Introduction. Nano-hydroxyapatite gel (NHG) has never been investigated for enamel remineralization. This study evaluated the effects of two concentrations of NHG on remineralization of an artificial carious lesion in comparison with nano-HA toothpaste (NHT) and fluoride varnish (FV). Materials and Methods. Carious lesions were prepared on 100 enamel samples and divided into 5 groups: FV, NHT, 20% NHG, and 30% NHG. One untreated (NT) group was left as control. The hardness of the surface was evaluated before, during, and after remineralization. Microhardness at various phases and the percent recovery of hardness (%HR) were determined and analyzed with ANOVA. Polarized-light micrographs (PLM) were evaluated for depth of the carious lesion. Results. Significantly different remineralization capability was indicated for tested agents ( p < 0.05 ). NHT was significantly capable of remineralization greater than NHG, FV, and NT ( p < 0.05 ). No noticeable difference in %HR between 20% NHG and 30% NHG ( p > 0.05 ) was found. Decreasing in the depth of caries lesion was notified by PLM as applying either NHT or NHG as greater than FV, with no reduction in the depth for NT. Conclusions. Nano-HA both in toothpaste and gel form was capable of remineralization better than fluoride varnish. Comparable remineralization of 20% versus 30% NHG was evidenced. NHG for both concentrations was recommended as a capable remineralizing agent for caries remineralization. Clinical Significance: This study indicated that an application of nano-HA gel is an attractive route to deliver the material and can be more effective and less toxic than conventional formulations and provide its effectiveness directly at the site of action, especially for a noncooperative young child and medicinally intimidated patients who may face with inconvenience in using toothbrush and toothpaste for hygiene control.

Objective: To investigate the effectiveness of a new fluoride varnish (Clinpro White Varnish, 3M Espe, Seefeld, Germany) with regard to the caries incidence within a 2-year period. Study design: A non-randomized sample of 400 children from the Kyffhäuser district (Thuringia, Germany) was divided into a fluoride group (FG, biannual application of fluoride varnish) and control group (CG, no intervention). (Non-)cavitated caries lesions were recorded using World Health Organization (WHO) and Universal Visual Scoring System (UniViSS) criteria. Parents were given questionnaires to gather information about their socio-economic status (SES). Non-parametric methods and binomial logistic regression were used for data analysis. Results: There was a significant increase in caries incidence in both groups. The number of non-cavitated carious lesions was significantly lower in the FG (mean 2.2; sd 2.3) compared with the CG (mean 2.9; sd 1.9). Initial statistical analysis revealed that fluoride varnish might prevent non-cavitated carious lesions. When including SES as a confounder into regression model, potential preventive effect was lost. Conclusions: This study underlines the importance of the multi-factorial etiology of caries and illustrates that the effectiveness of biannual fluoride varnish application was evident in non-cavitated carious lesions only.

Regenerative medicine-based approaches for caries treatment focus on biomimetic remineralization of initial carious lesions as a minimal invasive therapy. In vitro, self-assembling peptide P11-4 enhances remineralization of early carious lesions. To investigate the safety and clinical efficacy of P11-4 for treatment of initial caries, a randomized controlled single-blind study was conducted on children aged >5 y with visible active early caries on erupting permanent molars. Subjects were randomized to either the test group (P11-4 + fluoride varnish) or control group (fluoride varnish alone). Caries were assessed at baseline and at 3 and 6 mo posttreatment per laser fluorescence, a visual analog scale, the International Caries Detection and Assessment System, and Nyvad caries activity criteria. Intention-to-treat analyses were performed, and safety and clinical feasibility of the treatment approaches were assessed. Compared with the control group, the test group showed clinically and statistically significant improvement in all outcomes at 3 and 6 mo. The laser fluorescence readings (odds ratio = 3.5, P = 0.015) and visual analog scale scores (odds ratio = 7.9, P < .0001) were significantly lower for the test group, and they showed regression in the International Caries Detection and Assessment System caries index (odds ratio = 5.1, P = 0.018) and conversion from active to inactive lesions according to Nyvad criteria (odds ratio = 12.2, P < 0.0001). No adverse events occurred. The biomimetic mineralization facilitated by P11-4 in combination with fluoride application is a simple, safe, and effective noninvasive treatment for early carious lesions that is superior to the presently used gold standard of fluoride alone. By regenerating enamel tissue and preventing lesion progression, this novel approach could change clinical dental practice from a restorative to a therapeutic approach. This could avoid additional loss of healthy hard tissue during invasive restorative treatments, potentially enabling longer tooth life and thereby lowering long-term health costs ( ClinicalTrials.gov NCT02724592).

