The role of clinical pharmacology is to provide the scientific basis for rational prescribing: ‘patients may recover in spite of drugs … or because of them’ (Gaddum). This sums up the dilemma facing any doctor who prescribes a drug to a patient: it should certainly be the doctor’s explicit intention to do the patient some good, but the drug may actually harm the patient, and on rare occasions it can even kill them....

The review focused on pharmacokinetics of the six major subclasses of flavonoids: their distribution, consumption and metabolism. Particular attention is paid to the problem of the bioavailability of these plant polyphenolic compounds, the differences of the effects obtained in experiments in vitro and in vivo. Discusses the dependence of the efficiency of flavonoids by the characteristics of the processes of metabolism in humans. Key principles of clinical pharmacology inform prescribing in palliative care. Concepts discussed in this chapter include issues surrounding patient compliance, the importance of medicines reconciliation, the use of guidelines and formularies, and the role of unlicensed medications. Differences between efficacy, effectiveness, and cost-effectiveness; the nature of a benefit-to-risk analysis; and the meaning of pharmacokinetic variation and pharmacokinetic–pharmacodynamic relationships are then explored. Drug interactions, including adverse drug reactions, are discussed and finally, the topic of pharmacogenomics is covered. Antimicrobial drugs chosen for their activity against the causative pathogens of otitis media provide effective treatment for acute attacks. Prolonged administration of some of these agents has recently been shown to be of value in the prevention of symptomatic otitis. The role of drug therapy in the management of chronic or recurrent middle ear effusion is unknown at present.
