Purpose. To evaluate of functional and cosmetic effectiveness of lower eyelid sling technique with fascia lata graft in patients with lagophthalmos due to facial paralysis.Material and Method. Ten patients with a mean age of55.1±19.77years who underwent lower eyelid sling surgery with a fascia lata graft between September 2011 and January 2014 were included in this prospective study. Preoperatively and postoperatively patients were evaluated in terms of corneal epithelial defects, Schirmer’s test, and tear break-up time (TBUT). Cosmetically, vertical eyelid aperture, margin reflex distances 1 and 2 (MRD1 and MRD2) and scleral show were evaluated preoperatively and postoperatively.Results. One patient had facial paralysis on the right side whereas the other 9 patients had facial paralysis on the left side. Preoperatively, 3 patients were detected with corneal ulcer, whereas 7 patients were detected with persistent corneal epithelial defects localized in the lower half of the cornea. In the 3 patients with preoperative corneal ulcer, the ulcer recovered with corneal opacity, whereas in the 7 patients with punctate epitheliopathy, postoperative corneal transparency was obtained.Discussion. Lower eyelid sling technique with fascia lata graft is an effective technique for the repositioning of the lower eyelid and preventing the corneal complications.

We present a case of a 75-year-old patient who underwent reconstruction of the lower eyelid and lateral canthus following removal of 80% of the lower eyelid due to basal cell carcinoma. A Hughes transconjunctival eyelid-sharing flap was used to form the posterior lamella, and a free skin graft from the ipsilateral upper lid was used to recreate the anterior lamella. The lateral canthal ligament was reconstructed using a free fascia lata graft. A periosteal flap was not used due to local scarring that was the result of previous multiple lateral canthal surgeries. Skin preservation in the lateral canthal area and additional horizontal support for the lower eyelid were achieved by using a local advancement flap. This two-stage surgery produced excellent functional and cosmetic effects. Fascia lata free graft can be an alternative to the periosteal flap for reconstruction of the lateral canthal ligament when use of the lateral orbital periosteum is not feasible.

Incidence of multiple flexor tendon rupture following distal radius fractures is rare with very few cases being reported in literature. We present an unusual case of a patient who had come to us with complaints of weakness and paresthesia of the right hand of one month prior and with a past history of dorsal plating for distal radius fracture nine years ago. Radiographs showed a distal radius fracture malunion with intact dorsal plate and protrusion of screws through the volar cortex. On exploration, attritional ruptures of all digital flexors were found with sparing of the Flexor Pollicis Longus tendon. The fibrous mass was excised and flexors reconstructed with a fascia lata graft. Attempt was made to correct the malunion with radial and ulnar osteotomies. At one year the patient had excellent restoration of digital flexion.

 Background: Temporohyoid osteoarthropathy (THO) is a progressive disease that causes acute onset of peripheral vestibular signs with or without facial paralysis. Ankylosis of temporhyoid joint occurs which predispose to fractures of the involved bones and consequently causes the commonly neurological signs observed. Clinical signs vary depending on the stage of the disease and the nerves affected. Surgical treatment is advised to improve survival rates in which the ceratohyoidectomy is currently known as the most advantageous. The aim of the present study is to report a case and outcome of a ceratohyoidectomy procedure in a Criollo mare presenting THO of the right temporohyoid joint.Case: A 17-year-old Criollo mare was referred to the Equine clinical hospital of the Federal University of Pelotas with a 5-day history of facial paralysis on the right side, head tilt and difficulty to chew and swallow. Auricular, palpebral and labial ptosis along with deviation of the lip and nostril to the left were observed. A corneal ulcer was also identified in the right eye. Complementary imaging exams (endoscopy of the guttural pouches and radiography of the head) were performed and showed thickening of the right stylohyoid bone confirming a diagnosis of THO. Anti-inflammatory and antibiotic therapy were administered and the corneal ulcer was treated with topical antibiotics and autologous serum. Due to rapid deterioration of clinical signs, the mare was referred to surgery. A ceratohyoidectomty procedure was performed under general anesthesia. In this procedure, the ceratohyoid bone was disarticulated from the ceratohyoid-basihyoid joint and removed. During the procedure, a branch of the linguofacial vein was accidentally incised causing hemorrhage, the branch was identified and successfully ligated. Recovery was uneventful. Supportive treatment with anti-inflammatory and antibiotics was continued after surgery and two sessions of electro-acupuncture was also performed to improve the nerve paralysis. The electro-acupuncture was discontinued due to mare’s negative behavior on needle insertion in the face. The treatment of the ulcer was changed since no improvement was observed in the first days. Twenty-eight days after hospitalization, the mare was discharged with the ulcer healed and significant improvement of neurological signs. A complete recovery occurred within three months.Discussion: The Criollo mare was referred to the hospital presenting mild neurological signs consistent with vestibular alteration and facial nerve paralysis. The THO diagnosis was confirmed using complementary imaging exams in which the endoscopy of the guttural pouch is considered the most common when computed tomography, a more sensitive one, is not available. Unilateral ceratohyoidectomy was performed as a surgical choice of treatment since it has a higher survival rate and lower recurrence rate in comparison to medical treatment and to stylohyoidectomy. As the main intraoperative complication, a vessel was accidentally incised, however this is described to occur in some cases. Despite that, the procedure was successfully performed and the mare had a complete recovery of the neurological signs and corneal ulcer. In conclusion, this report showed that it is important to have a complete diagnosis of these diseases and a consistent treatment plan to improve patient’s survival and quality of life.Keywords: neurologic disease, peripheral vestibular signs, facial paralysis, ceratohyoid bone, ceratohyoidectomy.

