One of the most challenging and difficult task in clinical practice of an endodontist is to manage pain. Advancements in understanding of pain physiology and pharmacology have provided a key upon which quick decisions can be made for treatment of any endodontic case. Despite advancement in technology, patients perception and apprehension is till date considered as a standard to acknowledge the expertise of an endodontist when undergoing a routine endodontic therapy. The current article aims to reflect effective strategies of management of endodontic pain in pre-operative, operative and postoperative situations. In lieu of clinical relevance of this topic in day to day life of an endodontist the topic was also addressed as a scientific paper presentation in a national conference to welcome thoughts of colleagues at a broader platform.

PHE and FGDP(UK) co-published Guidance Notes for Dental Practitioners on the Safe Use of X-ray Equipment in 2020 to encompass all relevant information to safely use and maintain radiographic equipment by dental practitioners. This guidance supersedes the first edition, previously published in 2001 by the Department of Health, and there are a number of notable changes that should be highlighted to the dental team. The document is freely available and accessible on the FGDP(UK) website and should be an essential tool to ensure the safety of the dental team, patients and the public. CPD/Clinical Relevance: Members of the dental team must acknowledge the updated guidance notes and look to incorporate these changes into their working routine to ensure good clinical practice.

The routine scale and polish is the most commonly performed treatment procedure in general dental practice. With a large proportion of state dental health expenditure in the UK and Ireland being used to provide ‘routine scale and polish’ treatments, it is necessary to establish the true value of this treatment for patients. In this narrative review, we discuss the available evidence regarding provision of the routine scale and polish. Based on the available studies, there is little evidence to support the use of the routine scale and polish alone for the prevention of periodontal disease. CPD/Clinical Relevance: This paper evaluates the evidence surrounding the routine provision of scaling and polishing for the prevention of gingivitis and periodontitis.

The percentage of the UK population that is over 65 years old is growing and this brings a number of complex challenges for the dental profession. Patients are struggling to attend for appointments and those who do are attending with increasing physical and mental morbidities and greater care needs, with dementia, in particular, bringing significant difficulties and complications during dental management. This article describes the oral manifestations of dementia and provides clinical advice to practitioners in supporting these patients, and discusses communication, diagnosis, treatment planning, consent, the dental practice environment and home care advice. CPD/Clinical Relevance: This article highlights the challenges an older population brings to the dental profession, focusing on the complications dementia creates. It describes its oral manifestations, as well as providing clinical advice to help practitioners support these patients in all aspects of their care.

In the second of two papers on the recognition and management of potential medical emergencies that may occur in the dental clinic setting, the development of new algorithms to assist in making it easier for members of the dental team to recognize and treat such situations will be discussed. Used in conjunction with the M-DEWS2 scoring sheet as discussed in the first paper, these algorithms cover the eight common medical emergencies that may occur in dental practice. CPD/Clinical Relevance: All members of the dental team require the appropriate skills and knowledge to manage medical emergencies should they occur.
