Psychosocial Treatments for Substance Use Disorders

The most effective psychosocial modalities for treating substance use disorders are cognitive-behavioral interventions, motivational interviewing and motivational enhancement, contingency management, community reinforcement, behavioral couples and family therapies, and 12-step facilitation approaches. The foci of these interventions include substance use behavior, patients’ life contexts, and their social and personal resources. Limited evidence is available for these interventions’ differential effectiveness. Brief interventions are highly effective in the treatment of alcohol use disorders. However, as stand-alone treatments, they are best suited for individuals with mild to moderate alcohol use problems. Therapists who are interpersonally skilled, empathic, and nonconfrontational, and who develop a strong therapeutic alliance, are more effective at helping patients achieve better outcomes.

In conjunction with chapters 16 and 17, which described non-pharmacological approaches to pain management, this chapter reviews the available non-pharmacological approaches to substance use disorders (SUD). Specific subtopics include 12-Step programs and facilitation, motivational enhancement therapy, cognitive behavioral therapy, individual and group psychotherapies, contingency management, community reinforcement, family therapy, relapse management, and the terminal consideration of all therapies, termination (what is the duration of treatment)? Each subtopic is addressed in abstract rather than in comprehensive format, to allow the primary care provider to identify her level of interest and present level of understanding, and to inform further reading. The chapter concludes with a text box providing additional resources.

Our review of the literature indicates that among the most effective treatments for alcohol and illicit drug use disorders are cognitive-behavioral treatments, community reinforcement and contingency management approaches, 12-step facilitation and 12-step treatment, behavioral couples and family treatment, and motivational enhancement interventions. Most of these treatment modalities address not only drinking and/or drug use behavior but also patients’ life contexts, sense of self-efficacy, and coping skills; motivational interventions focus primarily on attempts to enhance individuals’ commitment to behavior change. Consistent with motivational interviewing principles, therapists who are interpersonally skilled, empathic, and less confrontational produce better patient outcomes, probably because they establish better therapeutic alliances with their patients. An effective strategy for many patients may be to provide lower intensity treatment for a longer duration—that is, treatment sessions spread at a lower rate over a longer period to match better the chronic, relapsing nature of many individuals’ substance use disorders. At this point, it seems wise to restrict brief interventions as a stand-alone treatment to patients with mild to moderate disorders. Longer term interventions and treatment in inpatient or residential settings should be reserved for patients with more severe, treatment-resistant substance use disorders, fewer social resources, more concomitant medical/psychiatric disorders, and a desire for longer term and/or residential treatment.

Intentional injuries affect millions of lives worldwide. The authors provide an overview of the epidemiological and preventive evidence on the relationship between substance use disorders (SUD) and intentional injuries. Emphasis is placed on suicide and intimate partner violence, as each area has received substantial research attention in relation to SUD. There is robust epidemiological evidence on the relationship between SUD, notably with alcohol use disorders, and most intentional injuries. Research has focused on the identification of factors that distinguish individuals with alcohol use disorders who are at particularly high risk for intentional injuries. Characterization of those with other drug use disorders who are at risk for engaging in intentional injuries and the role of SUD in intentional injuries has been less extensively investigated. The authors conclude with a discussion of public health approaches to the prevention of intentional injuries among individuals with SUD.

OBJECTIVE: To investigate in a community sample the association of suicide-related cognitions and behaviors ("thoughts of death", "desire for death", "suicidal thoughts", and "suicidal attempts") with the comorbidity of depressive disorders (major depressive episode or dysthymia) and alcohol or substance use disorders. METHOD: The sample was 1464 subjects interviewed in their homes using the Composite International Diagnostic Interview to generate DSM-III-R diagnosis. Descriptive statistics depicted the prevalence of suicide-related cognitions and behaviors by socio-demographic variables and diagnoses considered (major depressive episode, dysthymia, alcohol or substance use disorders). We performed a multivariate logistic regression analysis to estimate the effect of comorbid major depressive episode/dysthymia and alcohol or substance use disorders on each of the suicide-related cognitions and behaviors. RESULTS: The presence of major depressive episode and dysthymia was significantly associated with suicide-related cognitions and behaviors. In the regression models, suicide-related cognitions and behaviors were predicted by major depressive episode (OR = range 2.3-9.2) and dysthymia (OR = range 5.1-32.6), even in the presence of alcohol use disorders (OR = range 2.3-4.0) or alcohol or substance use disorders (OR = range 2.7-2.8). The interaction effect was observed between major depressive episode and alcohol use disorders, as well as between dysthymia and gender. Substance use disorders were excluded from most of the models. CONCLUSION: Presence of major depressive episode and dysthymia influences suicide-related cognitions and behaviors, independently of the presence of alcohol or substance use disorders. However, alcohol use disorders and gender interact with depressive disorders, displaying a differential effect on suicide-related cognitions and behaviors.
