Purpose The aim of our study was to investigate potential associations between sleep insufficiency and incident CVD. Methods In this cross-sectional study, 957 participants, (mean age 49.62 ± 14.79) from the region of Thrace, Greece were enrolled and classified into three groups [short (<6 h), normal (6-8 h) and long (>8 h) sleep duration]. CVD was assessed by a positive response to the following questions: "Have you been told by a doctor that you have had a heart attack or angina (chest pain or exertion that is relieved by medication)?" or "Have you been told by a doctor that you have had a stroke?". Participants’ sleep quality was estimated with the Epworth Sleepiness Scale, Athens Insomnia Scale, Pittsburgh Sleep Quality Index, and Berlin Questionnaire. Results The overall prevalence of CVD was 9.5%. The population with CVD exhibited reduced sleep duration and efficiency reduced by 33 min and 10%, respectively. After adjusting for all possible cofounders, short sleep duration was 3.07-times more frequent in patients with CVD and sleep duration of less than 5:33 hours could be a potential risk factor for CVD, especially among females. Additionally, CVD was significantly associated with excessive increased daytime sleepiness, insomnia, poor sleep quality and increased risk of obstructive sleep apnea. Conclusion(s): Our study depicts a strong correlation of sleep insufficiency with CVD and promotes early pharmacological or cognitive behavioral interventions in order to protect cardiovascular health. Short sleep duration or poor sleep quality has been associated with an increased risk of obesity. Although the underlying mechanism remains unclear, one proposed pathway is poor diet quality. This cross-sectional study investigated whether diet quality modifies the association between sleep status and obesity in Korean adults. We used the baseline data and samples of 737 men and 428 women (n = 1165) aged 19–64, who participated in the prospective Ewha–Boramae cohort study. Sleep duration was dichotomized into ≥7 h (adequate) and <7 h (insufficient). Pittsburgh Sleep Quality Index (PSQI) values, reflecting sleep quality, were dichotomized into >5 (poor quality) and ≤5 (good quality). Diet quality was evaluated by the Recommended Food Score (RFS). Obesity was associated with higher rates of insufficient sleep and poor sleep quality in women, but not in men. After adjustment for covariates, women with poor sleep quality had a higher risk of obesity than women with good sleep quality (OR = 2.198; 95% CI = 1.027–4.704); this association occurred only in the group with RFS ≤ median score. Our findings support a significant association between sleep quality and obesity, and this association has been potentially modified by dietary quality in women.

Objective: To determine the prevalence of sleep quality in the elderly within our outpatient department.Material and Method: A cross-sectional study was conducted by; calculating the sample size for a survey with ‘proportion to size’ at the outpatient department of Songklanagarind Hospital from; October to November, 2015. A total of 408 participants aged between 65 and 99 were interviewed. Demographic and sleep-related data were collected. Sleep quality was assessed using the Pittsburgh Sleep Quality Index (PSQI) – Thai version. The results were analyzed in terms of descriptive statistics and conducted using R-Epicalc and R-Survey software.Results: Our subjects were predominantly married females. The proportion of those in the 65-74 age range was 63.0%.Most of the participants (70.8%) were poor sleepers, with mean and median PSQI scores of 7.7 and 7.0, respectively.Regarding poor sleepers, their problems were; short sleep duration (99.7%), poor sleep efficiency (61.6%) and daytime dysfunction (51.6%).Conclusion: Similarly to previous studies, our findings showed a high prevalence of poor sleep quality in the elderlypopulation. A sleep duration of less than 6 hours was the major problem in the poor sleeper group. Gaining a betterinsight into sleep quality correlating with comorbidities is recommended.

An association between physical illness and sleep has been suggested. Disordered chewing might be a physical factor that is associated with sleep issues. This cross-sectional study aimed to determine whether chewing problems are associated with sleep in Japanese adults. Sleep and chewing issues were evaluated in 6,025 community residents using a self-reported questionnaire. The prevalence of poor sleep quality and sleeping for <6 h/day (short duration) were 15.6% and 29.4%, respectively. Multivariate logistic regression analyses showed that prevalence of poor sleep quality was significantly associated with self-reported medical history (odds ratio (OR), 1.30; p<0.001), self-reported symptoms (OR, 4.59; p<0.001), chewing problems (OR, 1.65; p<0.001), and poor glycemic control (OR, 1.43; p=0.035). The prevalence of short sleep duration was also significantly associated with female sex (OR, 1.23; p=0.001), self-reported symptoms (OR, 1.60; p<0.001), chewing problems (OR, 1.30; p=0.001), and being overweight (OR, 1.41; p<0.001). In conclusion, chewing problems were associated with poor sleep quality and short sleep duration among Japanese adults.

