Lack of a Relationship Between Glenohumeral External-Rotation Strength and Posterior Shoulder Tightness in Baseball Players

Context:Posterior shoulder tightness has been associated with altered shoulder range of motion (ROM) and several pathologic entities in baseball players. This tightness is hypothesized to be the result of the cumulative stress placed on the posterior shoulder during the deceleration phase of the throwing motion. The role of the posterior shoulder static restraints is to absorb this load while the glenohumeral (GH) external rotators eccentrically decelerate the arm after ball release and therefore also help dissipate this force. As such, the authors hypothesized that if the GH external rotators are weak, an excessive amount of this deceleration force is placed on the static restraints, which may lead to subsequent tightness.Objective:To compare the relationship between GH external-rotation strength and posterior shoulder tightness as measured by GH horizontal-adduction and internal-rotation ROM.Design:Descriptive study.Setting:Laboratory.Participants:45 professional baseball players.Main Outcome Measures:GH external-rotation strength and GH horizontal-adduction and internalrotation ROM.Results:GH external-rotation strength showed no relationship with either GH horizontal-adduction ROM (r2 = .02, P = .40) or GH internal-rotation ROM (r2 = .002, P = .77).Conclusion:There is little to no relationship between GH external-rotation strength and posterior shoulder tightness in professional baseball players. The posterior static restraints of the shoulder may absorb a large majority of the deceleration forces during the throwing motion. Although strengthening of the posterior shoulder dynamic restraints should not be overlooked, routine stretching of the static restraints may be more beneficial for decreasing posterior shoulder tightness and the subsequent risks associated with this tightness, although future research is warranted.

Context:Stretching prior to activity or as a rehabilitative intervention may promote increased throwing shoulder range of motion (ROM) in baseball pitchers.Objective:To evaluate the acute effects of Fauls modified passive stretching routine on throwing shoulder mobility in collegiate baseball players.Design:Repeated measures.Setting:Laboratory.Participants:Thirty collegiate baseball players with unimpaired shoulders.Interventions:Fauls modified passive stretching routine was performed on the throwing shoulder of each subject.Outcome Measures:Shoulder complex and passive isolated glenohumeral internal and external rotation ROM were measured with a goniometer, and posterior shoulder tightness was assessed with the Tyler’s test method using a carpenter’s square. Measurements were made bilaterally.Results:The dominant shoulder displayed significant increases in glenohumeral and shoulder complex internal and external rotation ROM and significantly decreased posterior shoulder tightness following the stretching routine.Conclusion:Application of the Fauls modified passive shoulder stretching routine results in acute gains in throwing shoulder mobility of collegiate baseball players.

Background: Posterior shoulder tightness, as demonstrated by limited internal rotation range of motion, is a suggested factor in many shoulder pathologies. Methods to increase posterior shoulder mobility may be beneficial. Hypothesis: Shoulder internal rotation range of motion will not change with either of 2 interventions: cross-body stretch alone and cross-body stretch plus posterior capsule joint mobilization. Study Design: Randomized controlled single-blinded clinical trial. Methods: The study comprised 39 college-age asymptomatic participants (7 men, 32 women) who were randomly assigned to 1 of 2 groups: stretching only (n, 20) and stretching plus posterior joint mobilizations (n, 19). All had a between-shoulder difference of internal rotation of 10° or more. Shoulder internal and external rotation was measured before and after a 4-week intervention period and 4 weeks postintervention. Participants in the stretching-only group performed the cross-body stretch on the limited side. Those in the other group (cross-body stretch plus joint mobilization) were treated with posterior joint mobilization techniques on the limited side. Results: Overall means for internal rotation of the treated shoulders significantly increased over baseline at the end of the intervention period and at 4 weeks postintervention. External rotation in all shoulders remained unchanged. By the end of intervention, total motion increased significantly from baseline but decreased significantly from the end of intervention to 4 weeks postintervention. Although not statistically significant, the second group (cross-body stretch plus joint mobilization) had greater increases in internal rotation. At 4 weeks postintervention, the second group had maintained its internal rotation gains to a greater degree than those of the stretching-only group. Conclusion: Internal rotation increased in both groups. Inclusion of joint mobilization in a rehabilitation program created trends toward increased shoulder internal rotation mobility. Clinical Relevance: Both methods—cross-body stretch and cross-body stretch plus joint mobilization—may be beneficial for those with limited internal rotation range of motion.

