BackgroundAvoidable complications for surgical patients still occur despite efforts to improve patient safety processes in operating rooms. Analysis of experiences of operating room nurses can contribute to better understanding of perioperative processes and flow, and why avoidable complications still occur.AimTo explore aspects of patient safety practice during joint replacement surgery through assessment of operating room nurse experiences.MethodA qualitative design using semistructured interviews with 21 operating room nurses currently involved in joint replacement surgery in Sweden. Inductive qualitative content analysis was used.ResultsThe operating room nurses described experiences with patient safety hazards on an organisational, team and individual level. Uncertainties concerning a reliable plan for the procedure and functional reporting, as well as documentation practices, were identified as important. Teamwork and collaboration were described as crucial at the team level, including being respected as valuable, having shared goals and common expectations. On the individual level, professional knowledge, skills and experience were needed to make corrective steps.ConclusionThe conditions to support patient safety, or limit complication risk, during joint replacement surgery continue to be at times inconsistent, and require steady performance attention. Operating room nurses make adjustments to help solve problems as they arise, where there are obvious risks for patient complications. The organisational patient safety management process still seems to allow deviation from established practice standards at times, and relies on individual-based corrective measures at the ‘bedside’ at times for good results.

The use of World Health Organization’s (WHO’s) Safe Surgery checklist is an established practice worldwide and contributes toward ensuring patient safety and collaborative teamwork. The aim of this study was to elucidate operating room nurses’ and operating room nursing students’ experiences and opinions about execution of and compliance with checklists. We chose a qualitative design with semistructured focus group discussions. Qualitative content analysis was conducted. Two main themes were identified; the Safe Surgery checklists have varied influence on teamwork and patient safety, and taking responsibility for executing the checks on the Safe Surgery checklist entails practical and ethical challenges. The experiences and opinions of operating room nurses and their students revealed differences of practices and attitudes toward checklist compliance and the intentions of checklist procedures. These differences are related to cultural and professional distances between team members and their understanding of the Safe Surgery checklists as a tool for patient safety.

Progress is constantly being made in almost all areas of life, including medicine. For this reason, various measures are being introdu-ced to make any work done more efficient, easier, more productive. Music can be used in medicine, especially in the operating room, as its effect on the members of the operating team is extremely positive. And while this is acknowledged, there is not much research on this topic – the impact of music on members of the operating team.Object – is the influence of music on work performance during joint replacement, from the point of view of surgical team members.Problem – is whether music during endoprosthesis surgery can increase the productivity of surgical team members?Aim – to analyze the influence of music on the performance of work during endoprosthesis operations, from the point of view of the members of the operating team – was aimed at implementing the research objectives:1) to review the peculiarities of joint endoprosthesis operations; 2) to reveal the influence of music on work in the operating room from the point of view of team members;3) to study the influence of music on the quality of work during joint arthroplasty. Methods: The method of questionnaire survey was chosen to study the attitude of the members of the operating team to the influence of music on the work productivity in the operating room. The questionnaire is considered to be the most appropriate method for explaining people’s opinions and behavior, and it is based on reliability. This was related to the aims of this study - to analyze the impact of music on performance during endoprosthesis operations. The study was conducted by submitting questionnaires to 100 members of the operating team (surgeon-operator, anesthesiologist, resuscitator, anesthesiologist, resident, anesthesia and intensive care nurse, operating nurse, and operating assistant) working in health care facilities. It should be noted that during the data collection, the members of the operational team filled in the questionnaires remotely - links to the online uploaded questionnaire were sent to each respondent. This method of conducting the study was chosen due to the current situation in Lithuania - the COVID-19 pande-mic.Results of the study. The study revealed that joint replacement surgery is seen as a complicated procedure, which is complicated by team changes, because it often increases tension, affects everyone’s joint work, feels general discomfort, becomes more difficult to concentrate, increa-ses the likelihood of errors. In order to make teamwork smoother, attention should be paid to the organization of work, the fostering of interperso-nal relationships, the assessment of one’s own characteristics and peculiarities of communication, and the stronger fostering of the work envi-ronment. Music can be an effective and valuable tool for making a positive impact on operational team members. Music can help reduce anxiety, encourage relaxation, suppress the response to stressful situations, and increase the effectiveness of actions. It is very important that the music is selected in the right genre to suit the members of the operating team.

The use of the Surgical Safety Checklist (SSC) is associated with improving patient care according to nursing process standards includes the quality of work of the operating room nurse team. The form of professionalism in the operating room is how the application of a surgical safety checklist as the standard procedure for patient safety in the operating room. This study aims to determine the relationship of characteristics, knowledge, and motivation of nurses in the application of the surgical safety checklist in the operating room of a Batam city hospital. This research is quantitative using an observational analytic research design. This study was conducted on 67 nurses who were taken by total sampling. This research was conducted in three Batam City Hospitals, with hospital accreditation at the same level. Data were analysed by univariate and bivariate using the chi-square test. The results of the study found that most nurses had education at diploma level, with a working period experiences of > 6 months (82%); good knowledge (53.7%) with low motivation (57.7%). There is a relationship between education (p = 0.042); length of work experience (p = 0.010); knowledge (p = 0.002); and motivation (p = 0.05) with the application of SSC. It is expected that health services carry out SSC following the applicable SOPs in the Hospital so that it can reduce work accident rates and improve patient safety.

Abstract Background Recent studies indicate that, in addition to antibody production, lymphocyte responses to SARS-CoV-2 may play an important role in protective immunity to COVID-19 and a percentage of the general population may exhibit lymphocyte memory due to unknown/asymptomatic exposure to SARS-CoV-2 or cross-reactivity to other more common coronaviruses pre-vaccination. Total joint replacement (TJR) candidates returning to elective surgeries (median age 68 years) may exhibit similar lymphocyte and/or antibody protection to COVID-19 prior to vaccination Methods In this retrospective study, we analyzed antibody titters, lymphocyte memory, and inflammatory biomarkers specific for the Spike and Nucleocapsid proteins of the SARS-CoV-2 virus in a cohort of n=73 returning TJR candidates (knees and/or hips) pre-operatively. Results Peripheral blood serum of TJR candidate patients exhibited a positivity rate of 18.4% and 4% for IgG antibodies specific for SARS-CoV-2 nucleocapsid and spike proteins, respectively. 13.5% of TJR candidates exhibited positive lymphocyte reactivity (SI > 2) to the SARS-CoV-2 nucleocapsid protein and 38% to the spike protein. SARS-CoV-2 reactive lymphocytes exhibited a higher production of inflammatory biomarkers (i.e., IL-1β, IL-6, TNFα, and IL-1RA) compared to non-reactive lymphocytes. Conclusions A percentage of TJR candidates returning for elective surgeries exhibit pre-vaccination positive SARS-CoV-2 antibodies and T cell memory responses with associated pro-inflammatory biomarkers. This is an important parameter for understanding immunity, risk profiles, and may aid pre-operative planning. Trial registration Retrospectively registered.