Background Migraine is associated with sleep disturbances in men and non-pregnant women. However, relatively little is known about sleep disturbances among pregnant migraineurs. We investigated sleep disturbances among pregnant women with and without history of migraine. Methods This cross-sectional study was conducted among 1324 women who were recruited during early pregnancy. Migraine diagnoses were based on the International Classification of Headache Disorders-II criteria. The Pittsburgh Sleep Quality Index (PSQI) questionnaire was used to evaluate sleep-related characteristics including sleep duration, sleep quality, excessive daytime sleepiness, and other sleep traits. Multivariable logistic regression procedures were used to estimate adjusted odds ratios (AORs) and 95% confidence intervals (CIs). Results Migraineurs were more likely than non-migraineurs to report short sleep duration (<6.5 hours) (AOR = 1.47, 95% CI 1.07–2.02), poor sleep quality (PSQI>5) (AOR = 1.73, 95% CI 1.35–2.23), and daytime dysfunction due to sleepiness (AOR = 1.51, 95% CI 1.12–2.02). Migraineurs were also more likely than non-migraineurs to report taking sleep medication during pregnancy (AOR = 1.71, 95% CI 1.20–2.42). Associations were generally similar for migraine with or without aura. The odds of sleep disturbances were particularly elevated among pre-pregnancy overweight migraineurs. Conclusion Migraine headache and sleep disturbances are common comorbid conditions among pregnant women.

Abstract Introduction Clinicians have faced unprecedented challenges in care delivery during the COVID-19 pandemic due to increases in patient volume/acuity, alongside fears of COVID-19 exposure. Increased burnout rates are associated with chronic health condition risk and adverse organizational outcomes. It remains unclear whether sleep is associated to burnout in clinicians treating COVID-19 patients. Methods A cross-sectional electronic survey was distributed via email across 3 hospital listserves from September to November, 2020. Clinicians delivering direct care to COVID-19 patients were eligible. Clinician burnout was measured using a single item from AHRQ’s Mini-Z survey. We assessed sleep using the Pittsburgh Sleep Quality Index (PSQI). Binary logistic regressions were used to determine the relationship between PSQI global score (global sleep quality) and burnout, controlling for age, race/ethnicity, gender, length of time employed, whether clinical role changed during COVID-19, and anxiety. In a separate model, we investigated the association between burnout and independent PSQI subcomponents: 1) sleep duration (“Hours of sleep per night”), and 2) subjective sleep quality (“How would you rate your sleep quality overall”) entered together, with the above covariates. Results The final sample included 315 clinicians, predominantly nurses (57% White, 15% Hispanic/Latino, 89% female). Burnout symptoms were reported by 61.6%, and poor global sleep quality (PSQI global score &gt;5) in 84.4% of participants. Poor global sleep quality (PSQI global score &gt;5 vs. ≤5) was significantly associated with the presence of burnout symptoms (OR: 2.52, 95% CI: 1.20–5.28, p=0.015). In the secondary model, self-reported sleep quality (rating of fairly or very bad vs. rating of fairly or very good) was significantly associated with burnout (OR: 4.13, 95% CI: 2.33–7.32, p&lt;0.05), whereas short sleep duration (&lt;6 h vs. ≥6 h) was not (OR: 0.726, 95% CI: 0.41–1.30, p=0.28). Conclusion Poor sleep quality is common and associated with increased burnout in clinicians delivering care to COVID-19 patients. Interestingly, sleep quality appears to be more strongly related to burnout than sleep duration. Increased evidence about the negative implications of poor sleep and burnout are emerging. Interdisciplinary efforts aimed at promoting effective sleep quality in clinicians during this pandemic may lead to improvements in long-term clinician physical and psychological health. Abstract: Introduction: Short sleep duration and poor sleep quality may be associated with weight gain; this association has not yet been studied in Roma (Gipsy) population. Aim: Our aim was to study sleep patterns in two adult Roma subgroups (the wealthy Gabor and the poor Lovari Roma), compared to the majority of Hungarian population, in relation to obesity, knowing that Roma population has specific socio-cultural characteristics, with a rapidly changing lifestyle. Method: A population-based cross-sectional survey was conducted in a rural region in Transylvania, where the above groups are cohabiting. The groups were age- and gender-matched. Results: Sleep duration was 7.18 ± 1.6 hours in the Gabor Roma, 7.67 ± 1.5 hours in the Lovari Roma and 7.37 ± 1.5 hours in the non-Roma group. In average, 70% of them had enough sleep (≥7 hours). 38.6% of Gabor Roma, 27.1% of Lovari Roma and 23.5% of non-Roma had poor-quality sleep (p = 0.05). Gabor Roma had significantly higher body mass index (31.1 ± 4.6 versus 27.4 ± 5.2 and 28.66 ± 5.7 kg/m2, p = 0.004), and this correlated inversely with sleep duration (F = 14.85, p<0.000). Conclusion: Gabor Roma had significantly higher percentage of poor-quality sleep. Sleep duration and sleep quality were linked with obesity, mainly in the Roma population. Background: Previous studies have demonstrated a link between both sleep quantity and quality and an increased risk of CVD, stroke, and diabetes. However only a few population-based studies have information on the association of sleep quantity and quality and subclinical atherosclerosis as measured by coronary artery calcium (CAC) and ankle brachial index (ABI) -- a marker for peripheral arterial disease. Methods: CHAS data were used to investigate cross-sectional associations of short sleep duration and poor sleep quality assessed by the Pittsburgh Sleep Quality Index (PSQI) (see Table Footnote for definition of poor sleep quality and quantity) and prevalence of having CAC (score > 0) or low ABI (<0.99). CAC, assessed by multi-detector computed tomography, was quantified by the Agatston’s method. ABI was calculated as the ratio of Doppler recorded systolic pressures in the lower and upper extremities (ABI >1.4 was excluded). Results: The study sample consists of 1,005 men and 390 women ages 65-84 (mean age 71) in 2007-10; 9% were African American. There are no differences in CAC prevalence among participants with or without short sleep duration (11.2% vs. 11.0%). For low ABI, these figures are 20% and10.8%, respectively. With multiple adjustments (see Table Footnote), there are no significant associations of short sleep duration or poor sleep total score with prevalent CAC. However, participants with short sleep duration are more likely to have low ABI, i.e., the odds (95% confidence interval) of having ABI <0.99 with sleep duration of <6hrs was almost two fold (1.02-3.13) compared to others (see Table). Conclusion: In older age, shorter sleep duration is associated with peripheral vascular disease. Sleep may represent a modifiable risk factor for CVD.