BACKGROUND: Posterior shoulder tightness as demonstrated by glenohumeral internal rotation deficit (GIRD) is a common contributor to many shoulder pathologies in overhead throwing athletes. PURPOSE: This study aimed to assess immediate effect of combining glenohumeral and scapulothoracic mobilization with stretching on improving internal rotation range of motion (ROM) in overhead athletes with GIRD METHODS: This study was a single-blind randomized controlled trial with parallel groups. The participants were 30 asymptomatic male volleyball players who had a loss of shoulder internal rotation ROM of 15 degrees or more on their dominant compared to their nondominant side. They were randomly assigned to 1 of 2 groups: stretching only (n = 15) or stretching plus joint mobilization (n = 15). Participants in the stretching group performed cross-body stretches, and those in stretching plus mobilization group were treated with both cross-body stretching and mobilization techniques for the glenohumeral and scapulothoracic joints. Both groups received the intervention every other day for 1 week. Shoulder internal and external ROM were measured before and after the intervention. RESULTS: Before the intervention, dominant-side deficits in internal rotation ROM compared to the nondominant side were 20.11±5.27 degrees in the stretching group and 21.87±8.06 degrees in the stretching plus mobilization group. After the intervention, internal rotation ROM deficit between the dominant and nondominant side decreased to 11.28±5.82 in the stretching group (P <  0.001) and 10.85±9.19 in the stretching plus mobilization group (P <  0.001). However, the between-group difference was not statistically significant (P = 0.389). External rotation ROM remained unchanged in both groups (P >  0.05). CONCLUSION: Both interventions (stretching and stretching plus mobilization) can have the same beneficial effect in decreasing GIRD in asymptomatic overhead athletes.

Abstract Context: The overhead throwing motion is complex, and restrictions in range of motion (ROM) at the hip may place additional demands on the shoulder that lead to injury. However, the relationship between hip and shoulder ROM in athletes with and without a history of shoulder injury is unknown. Objective: To (1) determine if differences exist in hip and shoulder ROM between professional baseball players with a history of shoulder injury and those with no history of shoulder injury and (2) assess relationships between hip and shoulder ROM in these players. Design: Cross-sectional study. Patients or Other Participants: Fifty-seven professional baseball players. Main Outcome Measure(s): Outcome measures consisted of hip extension and internal rotation, shoulder internal and external rotation, glenohumeral internal-rotation deficit, and history of shoulder injury. Differences in shoulder and hip ROM were assessed with a 1-way analysis of variance. Associations between hip and shoulder ROM were assessed with linear regression. Results: Nonpitchers with a history of shoulder injury had more external rotation and less internal rotation of the shoulder than nonpitchers with no history of shoulder injury. Glenohumeral internal-rotation deficit was greater in both pitchers and nonpitchers with a history of shoulder injury. The relationship between dominant hip extension and shoulder external rotation was significant for pitchers with a history of shoulder injury and nonpitchers with a history of shoulder injury. Conclusions: Shoulder injury may be associated with specific measures of hip and shoulder ROM, and hip extension and shoulder external rotation may be related in baseball players with a history of shoulder injury. Additional research is necessary to understand the specific mechanisms of shoulder injury in the throwing athlete.

Context:Overhead stress from both swimming and throwing in water-polo players might lead to alterations in shoulder mobility and subsequent injury.Objective:To evaluate clinical measures of shoulder mobility in college water-polo players.Design:Repeated measures.Setting:University athletic training facility.Patients:31 Division I water-polo athletes.Main Outcome Measures:Measures were obtained for both the dominant and nondominant shoulders. Scapular upward rotation was measured using a digital inclinometer, posterior shoulder tightness was assessed by recording horizontal adduction with the scapula stabilized, and passive isolated glenohumeral-joint internal- and external-rotation range of motion were measured using goniometry.Results:No significant difference was observed between sides for scapular upward rotation (P= .68), posterior shoulder tightness (P= .25), or internal rotation (P= .41). A significant difference between sides was present for external rotation (P< .0001) and total arc of motion (P= .039).Conclusions:The dominant shoulders demonstrated significantly greater external rotation and a significantly greater total arc of motion than the nondominant shoulders did.

