Abstract Aims The aim of this research was to study the appearance of soreness after dry needling (DN) and percutaneous needle electrolysis (PNE) in latent trigger points of the extensor muscle of the posterior forearm region. Material and Methods A pre-test/post-test pilot study was performed, in which 15 volunteers with latent trigger points in the extensor muscles of the posterior forearm region were randomly distributed into a group A who received treatment with DN and a group B who received treatment with PNE. Before and after the intervention, algometry measurements were performed at the site where the trigger point was found, and, after the intervention the patient was asked to complete two visual analog scales (VAS) to evaluate pain during the intervention and a second scale to measure pain once the needle had been removed. In addition, the same scale was performed after 24 hours, 72 hours, and 1 week after the dry needling. The statistical analysis was performed using the SPSS program version 24.0, evaluating the normality of the variables using the Shapiro-Wilk test, as the sample was less than 50 subjects. P values of > 0.05 were considered normal. Results The ANOVA revealed a significant effect for time on the decrease of the VAS scores, both in the groups of dry needling (P < 0.001), as well as in the group of PNE (P < 0.001). In contrast, a group-time interaction was not observed (P = 0.824), ruling out statistically significant differences between the DN group and the PNE group. The Student's t-test showed a significantly decreased pain pressure threshold between the pre and post algometry, both in the group of DN (P < 0.001) and the group of PNE (P < 0.003). However, no statistically significant differences were found between both groups in pre and post algometry (P = 0.457). Conclusion All patients presented post-needling soreness, without significant differences between DN and PNE interventions. The most intense pain was registered minutes after the dry needling, which then decreased without significant differences, reaching 0 after 72 hours, in most cases. Neither of the two techniques caused increased pain in response to pressure in a significantly different manner. No relationship was observed between a greater number of LTRs and post-needling pain. There was a correlation between pain experienced during the intervention and pain 24 hours after the needling. The levels of post-intervention pain secondary to the application of DN and PNE in latent trigger points of the extensor muscles of the posterior forearm region did not present significant differences among study subjects in both intervention groups.

The high prevalence, significant demand for treatment of patients with Chronic Pain, and the general difficulty of establishing an accurate diagnosis, together with psychosocial and behavioral conditions, lead many therapeutic approaches to failure or unexpected outcomes. The objective of the present report was to show the evolution of a clinical case of long-term and highintensity Myofascial Pain, in which the patient had been undergoing treatment with Tricyclic Antidepressants (Nortriptyline Hydrochloride) for two years, using minimally invasive conduct. In order to reach the Diagnostic Hypothesis of Myalgia, the DC/TMD (Diagnosis Criteria for Temporomandibular Disorders) protocol was employed, in which the subdivision of this form of TMD was attained. We found substantial myofascial impairment of chewing and cervical muscles, with referred pain and secondary cephalea. The temporomandibular joints (TMJ) did not exhibit any clinical changes in movement, noise or arthralgia. After locating the myofascial trigger points (TrPs), Dry Needling was incorporated, with manipulation of the oral and cervical musculature (with and without resistance), in addition to electrotherapy with TENS (Transcutaneous Electrical Nerve Stimulation) coupled with heat application. The patient was instructed to perform cervical and masseter stretching exercises twice daily and to carry out sleep and medication prophylaxis. The result was rewarding. After seven visits in six months, with an interval from the sixth to the seventh visit of sixty days (follow-up), the patient was discharged and scheduled to return after a six-month period. Pain episodes continue to occur occasionally, but with low intensity, score 3 on the visual analog scale (VAS), and short duration. The course of treatment was well administered by the patient, without the use of antidepressants and cephalea and myofascial pain, and without the use of interocclusal devices, as shown in this report.

SummaryThe objective of exercise in the treatment of osteoporosis is to improve axial stability through strengthening of back extensor muscles. Therefore, a back extension exercise program specific to one’s musculoskeletal competence and pain can be performed in a sitting position and later advanced to the prone position. When fragility is resolved, back extension is performed against resistance applied to the upper back. A significant reduction in back pain, kyphosis, and risk of falls and an improvement in the level of physical activity have been achieved through the SPEED (Spinal Proprioceptive Extension Exercise Dynamic) program. In addition, the application of a “Posture Training Support” (PTS) using a backpack may decrease kyphosis and pain related not only to compression fractures but also reduce iliocostal friction. Therapeutic exercise should address osteo - porosis-related deformities of axial posture, which can increase risk of fall and fracture. Thus, the role of a therapeutic exercise program is to increase muscle strength safely, decrease immobility-related complications, and prevent fall and fracture. As with pharmacotherapy, therapeutic exercises are individualized.

There are many reasons for postural disorder development, but dysfunction of the cervical spine muscles is the most disputable reason. The purpose of the study is to reveal the influence of latent myogenic trigger zones of the cervical muscles on postural resistance. Materials and Methods. The authors used the method of computer posturography. Latent myogenic trigger zones (LMTZs) were determined by an indurated nodule or bundle in the neck muscle and by hyperesthesia in this zona. Assessment of postural resistance in subjects with LMTZs was carried out using standard posturography tests and Romberg test. Both classical and vector indicators were evaluated in a standard sample. Results. In fact, only vector indicators changed significantly in subjects with multiple LMTZs (4 or more). Thus, a decrease in the equilibrium function quality was established, as well as an increase in the average linear velocity of the pressure center and an increase in the normalized vectorogram area if compared with the indices in the control group and the group with sporadic LMTZs (from 1 to 3). The obtained results indicate a postural control decrease in individuals with multiple LMTZs. Subjects with sporadic LMTZs demonstrated a significant decrease in classical and a number of vector indicators, which shows the improvement in postural resistance in this test group. In Romberg test, all subjects with LMTZs, regardless of the zone number, showed an increasd Romberg coefficient. Conclusion. Multiple LMTZs of cervical muscles can reduce postural stability through increased afferentation from the cervical proprioreceptors. It is more obvious during in case of eye control elimination. Выделяют множество причин развития постуральных нарушений, среди которых дисфункция мышц шейного отдела позвоночника является наиболее спорной. Цель исследования. Выявить влияние латентных миогенных триггерных зон шейной мускулатуры на постуральную устойчивость. Материалы и методы. Использовался метод компьютерной стабилографии. Латентные миогенные триггерные зоны (лМТЗ) определялись по наличию в мышце шеи уплотненного узелка или пучка и по повышенной болевой чувствительности в этой области. Оценка постуральной устойчивости у испытуемых с лМТЗ осуществлялась с помощью стандартного стабилографического тестирования и теста Ромберга. В стандартной пробе оценивались как классические, так и векторные показатели. Результаты. Показано, что у испытуемых с множественными лМТЗ (4 и более) достоверно изменяются только векторные показатели. Так, установлено снижение качества функции равновесия, а также повышение средней линейной скорости центра давления и увеличение нормированной площади векторограммы по сравнению показателями в группе контроля и группе с единичными лМТЗ (от 1 до 3), что указывает на снижение постурального контроля у лиц с множественными лМТЗ. У испытуемых с единичными лМТЗ отмечено достоверное снижение классических показателей и ряда векторных, что свидетельствует об улучшении постуральной устойчивости в данной группе испытуемых. В пробе Ромберга все субъекты с лМТЗ независимо от количества последних показали повышение коэффициента Ромберга. Выводы. Множественные лМТЗ шейной мускулатуры могут снизить постуральную устойчивость через усиление афферентации от шейных проприорецепторов, что наиболее выражено при элиминации зрительного контроля.
