Enhancing mental health literacy in India to reduce stigma: the fountainhead to improve help-seeking behaviour

Purpose – Lack of understanding around mental illness and stigma are an overwhelming barrier in help-seeking behaviour for mental health concerns. The purpose of this paper is to examine mental health literacy and social attitudes as instrumental factors in building capacity of the demand-side to support and access mental health care at the community level in India. Design/methodology/approach – Knowledge, Attitude and Practice surveys were administered to 521 persons from the general population, distributed equally in the age range of 15-60 years. The study included 52 respondents per district from ten districts across five states in India, namely Andhra Pradesh, Assam, Delhi, Gujarat and Uttar Pradesh. The responses were collected and analysed thematically, keeping in mind the relevance of these findings as contributors to knowledge of mental health and to the construct of stigma. Findings – Pervasive socio-cultural factors, especially stigma inhibit access to basic mental health information and care, despite knowledge that mental illness is treatable. Degrading treatment, loss of personal liberty and social exclusion, i.e. compromised human rights at the community level are widespread. Self-reported attitudes when encountering a person with mental illness show that respondents act out of fear and are guided by misinformation and myths. Extant knowledge on mental health is attributed predominantly to informal networks, as a potential resource to be strengthened. Practical implications – Realising mental health care, including help-seeking behaviour calls for greater knowledge-sharing, sensitisation and community engagement. Originality/value – This paper fulfils an identified need to study current levels of mental health literacy and underlying perceptions that contribute to the persistent treatment gap.

Purpose The term “Mental health literacy” is defined as knowledge and beliefs about mental disorders which aid their recognition, management or prevention. The importance of health literacy for physical health is widely studied; however, the area of mental health literacy in Pakistan has been comparatively neglected. The purpose of this paper is to address the knowledge about mental health in people living in Pakistan. Design/methodology/approach Relevant literature relating to mental health literacy was identified through various database searches. The databases searched included: PubMed, Cochrane database of Systemic Reviews, PsycINFO using the terms mental health, mental health literacy, mental health education, Pakistan. Findings Literature suggests that there is dearth of knowledge about mental illnesses and their treatment among public. This review also highlights the importance of mental health literacy among professionals working in the field of health care. In Pakistan, due to low literacy rate, a high percentage of poverty and dearth of trained professionals warrants an emendation in approaches established for attaining the goal of public health and psychiatric care. Practical implications Findings have implications for practitioners in the field of mental health care as well as designing targeted interventions for enhancing mental health literacy and help-seeking behavior in the future. Originality/value A limited understanding and lack of improvement in mental health literacy may interfere with society’s acceptance of evidence-based mental health care which may hamper the delivery of adequate mental health services to the needy.

Purpose Many university students in the UK experience mental health problems and little is known about their overall mental health literacy and help-seeking behaviours. The purpose of this paper is to ascertain levels of mental health literacy in UK university students and to examine whether mental health literacy is associated with better mental health outcomes and intentions to seek professional care. Design/methodology/approach A total of 380 university students at a university in the south of England completed online surveys measuring multiple dimensions of mental health literacy, help-seeking behaviour, distress, and well-being. Findings Mental health literacy in the students sampled was lower than seen in previous research. Women exhibited higher levels of mental health literacy than men and postgraduate students scored higher than undergraduate students. Participants with previous mental health problems had higher levels of mental health literacy than those with no history of mental health problems. Individuals were most likely to want to seek support from a partner or family member and most participants indicated they would be able to access mental health information online. Mental health literacy was significantly positively correlated with help-seeking behaviour, but not significantly correlated with distress or well-being. Practical implications Strategies, such as anonymous online resources, should be designed to help UK university students become more knowledgeable about mental health and comfortable with seeking appropriate support. Originality/value This study is the first to examine multiple dimensions of mental health literacy in UK university students and compare it to help-seeking behaviour, distress, and well-being.

[ACCESS RESTRICTED TO THE UNIVERSITY OF MISSOURI AT REQUEST OF AUTHOR.] Teacher mental health literacy (MHL) includes their attitudes, knowledge, and help-seeking abilities to support youth mental health. Students' access to support in schools depends on their teacher, but concerningly pre- and in-service training lacks MHL. Therefore, this study used a randomized control trial to evaluate a novel MHL training with elementary teachers. Mental Health CARE in Schools is an in-service training for classroom teachers. Training targets how to create mentally healthy classrooms, actively observe and report risk, and embed supports for students experiencing problems. Primary participants included (n = 82) classroom teachers and their students (n = 1, 497). The training resulted in large-sized effects on teachers' selfreported MHL for the trained participants when compared to those in the waitlist condition and from their pre-test scores. Additionally, the current study explored the correspondence between teacher self-reported MHL and their Social, Academic, and Emotional Behavior Risk Screening (SAEBRS) results for their classroom of students. Multilevel modeling identified that in addition to teacher rater differences, additional variance in student scores could be accounted for by teachers' MHL. Teachers' level of self-reported MHL had a positive relationship with their students' SAEBRS scores and an inverse relationship with SAEBRS base rates of risk. Initial evidence from this empirical investigation extends the theory of teacher MHL, yielding implications for both educators and researchers. Limitations and future directions are discussed.

