Objectives: The aim of this controlled follow-up study was to compare the need for revision surgery, long-term efficacy, and satisfaction in chronic rhinosinusitis patients who had undergone maxillary sinus operation with either balloon sinuplasty or traditional endoscopic sinus surgery (ESS) technique. Methods: Thirty-nine ESS patients and 36 balloon patients of our previously described cohort, who had been primarily operated in 2008 to 2010, were contacted by phone. Symptoms, satisfaction, and need for revision surgery were asked. In addition, we collected data of patients who had undergone primary maxillary sinus balloon sinuplasty in the Helsinki University Hospital during the years 2005 to 2019. As a control group, we collected data of patients who had undergone primary maxillary sinus ESS at 3 Finnish University Hospitals, and 1 Central Hospital in years 2005, 2008, and 2011. Results: Altogether, 77 balloon patients and 82 ESS patients were included. The mean follow-up time was 5.3 years in balloon group and 9.8 years in ESS group. Revision surgery was performed on 17 balloon patients and 6 ESS patients. In the survival analysis, the balloon sinuplasty associated significantly with a higher risk of revision surgery compared to ESS. According to the phone interviews, 82% of ESS patients and 75% of balloon patients were very satisfied with the primary operation. Conclusion: Although the patient groups expressed equal satisfaction and change in symptoms after the operations, the need for revision surgery was higher after balloon sinuplasty than after ESS. This should be emphasized when counselling patients regarding surgical options.

AbstractObjectives:Endoscopic sinus surgery has been widely performed to treat nose and paranasal diseases. However, it is difficult to manipulate anterior wall lesions of the maxillary sinus using conventional surgical instruments. This paper presents a method of performing endoscopic surgery for anterior wall lesions of the maxillary sinus, using a 135° reflective CO2 laser.Method:A CO2 laser with a 135° reflective tip on the pipe-guide handpiece and a CO2 laser angulated to the same degree were used. The pipe-guide handpiece with reflective tip was inserted into the nasal cavity and the base of the maxillary sinus anterior wall lesion was vaporised and removed via an enlarged natural ostium. During the procedure, the maxillary antrum was visualised with a 70° endoscope. Ten cases of maxillary sinus anterior wall lesion underwent this surgical procedure.Results:In all cases, the base of the maxillary sinus anterior wall lesion was removed completely and recurrence avoided.Conclusion:This method is a reliable procedure enabling endoscopic sinus surgery for anterior wall lesions of the maxillary sinus.

Ameloblastoma (AM) in the maxillary sinus is rare. This benign entity shows locally invasive, destructive and aggressive behaviour and a high rate of recurrence. Therefore, the course of treatment is radical resection. We report the case of a 38-year-old man presenting with signs of recurrent sinusitis in the Ear, Nose and Throat Department. Transnasal flexible endoscopy revealed a cystic mass in the right inferior and middle nasal passage. CT scan showed an obliterated right maxillary sinus with a ballooning effect and pressure atrophy of the lateral sinus wall, without possible differentiation of the middle and low nasal turbinate. The patient was treated with transnasal functional sinus surgery; pathology stated AM. AM in the maxillary sinus is rare, locally destructive and therefore as a gold standard is resected radically to prevent recurrence. We demonstrate a conservative approach; explicitly, we combined a transvestibular and functional endoscopic sinus surgery resection of the AM to maintain function and reduce the possibility of postoperative impairments. Whether the strategy of treatment for AM is conservative, it nonetheless can result in a recurrence-free status. Nevertheless, inclusion into an oncological follow-up-programme with regularly performed MRI and CT is recommended.

Introduction: When a dental element is lost in the posterior region of the maxilla, there is natural reabsorption of the alveolar process and at the same time there will be pneumatization of the maxillary sinus. It will increase its volume towards the place where the roots existed and this will often make it difficult or impossible to restore implants in place. For this reason, the procedure for elevating the floor of the maxillary sinus or short implants should be performed when possible. In this context, allogeneic, xenogenous, and alloplastic bone grafts are an alternative for the treatment of bone defects in the jaws, since they avoid the need for a second surgical access. However, due to the need for processing to eliminate antigenic components, these grafts are only osteoconductive with a lower bone formation potential compared to autogenous bone grafts. Also, in this context, in the last 20 years, platelet concentrates have been proposed as regenerative materials in tissue regeneration procedures. Among the platelet concentrates proposed in the literature, PRP and FRP are found to act as autogenous platelet aggregates with osteoinductive properties. Objective: The present study aimed to conduct a wide literature review on maxillary sinus surgery using fibrin-rich plasma. Methods: Experimental and clinical studies (case reports, retrospective, prospective and randomized) with qualitative and/or quantitative analysis were included. Results: The total of 48 articles were found involving Maxillary sinus surgery, Fibrin-rich plasma, and Biomaterials, of which 22 were selected to compose the present study. Conclusion: Based on the literary findings, it was shown that the FRP is favorable for bone formation processes for dental implants, especially when combined with xenografts.

AbstractMaxillary sinus disease can lead to dehiscence of the orbital floor. Using endoscopic sinus surgery it is possible to simultaneously reconstruct the orbital floor and to assess and treat underlying sinus disease.
