Synchronous bilateral testicular neoplasms in a black-capped capuchin (Sapajus apella)

A 31-year-old, 5.01 kg, intact male black-capped capuchin (Sapajus apella) was presented for examination after zookeepers noted an enlarged right testicle. Fine-needle aspiration with cytological evaluation identified a seminoma of the right testis. Castration was elected for this non-breeding male, and both testicles were submitted for histopathology. Histopathology confirmed the presence of a seminoma in the right testicle and, incidentally, a mass in the left testis consistent with an interstitial cell tumour was also diagnosed. This report describes the first case of seminoma or interstitial cell neoplasia in a black-capped capuchin, as well as the first case of a non-human primate with synchronous, bilateral testicular neoplasms.

Abstract Background Lymph node inclusions are foci of ectopic tissue in lymph nodes, which were reported in different areas of the body. However, inclusions in the mediastinal lymph node are rare. Here, we report the first case of glandular inclusion within the parenchyma of the intrapulmonary lymph node in a patient with primary lung adenocarcinoma. Case presentation A computed tomography (CT) scan showed a solid pulmonary nodule in the right upper lobe in a 44-year-old man. After a fine needle aspiration biopsy diagnosis of adenocarcinoma, lobectomy and lymph dissection were performed. Histological sections of the lung demonstrated a papillary predominant adenocarcinoma and one intrapulmonary lymph node, which displayed glandular inclusion occupying the node parenchyma. The gland inclusion was very similar to metastasis, but was formed by two layers of epithelial cells, and the abluminal cells were positive for P63, P40, and CK5/6. The patient has remained alive without recurrence and metastasis at the last follow-up before publication. Conclusions It is very important to correctly diagnose a lymph node inclusion for proper clinical management.

Latar Belakang: Pleomorfik adenoma (mixed tumor) adalah tumor kelenjar ludah yang paling sering terjadi (65%) pada kelenjar ludah mayor dan minor. Lokasi intraoral yang paling sering ditemukan adalah palatum. Tujuan: membahas gambaran klinis dan penatalaksanaan pleomorfik adenoma pada platinum. Kasus: dilaporkan 2 buah kasus pleomorfik adenoma pada palatum. Kasus pertama seorang laki-laki usia 29 tahun dengan benjolan pada palatum kanan ukuran 3x2 cm, konsistensi kenyal, berbatas tegas, warna seperti jaringan sekitar dan tidak nyeri. Pasien pernah menjalani operasi pada palatum kanan pada tahun 2005, dan kira-kira 3 tahun setelah operasi benjolan tersebut kambuh di tempat yang sama. Hasil biopsi aspirasi jarum halus adalah mixed tumor. Kasus kedua seorang wanita 22 tahun dengan benjolan pada palatum kiri ukuran 2x1,5 cm, timbul sejak 3 tahu yang lalu, warna seperti jaringan sekitar, konsistensi kenyal, dan tidak nyeri. Riwayat pesien menggunakan kontrasepsi hormonal. Hasil biopsi condong pada adenoma pleomorfik dengan bagian onkositik adenoma dan clear sel adenoma. Penatalaksanaan: dilakukan eksisi luas di bawah anestesi umum pada kedua kasus tersebut, dengan batas 1 cm dari tepi lesi pada jaringan sehat. Kesimpulan : telah dilakukan eksisi luas untuk penanganan kedua kasus pleomorfik adenoma dan palatum. Tidak di temukan rekurensi 1 tahun setelah operasi (kasus 1) dan 2 tahun setelah operasi (kasus 2). Blackground: pleomorphic adenoma (mixed tumor) is the most common tumor of the salivary glands (65%) of the major dan minor salivary glands. Palatum is the most common site in intraoral. Purpose: to elaborate clinical feature and management of pleomophic adenoma of palate. Cases: we reported 2 cases pleomorphic adenoma of palate. The first case was a29 years old male patient with a swelling at the right side of the palate, 3x2 cm sized mass, rubbery in consistency, well demarcated, pinkish in color, and pain less. He had undergone an operation at the right palate in 2005, but 3 years after the operation he got reccurence. The result of fine needle aspiration biopsy was mixed tumor. The second case was fermale 22 years old patient with a swelling at the left side of palate, 2x1,5 cm sized mass, was present 3 years before coming to the clinic, pinkish color, rubbery consistency and painless. She has been using hormonal contraception. The result of incisional biopsy was pleomorphic adenoma with the oncocityc adenoma part and clear cell adenoma. Management: widw exicion was performed under general anesthesia in both cases, with a limit 1 cm clinical margin at its periphery. Conclusion: both patients with pleomorphic adenoma ao palate were treated by wide excision. No recurrence were observed in 1 year (firs case) and 2 years (second case) after the surgery.

Two cases of pulmonary vascular anomaly in unrelated adult alpacas ( Vicugna pacos) are described. In the first case, a 9-year-old intact male alpaca presented at Oregon State University Veterinary Teaching Hospital with bilateral epistaxis and died the subsequent day following severe hemorrhage from the mouth and nostrils. At necropsy, a tortuous vascular lesion was identified in the right cranial lung lobe, associated with hemorrhage into airways. In the second case, a 2-year-old female alpaca presented with postpartum anorexia, opisthotonus, and recumbency. In this second case, a similar vascular lesion was identified in the right cranial lung lobe but without associated hemorrhage. Histopathological examination of the lesion in both cases revealed numerous dilated, irregular blood vessels with marked variation in wall thickness within vessels, surrounded by foci of extramedullary hematopoiesis. Diagnoses of locally extensive pulmonary vascular anomalies (arteriovenous malformations) were made.

