Abstract Background We report an adult patient with features of VACTERL - oesophageal and anal atresia, with cardiac malformation. In infancy he underwent colonic interposition for oesophageal atresia using the transverse colon as a conduit. As a teenager he underwent formation of a Koch continent ileostomy after previous stoma formation for anal atresia. Eventual failure of the Koch pouch nipple valve necessitated a laparotomy for attempted refashioning. Unfortunately, this operation was beset by life-threatening complications and a protracted post-operative course. He presented to our hospital with a laparostomy, high output enterocutaneous fistulae (ECF) necessitating home total parenteral nutrition (TPN), severe cachexia and deconditioning. Method Conservative therapy was utilised to improve the nutritional and physiological status of the patient. Simultaneously, attempts were made to define the anatomy of his abdomen in relation to previous surgery. However, operation notes for the original procedures (oesophageal reconstruction and Koch pouch formation) were lost due to closure of the hospital which he attended at the time. A laparotomy was subsequently performed to manage the ECFs. Most of the small intestine was unsalvageable due to multiple fistulae and adhesions, leaving 35cm of small bowel terminating in an end ileostomy. A cholecystectomy was performed to mitigate gallstone formation. Conclusions Efficient communication is imperative in the management of complex patients. The loss of original operation notes made interpretation of subsequent imaging and planning of surgery troublesome. In contrast, efficient communication between teams involved in his peri-operative care, particularly the respiratory and intensive care teams facilitated a smooth post-operative course and successful discharge from hospital.

Acute cholecystitis is a major health problem. There are multiple etiologies to be considered and early recognition of the condition is important to optimize management and outcome. We report the first case in the medical literature of symptomatic acute cholecystitis triggered by ceftriaxone-associated gallbladder sludge formation and, importantly, solid ceftriaxone gallstone formation in an adult patient with underlying mineral and pigment cholecystolithiasis, necessitating cholecystectomy. This case serves as a reminder for physicians to keep this uncommon cause of cholecystolithiasis and cholecystitis in mind in patients who receive prolonged ceftriaxone therapy. These patients should be cautioned to promptly report to their physicians any signs or symptoms of cholecystitis in order to ensure timely and appropriate evaluation.

e11596 Background: We want determine the effect of early discharge from hospital after surgery for breast cancer on physical and psychological illness. Moreover the study aimed to evaluate the management of post operative symptoms and post operative care. Methods: From January 2006 to July 2008, all breast cancer patients undergoing conservative breast cancer surgery or total mastectomy were offered surgery as an outpatient at One- Day Surgery Unit. Results: Three hundred seventy five patients underwent outpatient surgery for a total of four hundred four surgical procedures. The age of 375 patients varied from 14 years old to 94 years old, mean age was 55; in 79 of 404 (19.5%) surgical procedures patient remained in the hospital, for one night, after a surgical procedure regarding total axillary resection. Patients characteristics and post-operative complications were recorded for all 375 patients and 404 surgical procedures. All complications were treated by medical or nursing care. Complications included axillary seroma in nine patients (2.2 %), abscess in three patients (0.9%), pain in two patient (0.5%), hematoma formation in five patients (1.2%) for a total of 17 complications (4.2%). In our series, no hospital readmission was recorded during the first night and the first week and no death occurred. Conclusions: One day surgery for breast cancer patients is a totally safe and feasible procedure. We haven’t an increased rates of physical or psychological illness from early discharge after surgery for breast cancer. This policy can be recommended for patients with support at home. No significant financial relationships to disclose.

Background: Anal atresia is a congenital defect and is related to the flaw of an opening in the membrane that separates the endoderm of the posterior intestine from the ectodermic anal membrane. This anomaly can cause debilitating disease, death, or abnormal development of the animal. Clinical presentation of urethrorectal fistula in domestic animals differs between males and females, as in the former there is passage of the urine through the anus, whereas in females there is elimination of the feces through the vulva. The absence of description of this disease in horses stimulated describing a case of surgical resolution of anal atresia, rectal bulb agenesis, and pneumovagina of an adult mare.Case: Mare with a history of defecating through the vagina. In the anamnesis, it was determined that the owner noticed the problem after observing the animal during a walk. Clinical examination verified absence of the anus with stool evacuation through the vagina, pneumovagina, and signs of cystitis, as the animal frequently urinated small volumes. Complementary laboratory tests of blood and urine were carried out for the diagnosis of cystitis, serum biochemistry, hemogram, and urinalysis being evaluated. The operation was scheduled for eight days after clinical tests and sodium ceftiofur was prescribed, at a dose of 4.4 mg/kg, administered intramuscularly for seven days. On the date chosen for surgical intervention, the animal was fasted for the previous 12 h; procedures began with the containment of the animal in a stock, followed by pre-anesthesia with detomidine, at a dose of 20 µg/kg, administered intravenously in solution at 1%. Anesthesia was a low peridural with 2% lidocaine at a total dose of 15 mL, to be able to work in the quadruped position. After containment of the tail and taking antiseptic and aseptic care, sutures were made in the skin and vaginal mucous, as it exhibited a solution of continuity. In the mucous, suture consisted of separate simple stitches made with perforation needle and 2-0 polyglactin 910 suture, while in the skin Wolff stitches were applied using a blade needle and 2-0 nylon suture. Then, a circular incision was made in the skin in the anus region to remove local skin; this revealed the rectum which, being it far from the required location, was pulled to the new opening. The rectum was fixed to the skin by suture: four simple stiches, with a perforation needle and zero nylon suture, to serve as repairs, followed by simple stitches between the repairs, with the same type of suture. Regarding the pneumovagina, reconstruction of the perineum and application of modified Calisck surgical technique were performed to correct the vulva defect. In the post-operative phase, 4.4 mg/kg of sodium ceftiofur was administered intramuscularly for seven days, and Flunixin Meglumine, at a dose of 1.1 mg/ml for every 50 kg of body weight, was administered intravenously for four days. Furthermore, rectal enemas (3 L of warm aqueous solution associated with glycerin) were administered every eight hours for 14 consecutive days, and the wound was cleaned with 2% chlorhexidine degerming solution. The skin sutures were removed after 15 days and the animal was discharged.Discussion: Alteration of the digestive tract in horses presents nuances in genetic dependence, embryology, and congenital defects, among others. Regarding the possibility of surgical correction, it is necessary to observe the characteristics of the lesion, the handling to which the animal is subjected, care of the surgical wound, and physical conditions where surgical procedures are carried out. The results obtained with this intervention and pre-, trans-, and post-operative care were effective in reestablishing the patient.