ABSTRACT Aim The aim of this systematic review was to assess the in vivo scientific evidence regarding the ability of a recently developed light fluorescence device, SoproLife® (Sopro-Acteon group, La Ciotat, France) in detecting occlusal carious lesions. The PubMed database was searched for in vivo trials that evaluated the validity of the SoproLife® camera for the detection of occlusal carious lesions. Among the 11 articles originally identified with the keyword “Soprolife,” only three articles were included. The three included surveys used the International Caries Detection and Assessment System (ICDAS)-II criteria as gold standard for the assessment of SoproLife® compared or not to other detection devices (DIAGNOdent® and Spectra Caries Detection Aid®). Two of the included studies reported only on permanent teeth or both primary and permanent teeth. The SoproLife® validity values varied markedly among studies with a sensitivity ranging between 0.43 and 0.95 and a specificity between 0.55 and 1. Interobserver reproducibility with the SoproLife® was reported in two of the three studies (0.98 and 0.72) and none of the studies reported about intraobserver reproducibility. No clear-cut conclusion can be made based on the three included clinical studies; further in vivo investigations are needed to confirm the validity of the SoproLife® camera in terms of detection of occlusal carious lesions. Further observations are described on the measurement of the incorporation of inorganic phosphate labelled with P32 into the inorganic phosphate of the adrenal gland to assess the immediate pituitary–adrenal response when cold acclimatized and non-acclimatized rats are exposed to more severe cold (2 hours at −5 °C). In rats acclimatized to cold by conditioning to 3 °C for 4 weeks, this immediate pituitary–adrenal response was considerably less than that in non-acclimatized rats maintained at room temperature (22 °C). The reduction in the immediate pituitary–adrenal response took 3 to 4 weeks to develop and persisted for 12 hours, but not for 4 days. Rats that were conditioned to −5 °C by exposures for 2 or 6 hours daily for 4 weeks showed no reduction in the immediate pituitary–adrenal response to more severe cold, but there was a significant decrease in this response in rats conditioned for 6 hours daily for 8 weeks.Rats acclimatized to cold by conditioning to 3 °C for 4 weeks showed greater survival when exposed to an environmental temperature of −15 °C than rats conditioned to 22 °C. Rats that were conditioned to −5 °C for brief daily periods (2 hours or 6 hours) for 4 weeks or 8 weeks also survived exposure to severe cold (−22 °C) better than rats maintained at room temperature.In general, significant increases in adrenal weight were found in those cold-conditioned rats that showed a reduced pituitary–adrenal response. However, it is concluded that the development of increased survival on exposure to severe cold, by a process of conditioning to less severe cold, is not necessarily accompanied by a reduction in the immediate pituitary–adrenal response to severe cold, or by an increase in weight of the adrenal glands.

Objective: To compare the effect of glass ionomer cement (GIC) and fluoride varnish (F-varnish) on artificial proximal caries in situ. Study design: Two 1×3 mm2 enamel slabs, divided into three 1×1 mm2 windows,were prepared from each proximal surface of 18 premolars. Each slab was distributed into a control area,an intact enamel area and a test area. Artificial lesions were created on the control and test areas by immersing in demineralizing solution for 24 hours. The test windows were either applied with GIC or F-varnish and the full slabs inserted into orthodontic brackets as carriers, which were then bonded to the buccal surfaces of the 6 upper posterior teeth of 6 volunteers using non-fluoride dentifrice for 30 days. There was a 1-week washout period in this crossover study. The middle specimens of the three windows were cross-sectioned to analyze the carious lesion area by a polarized light microscope and calculated with Image-Pro plus® software.Results: The average carious lesion area under GIC was significantly less than that under F-varnish(p&lt;0.05). Conclusion: GIC promotes more remineralization of artificial carious lesions on proximal surfaces than F-varnish.

ABSTRACT Introduction The detection of carious lesions in the initial stages of development is very important to prevent the occurrence of cavitation. Visual examination and the use of a dental probe, bite-wing radiography, and fiberoptic transillumination (FOTI) have long been recommended for this purpose. Visual examination and probing of suspected lesions are useful for detecting occlusal caries, but achieve no gain of sensitivity and might cause irreversible tooth damage. Bite-wing radiography helps to detect approximal lesions better than clinical examination and probing the lesion. But the diagnostic performance of bite-wing radiography at approximal and occlusal sites is different. The FOTI is a quick and inexpensive method that can enhance visual examination of all tooth surfaces. The aim of this in vivo study is to compare FOTI with bite-wing radiography and visual examination in the detection of approximal and occlusal caries. Materials and methods A total of 46 patients without missing teeth and dentures in the posterior region of jaws were examined for the evaluation of all premolar and molar teeth contacts. Three blinded practitioners examined the patients. First one evaluated radiologically, second one visually, and the last one evaluated with FOTI. Results According to statistical results, bite-wing radiograph had the highest sensitivity. However, evaluation with FOTI had acceptable results to detect approximal caries.
