Do ECE Public Policies Consider Young Children's Agency? A Study in São Paulo, Brazil

Abstract Background The state of São Paulo recorded a significant reduction in infant mortality, but the desired reduction in maternal mortality was not achieved. Knowledge of the factors with impact on these indicators would be of help in formulating public policies. The aims of this study were to evaluate the relations between socioeconomic and demographic factors, health care model and both infant mortality and maternal mortality in the state of São Paulo, Brazil. Methods In this ecological study, data from national official open sources were used. Analyzed were 645 municipalities in the state of São Paulo, Brazil. For each municipality, the infant mortality and maternal mortality rates were calculated for every 1000 live births, 2013. The association between these rates, socioeconomic variables, demographic models and the primary care organization model in the municipality were verified. We used the zero-inflated negative binomial model. Gross analysis was performed and then multiple regression models were estimated. For associations, we adopted “p” at 5%. Results The increase in the HDI of the city and proportion of Family Health Care Strategy implemented were significantly associated with the reduction in both infant mortality (neonatal + post-neonatal) and maternal mortality rates. In turn, the increase in birth and caesarean delivery rates were associated with the increase in infant and maternal mortality rates. Conclusions It was concluded that the Family Health Care Strategy model that contributed to the reduction in infant (neonatal + post-neonatal) and maternal mortality rates, and so did actors such as HDI and cesarean section. Thus, public health managers should prefer this model. Key messages Implementation of public policies with specific focus on attenuating these factors and making it possible to optimize resources, and not interrupting the FHS. Knowledge of the factors with impact on these indicators would be of help in formulating public policies.

Abstract This paper compares the energy consumption, CO2 emissions and public policies of two mega-cities, Sao Paulo (SP) and Shanghai (SH), in order to identify their GHG emissions mitigation policies. Both cities have experienced rapid growth of the automotive sectors resulting in sizable pollution and CO2 emission challenges. SP has successfully implemented the ethanol and encouraged the growth of the fleet of light-duty vehicles. SH has coal-based power generation and restricted the ownership of the vehicles in an attempt to reduce GHG emissions, invested in public transportation and electric mobility. Tabular analysis of secondary data was adopted in this study, revealing also that SP has considerably expanded individual transportation. Despite investments in ethanol, the city could not contain the increase in CO2 emissions from road transportation. SH invested in public transportation and inhibited individual transportation, but also failed to contain CO2 emissions. Mitigation policies and measures taken were not sufficient to prevent growth of CO2 emissions in both cities. To reduce CO2 emissions in transportation, SP and SH should focus on public policies to encourage public and clean transportation and limit the burning of fossil fuels.

The objective of this research was to analyse the public policies of urban mobility promoted by the São Paulo City Hall, through bodies such as CET (Traffic Engineering Company), SPTrans (São Paulo Transportations SA) and the Municipal Transport Secretariat, between 2013 and 2016. The period corresponds to the mandate of the then mayor of São Paulo, Fernando Haddad (PT). The analysis was based on a historical and theoretical recapitulation to better understand and evaluate the transformations in the orientation and execution of the policies of that period in question.

Visceral leishmaniasis (VL) is a public health problem in Brazilian municipalities. As much as there is a planning of public policies regards VL in São Paulo State, new cases have been reported and spread. This paper aims to discuss how the Center for Zoonoses Control conducts its actions spatially in endemic city of Presidente Prudente, São Paulo State. Data are from the Municipal Health Department of Presidente Prudente, Adolfo Lutz Institute, and Brazilian Institute of Geography and Statistics. We spatially estimated the dog population per census tract and used geoprocessing tools to perform choropleth maps, spatial trends, and spatial autocorrelation. We found a spatial pattern of higher prevalence in the city’s outskirt and a positive statistically significant spatial autocorrelation (I = 0.2, p-value < 0.000) with clusters of high-high relationships in the Northwest part of the city. Moreover, we identified a different direction in the path of the conducted serosurveys versus the canine VL trend, which stresses the fragility of the Center for Zoonoses Control actions to control the disease. The Center for Zoonoses Control always seems to chase the disease. The spatial analysis may be useful for rethinking how the service works and helps in public policies.