Background: Neurogenic thoracic outlet syndrome (nTOS) is becoming more recognized as a diagnosis in the throwing athlete. Currently, there is limited information on the clinical presentation and development of nTOS in baseball players. Purpose: To compare passive shoulder range of motion (ROM) and anatomic humeral retrotorsion (HRT) of baseball players diagnosed with nTOS with a group of healthy, matched controls. Study Design: Cross-sectional study; Level of evidence, 3. Methods: A total of 53 adolescent baseball players diagnosed with nTOS (age, 17.2 ± 2.3 years; height, 180.9 ± 10.1 cm; weight, 80.0 ± 13.3 kg) were compared with 53 healthy baseball players (age, 17.2 ± 2.4 years; height, 183.9 ± 9.0 cm; weight, 83.8 ± 11.5 kg). Participants were measured for shoulder internal rotation (IR) and external rotation (ER) ROM and HRT. All measurements were taken bilaterally, and the differences (throwing to nonthrowing arm) were used to calculate mean values for glenohumeral internal rotation difference, glenohumeral external rotation difference (GERD), total rotational motion difference (TRMdiff), and anatomic humeral retrotorsion difference. Group comparisons were made between the nTOS and control players using multivariate analysis of variance, and descriptive comparisons were made with independent t tests. Results: There were no significant differences between groups in age, height, weight, or years of experience. Players in the nTOS group had significantly less throwing arm ER compared with controls (103.4° ± 10.4° vs 109.6° ± 7.5°, respectively; P = .001) and GERD (3.0° ± 9.2° vs 8.8° ± 9.2°, respectively; P = .002). TRMdiff was significantly greater in nTOS (–11.1° ± 11.1°) than in controls (–3.7° ± 9.4°) ( P < .001). Conclusion: In the current study, adolescent baseball players diagnosed with nTOS were evaluated with shoulder ROM differences when compared with a matched healthy cohort. A loss of throwing arm ER appeared to be the main factor behind shoulder ROM changes in the nTOS group.

BACKGROUND: The detection of primary risk factors through the assessment of shoulder range of motion (ROM) and muscle function could prevent injuries in the initial stages of the handball (HB) practice. OBJECTIVE: To compare shoulder ROM, muscle strength and muscle extensibility between throwing and non-throwing shoulders in young HB athletes and between non-HB athletes. METHODS: A cross-sectional study was designed. Participants aged between 14–16 years that practice physical activity regularly were included and divided in two groups. The HB group included volunteers who practiced HB, the control group included volunteers who practiced other activities. The variables measured were internal rotation and external rotation ROM, maximum isometric strength, and extensibility of the tissues of the posterior part of the throwing and non-throwing shoulder. RESULTS: Thirty-seven subjects were included. The HB group (n= 19) showed greater ROM in both shoulders compared to the control group (n= 18) (p< 0.05). The throwing shoulder of the HB group showed less internal rotation ROM, greater external rotation ROM and greater internal rotation strength when compared to the non-throwing shoulder (p< 0.05). CONCLUSIONS: The results of the study suggest that young HB athletes present differences in shoulder ROM compared to non-HB athletes. Also, HB athletes showed differences in ROM and strength between the TS and non-TS. Hence, the assessment of the shoulder ROM and muscle function should be taken into consideration to avoid injuries in this population.

Abstract Context: Shoulder range of motion (ROM) and strength are key injury evaluation components for overhead athletes. Most normative values are derived from male baseball players with limited information specific to female softball players. Objective: To determine between-limb differences in shoulder ROM and strength in healthy collegiate softball players. Design: Descriptive laboratory study. Setting: University research laboratory and collegiate athletic training room Participants: Twenty-three healthy collegiate softball players (age=19.9 ± 1.2y; height=170.5 ± 4.3cm; mass=78.4 ± 11.3kg). Interventions: Outcome measures included shoulder ROM (internal [IR] and external rotation [ER]), isometric strength (IR, ER, flexion, abduction [135 degrees], and horizontal abduction), and a measure of dynamic strength (Upper Quarter Y Balance Test [UQYBT]). Main Outcome Measures: Paired sample t-tests were used to determine between limb differences for each outcome measure. Results: Participants had significantly more ER ROM (12° more) and significantly less IR ROM (12° less) on the dominant arm, relative to the non-dominant arm. There were no significant differences between limbs for any of the isometric strength measures or for the UQYBT reach directions. Conclusions: While female collegiate softball players demonstrated typical changes in ER and IR ROM in the dominant arm, they demonstrated relatively symmetrical performance across strength measures, which contrasts with previous studies using male baseball players.