Dura mater grafts are commonly applied in neurosurgery. The bovine pericardium is safe and associated with rare complications. We report a case of a patient presenting with tuberculum sellae meningioma and unusual complication after frontotemporal duroplasty using bovine pericardium. After two days of good postoperative evolution, the patient presents with neurological worsening and CT scan shows intraparenchymal hematoma on the right, with perilesional edema, requiring surgery to drain and replace the pericardium of a fascia lata graft. with good postoperative evolution. Pediculated pericranium, temporal fascia and fascia lata are the most effective and safe choice to replace the dura mater. Among heterologous grafts, bovine pericardium is associated with few complications. However, complications such as inflammationand infection are related to the substance glutaraldehyde, present in such graft.

Equinus deformity is a condition in which dorsiflexion movement of the ankle joint is limited or decreased. Equinus deformity arises due to contracture/shortening/tightening in achilles tendon or soleus muscles or gastrocnemius muscle. The lengthening procedure of the achilles tendon can be done by various techniques such as the open or percutaneous method and Z-plasty versus sliding technique. Here we reported a case of post-traumatic equinus deformity of the right foot. The patient’s dorsiflexion movement was restricted. In this case, the patient was operated for open Z-plasty of the achilles tendon with rollover tensor fascia lata graft for lengthening of the contracted achilles tendon. The patient was discharged on the third postoperative day uneventfully. After 7 months of follow-up, the patient was walking normally without limping with the full range of motion at the right ankle joint as similar to his left ankle joint.

A five-year-old male domestic shorthair cat has been referred with a two-week history of lameness affecting the right hind limb. Gait assessment revealed an involuntary flexion of the stifle during the stance phase with excessive hip extension and stifle flexion in the terminal phase of the stride. Physical examination and radiographs have been performed and were consistent with patellar ligament rupture. Surgical correction consisted of primary tendon tenorrhaphy, circumpatellar suture and fascia lata graft augmentation. Eight months following surgery, validated owner questionnaire confirmed an excellent outcome.

Fazio-Londe syndrome is a rare neurodegenerative disorder caused by riboflavin transporter deficiency. Clinical presentation includes variable hypotonia or muscle weakness, sensory gait ataxia, optic atrophy, sensorineural hearing loss, and bulbar palsies. We described the case of a 37-year-old patient with a suspected Fazio-Londe syndrome who was referred to us for oral incompetence with dysarthria and drooling and impaired facial expression. First, we provided static sling suspension of the lower lip with Silhouette sutures (Silhouette Lift). Due to recurrent sutures exposure, we had to remove the sutures and insert a bilateral fascia lata graft to resuspend the lower lip. Two months postoperatively, the patient had good oral competence and there were no complications. In our opinion, this is a simple and minimally invasive procedure that can restore the right lower lip position in challenging patients.

A 5-year-old Mongolian mare (Equus caballus Linnaeus, 1758) was observed to have corneal opacity and excessive ocular discharge. An ophthalmic examination revealed a moving thread-like cylindrical worm in the anterior chamber of the right eye. The parasite was successfully removed surgically. The worm was observed under light microscopy and confirmed as Setaria digitata by 12S rRNA gene amplification and sequencing. Phylogenetic analysis demonstrated similarity with Setaria digitata in the National Center for Biotechnology Information (NCBI) GenBank database isolated from other Asian countries. This report is the first confirmed case of equine ocular setariasis by molecular diagnosis in China, which may indicate its presence in livestock and promote research on its epidemiology.

Abstract Background Repair of a perforated tympanic membrane (myringoplasty) can facilitate normal middle ear function, resist infection, and help re-establish normal hearing. Autogenous graft materials are the most popular graft materials used in myringoplasty because of their easy acceptability by the body. This study is conducted to compare between temporalis fascia graft and fascia lata graft in myringoplasty for patients with tubo-tympanic dry perforation. Results A total of 60 patients with persistent dry tympanic membrane perforation were included in our study during the period from January 2018 to May 2020. Patients underwent myringoplasty with temporalis fascia (30 patients as group A) or fascia lata (30 patients as group B). Patients were scheduled for follow-up visits concerning graft status, ear discharge, and audiograms. The mean postoperative air-bone gap in group A was 17.5 ± 4 after 1 month and 8.6 ± 6.9 after 3 months, while in group B, the mean postoperative air-bone gap was 17.6 ± 4.9 after 1 month and 9.4 ± 7.5 after 3 months. There was 90% success in graft uptake in group A, while there was 80% success in group B. Conclusion Using temporalis fascia is still the best and most trustworthy technique of myringoplasty compared to fascia lata graft. However, fascia lata can be a good alternative to temporalis fascia especially in cases of revision myringoplasty, ears having large perforation, or near-total perforation where the chances of residual perforation are high because of the limited margin of remnant tympanic membrane overlapping the graft.