Resumo: Este estudo teve como objetivo conhecer a percepção do cuidador do idoso submetido à cirurgia ortopédica como opção de tratamento para fratura de fêmur, sobre o trauma, os cuidados no pós-operatório e prevenção. A pesquisa foi realizada no Instituto de Ortopedia e Traumatologia do Hospital das Clínicas da Faculdade de Medicina da Universidade de São Paulo, a coleta de dados se deu por meio da realização de entrevistas, tendo participado do estudo 15 cuidadores. Em 86 % dos casos, a fratura fora ocasionada por queda, tendo ocorrido predominantemente em ambiente domiciliar. Quanto aos cuidados pós-operatórios, 60% dos cuidadores não citaram qualquer cuidado especial referente ao vestuário, enquanto apenas 20% referiram a deambulação sem carga. Quanto à prevenção das fraturas em idosos, apenas 26,66% dos cuidadores citaram como medidas, ações simples como a retirada de tapetes e 13,33% deles acreditam que não há como prevenir as quedas e fraturas em idosos. Diante dos resultados, conclui-se que há a necessidade de repensar a assistência de enfermagem quanto ao planejamento da alta hospitalar. Palavras-chave: Idoso. Cuidador. Fratura de Fêmur. Abstract: This study had the objective of knowing the elder´s caregiver´s perception who was submited by an orthopedic surgery as a treatment option for femur fracture, about the trauma, the post-operative care and prevention. The research was realized at the Orthopedic and Traumatologic Institute of São Paulo`s University Clinics Hospital, the datas were collected by interviews, 15 caregivers participated of the study, 86% of the cases of fractures were caused by falls that happened in their majority in the elder´s home. About the post-operative care, 60% of the caregivers didn´t mentioned any special care relating to dress while only 20% mentioned the locomotion without weight. About the prevention of the fracture in elders, only 26,66%of the caregivers mentioned simple actions as take out the carpets and 13,33% of them believe that there is no way to prevent falls and fractures in elders. Then, we conclude that it´s vital that we think more carefully about the nursing care relating to the planning of the discharge from hospital. Keywords: Elder. Caregiver. Femur Fracture.

Fecal incontinence after surgeries for anal atresia is not rare. One of the possible treatment alternatives – adynamic graciloplasty. We present a case of an adult patient to whom adynamic gracyloplasty was performed for fecal incontinence after single attempt of sphincter repair in childhood. There was a symp­tomatic improvement postoperatively, as confirmed by Wexner and FISI index. Adynamic graciloplasty could be more often used as a treatment option of fecal incontinence in adult patients with history of anal atresia.

Approximately 1 million people in the United States alone develop gallstones each year. The incidence is higher in women than in men and the ratio being 4 ≥ 1. A correlation has also been suggested between oral contraceptives and cholelithiasis. In addition, postmenopausal or cancer estrogen therapy has been reported to be a factor responsible for gallstone formation. Female sex hormone receptors have been detected not only in the gallbladder musculature, but also in its epithelium. As a follow up to experiments effectuated in the male and the ovariectomized Syrian hamster, this report shows that, a combination of a low cholesterol diet with female sex steroid treatment contributes to the formation of gallstone-like deposits, while modifying the surface epithelium morphology. Syrian hamsters (F1B strain, BioBreeders, Watertown MA) were housed under 12h light: 12 h dark cycle, at 20 °C, fed Purina chow and water ad libitum. Several duration/treatment groups were studied, but this report will focus on data obtained with the group injected weekly with estradiol valerate (E weekly, s.c. 8-10 μg/100 g.b.w., in corn oil) and with i.m. medroxyprogesterone acetate (DepoProvera Upjohn Co., Kalamazoo, MI; 8-10 mg/100 g.b.w.) for a 3-month period. Other parameters (blood and bile) were also studied but not reported here.