Abstract Background Gasless trans-axillary endoscopic thyroidectomy (GTAET) has satisfactory cosmetic effects for the patients who have benign goiter and small thyroid carcinoma, however the complications of this surgical procedure have not been fully documented. Ipsilateral hypoglossal nerve palsy (IHNP) associated with GTAET has never been reported before. Case presentation A 33-year old male patient presented with a 4 × 5 mm solid thyroid nodule in the right lobe. Papillary thyroid carcinoma was confirmed by the fine needle aspiration. He had strong cosmetic demand, therefore GTAET for right lobectomy and central cervical lymphadenectomy was performed in a supine position with cervical extension. Six hours after the operation, he developed tongue deviation to the right side, speech and swallowing difficulties, indicating IHNP. Head and cervical MRI showed no abnormality. The intravenous steroid was used for three days, and oral vitamin B1 and mecobalamin was prescribed for 1 month. Nine days after surgery, he was discharged. Three months after the operation, all the symptoms were completely resolved. Conclusions To the best of the authors’ knowledge, this is the first case of IHNP after GTAET, which will be valuable to add our knowledge to diagnose and treat rare complications of GTAET.

Living donor liver transplantation is a relatively new procedure in which the right side of the liver is harvested in a healthy donor and transplanted into a recipient. After the first case in 1994, over 3000 cases have been done worldwide. This review summarizes the reasons why the procedure is needed, describes its main technical aspects, highlights the boundaries in which it can be done safely, summarizes the current experience worldwide and describes the main points of the program in our unit. We argue that living-donor transplantation is a viable alternative to a long time on the waiting list for several patients, and it can be performed safely and successfully provided that all precautions are undertaken to minimize the risks in the donor and to increase the chances of a good outcome in the recipients. If these prerequisites are met, and within the framework of a structured multidisciplinary program, we believe that living-donor liver transplantation should be funded by health insurances as a recognized therapeutic option.

Vitamin K4 was labelled with 99mTc with an efficiency higher than 97%. The compound was stable up to 24 h at room temperature, and its biodistribution in NMRI mice indicated its in vivo stability. Blood radioactivity levels were high over a wide range. 10% of the injected activity remained in blood after 24 h. Excretion was mostly via kidneys. Only the liver and kidneys concentrated appreciable amounts of radioactivity. Testis/soft tissue ratios were 1.4 and 1.57 at 6 and 24 h, respectively. Testis/blood ratios were lower than 1. In vitro studies with mouse blood indicated that 33.9 ±9.6% of the radioactivity was associated with RBCs; it was washed out almost completely with saline. Protein binding was 28.7 ±6.3% as determined by TCA precipitation. Blood clearance of 99mTc-l<4 in normal subjects showed a slow decrease of radioactivity, reaching a plateau after 16 h at 20% of the injected activity. In scintigraphic images in men the testes could be well visualized. The right/left testis ratio was 1.08 ±0.13. Testis/soft tissue and testis/blood activity ratios were highest at 3 h. These ratios were higher than those obtained with pertechnetate at 20 min post injection.99mTc-l<4 appears to be a promising radiopharmaceutical for the scintigraphic visualization of testes.

Gastrointestinal stromal tumors (GISTs) are known to originate specifically from the intestinal cells of Cajal located in the gastrointestinal mesenchyme. GISTs developing outside of the digestive tract have barely been reported. We encountered a first case of large primary GISTs in the liver with cystic changes. A 63-year-old man with a past history of brain infarction visited our hospital. The computed tomography (CT) revealed a 6-cm and a 10-cm mass in the right and the caudal lobe of the liver, respectively. These tumors have marginal enhancement in the arterial phase; however, they presented as hypodense in the internal tumor sites. Both liver tumors had cystic changes. Gastrointestinal examinations using endoscopy revealed no other gastrointestinal tumors, and [18F]-fluoro-2-deoxy-D-glucose positron emission tomography/CT revealed multiple bone metastases in addition to the liver tumors. The liver tumor specimens were composed of spindle cells, and the immunohistochemical staining for c-Kit and for DOG1, as discovered on GIST, was positive. The patient was diagnosed with primary hepatic GIST with cystic changes.

Background. Thyroid carcinoma complicated by hemiagenesis is very rare, and previous reports have not described this cancer on the side of the absent lobe. Methods and Results. We report the case of a 64-year-old woman in whom left thyroid hemiagenesis was discovered incidentally during investigations of abnormal sensation during swallowing. A tumorous 1.4 cm lesion was also found on the side of the absent lobe, left of the isthmus. Fine-needle aspiration biopsy revealed class V papillary carcinoma, but no lymph node metastases. Total thyroidectomy was performed for stage cT1bN0M0 carcinoma. Histopathology revealed normal thyroid tissues in the right lobe and isthmus, while the left lobe was absent. The mostly papillary carcinoma was adjacent to the truncated thyroid tissue, with a portion histologically consistent with poorly differentiated carcinoma. Conclusions. All previously reported cases of thyroid cancer complicated by hemiagenesis have represented carcinoma occurring within the present lobe. This case is extremely rare.

Abstract Background Emphysematous pyelonephritis (EPN) with gas in the inferior vena cava (IVC) is a rare presentation and to our knowledge, this is the first case report in the urologic literature. Case presentation A 35-Year-old obese diabetic Hispanic female presented to the emergency room with a clinical picture of septic shock. Prompt computerized tomography scan revealed EPN with gas throughout the right renal parenchyma and extending to the right renal vein, IVC, and pulmonary artery. She died before surgical intervention Conclusion This case demonstrates that patients presenting with severe EPN have a high mortality risk and providers should acknowledge that septic shock, endogenous air emboli, or a combination of both could result in cardiovascular collapse and sudden death.