Background: Prior research has shown a J shaped association between parity and increased risk of cardiovascular disease (CVD), and several confounding factors have been identified. Sleep is emerging as an important, potentially modifiable risk factor for CVD, however few data have examined the relationship between parity and sleep. Methods: We studied 50 women in the AHA Go Red for Women Strategically Focused Research Network who provided information on pregnancy history and sleep patterns [56% non-white (n=28), mean age = 41±18 y]. The Pittsburgh Sleep Quality Index was used to assess sleep duration and quality, and the Insomnia Severity Index was used to assess level of insomnia. Parity was assessed using a standardized questionnaire as any pregnancies lasting > 6 mo. We used linear and logistic regression to examine the cross-sectional relation between sleep (duration, quality, and insomnia severity) and parity. Results: Prior pregnancy was reported in 32% (16 of 50) women and 36% (18 of 50) sleep ≤ 6 h/night. Parous women were significantly older (55 vs 34 y), more likely to have a BMI ≥25 kg/m 2 , HTN, and hyperlipidemia compared to nulliparous women. Sleep duration was significantly shorter in parous women (6 h vs 7 h), and the relationship remained significant for primiparous women (1 birth) after adjustment ( Figure ). More than half of parous women reported poor sleep quality (56%, 9 of 16) and insomnia (63%, 10 of 16) compared to 29% of nulliparous women (10 of 34) who reported each sleep disorder. After adjustment for age, there was no difference in sleep quality or insomnia between groups (p>0.05). Conclusion: In a diverse cohort of women, sleep duration was inversely related to parity. Primiparous women may be at heightened risk for the adverse cardiometabolic consequences of inadequate sleep duration, and may represent a teachable moment for CVD prevention. Ongoing studies to examine the relationship between sleep and pregnancy may provide important information on the pathways through which these risk factors modify future CVD.

Abstract Objective: To assess the association between total alcohol intake, specific alcoholic beverages and sleep quality in a community-based cohort. Design: A cross-sectional study. Setting: The Kailuan community, China. Participants: Included were 11 905 participants who were free of a history of CVD, cancer, Parkinson’s disease, dementia and head injury in or prior to 2012. Alcohol consumption (amount and frequency intake) and alcoholic beverage type were collected in 2006 (baseline) and 2012. Participants were grouped into non-, light- (women: 0–0·4 serving/d; men: 0–0·9 serving/d), moderate- (women: 0·5–1·0 serving/d; men: 1·0–2·0 servings/d) and heavy- (women: >1·0 servings/d; men: >2·0 servings/d) drinkers. Overall sleep quality was measured in 2012 and included four sleep parameters (insomnia, daytime sleepiness, sleep duration, snoring/obstructive sleep apnoea). Results: We observed a dose–response association between higher alcohol consumption in 2006 and worse sleep quality in 2012 (Ptrend < 0·001), after adjusting for age, sex, socio-economic status, smoking status, physical activity, obesity, plasma lipid profiles, diabetes and hypertension. A similar association was observed when alcohol consumption in 2012 was used as exposure. Alcohol was associated with higher odds of having short sleep duration (adjusted OR for heavy- v. non-drinkers = 1·31; 95 % CI: 1·09, 1·57) and snoring (adjusted OR for heavy- v. non-drinkers: 1·38; 95 % CI: 1·22, 1·57). Consumption of hard liquor, but not beer or wine, was significantly associated with poor sleep quality. Conclusions: Higher alcohol consumption was associated with poorer sleep quality and higher odds of having snoring and short sleep duration.