Introduction: Hispanics/Latinos utilize mental health care at a rate that is about half that of non-Hispanic Whites. Stigma and cultural and religious values play a significant role in mental health care utilization disparities. The purpose of this study was to explore beliefs about mental illness among Latino faith-based communities. Methods: A qualitative approach was used to explore perceptions of mental illness that were elicited during “El Buen Consejo,” a mental health literacy intervention delivered within three faith-based settings. Results: Participants were socialized to believe that persons with mental illness were dangerous, out of control, and suffering from an incurable illness that resulted in rejection and ostracism. Most families would deny the existence of depression and mental illness, unless symptoms greatly interfered with daily functioning or were life-threatening. Religious coping such as prayer, and faith in God, were believed to be protective factors. Causal attributions for depression were both biomedical and religious, such as lack of faith, not praying, demons, and sinful behaviors of parents. Conclusion: Latinos rely upon churches as a major social, educational, and spiritual resource. The cultural values among Latinos can be a source of strength but also contribute to stigma. The faith-based community is an important target for mental health literacy and antistigma interventions.

Although research has explored stress in coaches, little is known about their mental health literacy and how this is associated with their mental health. Their mental health literacy may have an impact on the wellbeing of their young players. This study examined levels of mental health literacy, help-seeking behaviours, distress, and wellbeing, as well as relationships amongst these variables, in coaches in the United Kingdom. Coaches were recruited through social media and asked to fill out questionnaires. A total of 103 coaches ( n = 65 men, n = 37 women, n = 1 other gender) participated in the study. Coaches had an average age of 27.9 (SD = 10.6) years. Most self-identified as heterosexual ( n = 85, 82.5%), had four or more years of experience ( n = 46, 44.7%), coached children ( n = 77, 74.8%), and coached at either the beginner ( n = 30, 29.1%) or amateur levels ( n = 66, 64.1%). In total, 20 (19.4%) coaches indicated a previous diagnosis of a mental disorder. A total of 51 (49.5%) coaches indicated that, at the time of the completion of the survey, they exhibited symptoms of a mental disorder. Overall, the average mental health literacy score was 123.10 and lower than in previous studies. Mental health literacy was not significantly associated with help-seeking behaviour, distress, or wellbeing. General help-seeking behaviour was positively correlated with wellbeing. The results suggest that coach certification programmes could adopt strategies to help coaches improve their knowledge of mental health services and how such services may be accessed. These changes may improve the wellbeing of coaches and may benefit their athletes.

Purpose Research on mental illness stigma in the Arab world has traditionally focused on socio-cultural barriers that deprive persons with mental illness from their fundamental human right for privacy and informed consent. The purpose of this paper is to address the question whether or not mental health legislations in a number of Arab countries effectively safeguard the human rights of people with mental illness and protect them from stigmatizing and discriminatory practices. Design/methodology/approach A qualitative review of literature was performed over two rounds of search, targeting published research on mental illness stigma in the Arab world from year 2000 until now and existing national mental health legislations in the Arab world, using English and Arabic databases. Findings The review reveals that beyond society and culture, persistence of mental illness stigma in the Arab world may be explained by absent or inefficient monitoring mechanisms of mental health legislations and policies within the health-care setting. Although integration of mental health services into the primary health care system is being gradually implemented as a step toward de-stigmatization of mental illness, more remains to be done to change the stigmatizing behavior of the health personnel toward mental illness. Originality/value Mental health authorities in the Arab world need to be more aware of the public perceptions explaining people’s fear and reluctance to seek mental health care, so as to ensure that the control and monitoring mechanisms at both the primary and mental health care levels foster a human rights, culturally competent, patient-friendly and non-stigmatizing model of mental health care.

AbstractResearch indicates that individuals who have high levels of mental health literacy engage in help-seeking for mental health issues more in comparison to those with low mental health literacy. It is therefore important to ensure that young people are able to identify signs of psychological distress and identify appropriate supports and resources when necessary. Preliminary research has identified gender differences in help-seeking and identification of mental health problems. This study aimed to investigate the relationship between mental health literacy, help-seeking intentions and wellbeing in senior high school students. The sample consisted of 22 females and 10 male students, ranging from 16–18 years. Findings indicated a positive correlation between wellbeing and general help-seeking, as well as wellbeing and help-seeking for suicidal ideation. Gender differences were identified for aspects of mental health literacy and help-seeking intentions. Results of this research have implications for practitioners in fostering positive outcomes and developing targeted interventions towards improving mental health literacy and help-seeking behaviour in the future.
