the hypocaloric diet for 12 weeks. The samples were collected at baseline and after the end of the supplementation. The primary outcome was the measure of anthropometric changes, glycemic indices, and lipid profile. The secondary outcomes included inflammatory status and blood pressure. Results: Thylakoid-rich spinach extract supplementation combined with the restricted-calorie diet was associated with significant reductions in abdominal obesity indicators, insulin resistance, lipid accumulation product (LAP), and diastolic blood pressure (DBP) (P< 0.05). Significant decreases were also seen in serum triglycerides (TG), insulin, high-sensitivity C-reactive protein (hs-CRP), and total testosterone compared with the placebo group (P< 0.05). However, no significant differences in systolic blood pressure (SBP) and in serum levels of total cholesterol, low-density lipoprotein cholesterol (LDL-C), high-density lipoprotein-cholesterol (HDL-C), and fasting blood glucose were evident between groups at the end of study ( P>0.05). Conclusions: Thylakoid-rich spinach extract supplementation combined with the calorie restriction for 12 weeks may improve the cardiometabolic risk factors in obese women with PCOS.Trial registration: The study was approved by the Ethics Committee of Research Vice-chancellor of Tabriz University of Medical Sciences, Tabriz, Iran, and was registered in the Iranian Registry of Clinical Trials (IRCT20140907019082N9).

Abstract Context Adolescents with polycystic ovary syndrome (PCOS) have increased incidence of cardiometabolic risk factors including dyslipidemia. Atherogenic apolipoprotein (apo) B-lipoprotein remnants are associated with increased cardiovascular disease (CVD) risk. Objective The aim of this study was to determine the concentrations of fasting plasma apoB-lipoprotein remnants, apoB48 and apoB100, and their association with cardiometabolic risk factors and androgen indices in adolescent girls with and without PCOS. Design, setting and participants Participants (n = 184) aged 17 years were recruited in the Menstruation in Teenagers Study from the Western Australian Pregnancy Cohort (Raine) Study. The main outcome measures Fasting plasma apo-B48 and -B100 lipoprotein remnant concentrations in adolescent girls with and without PCOS. Results Fasting plasma apoB48-lipoprotein remnants but not apoB100-lipoprotein remnants were elevated in adolescent girls with increased cardiometabolic risk compared with those with lower cardiometabolic risk (13.91 ± 5.06 vs 12.09 ± 4.47 µg/mL, P &lt; .01). ApoB48-lipoprotein remnants were positively correlated with fasting plasma triglycerides (b = .43, P &lt; .0001). The prevalence of increased cardiometabolic risk factors was 2-fold higher in those diagnosed with PCOS (35.3%) than in those without PCOS (16.3%).Conclusion: Adolescents with PCOS have a 2-fold higher incidence of cardiometabolic risk factors than those without PCOS. Fasting apoB48-lipoprotein remnants are elevated in adolescent girls with a high prevalence of cardiometabolic risk factors.

Aims: We aimed to investigate the impact of following a diet to induce weight loss (500 kcal deficit per day) over DNA damage and cardiometabolic risk factors in women with overweight/obesity diagnosed with polycystic ovary syndrome (PCOS). Methods: A study was conducted in Natal, RN, Brazil selecting overweight/obese (body mass index ≥25 and <39 kg/m2) women (18-35 years). The levels of DNA damage were assessed by a single cell gel electrophoresis. Repeated 24 h dietary recall questionnaires, anthropometry, biochemical profile and sex hormones were collected at baseline and after 12 weeks of intervention. Results: Women exhibiting a decrease in the markers of DNA damage: tail intensity (24.35 ± 5.86 - pre diet vs. 17.15 ± 5.04 - post-diet; p < 0.001) and tail moment (20.47 ± 7.85 - pre diet vs. 14.13 ± 6.29 - post-diet; p < 0.002). Reduction of calorie intake, weight loss, decreased sexual hormone and cardiometabolic markers such as insulin, homeostasis model assessment of insulin resistance and low-density lipoprotein cholesterol were verified In the multivariate regression analysis, quantitative insulin sensitivity check index and progesterone were responsible for the variation markers in DNA damage before the diet, losing its influence upon diet. Conclusion: DNA damage and the impact of cardiometabolic risk factors decreased after the intervention in women with PCOS, indicating the relevance of a nutritional approach in this group of patients.
