Spontaneous ovarian hyperstimulation syndrome (sOHSS) usually occurs in patients with a spontaneous ovulation cycle, especially in those with multiple pregnancies combined with hypothyroidism and polycystic ovary syndrome. sOHSS rarely occurs in women who are not pregnant. A 23-year-old woman with obvious abdominal distension visited our hospital. The patient was not pregnant and had not undergone controlled superovulation. Apart from abdominal distension, the patient denied any symptom of obvious incentives, abdominal pain, abnormal vaginal bleeding, or drainage. Biochemical analysis showed a high carbohydrate antigen-125 level and low total protein and albumin levels. Abdominal ultrasound and computed tomography showed a large amount of ascites and cystic uneven masses with an irregular shape in the area of the ovaries and fallopian tubes. Post-surgical histopathology indicated the diagnosis of sOHSS. Wedge resection of both ovaries was performed. Symptomatic treatment was further performed and the patient recovered well. Our findings indicate that sOHSS can occur in women who are not pregnant. Additionally, besides the follicle-stimulating hormone receptor gene mutation hypothesis, the pathogenesis of sOHSS should be further studied.

Development of ovarian hyperstimulation syndrome (OHSS) is very rare in a spontaneous ovulatory cycle and it is usually seen during pregnancy. In the etiology of OHSS, higher hCG (molar pregnancies or multiple pregnancies) and thyroid-stimulating hormone (TSH) levels have been accused. In recent years, some follicle-stimulating hormone (FSH) receptor (FSHR) gene mutations have been described in patients with OHSS in the first trimester with normal hCG levels. Herein, we report two cases of FSHR gene mutation during the investigation of the etiology of spontaneous OHSS. Although OHSS is typically associated with ovulation induction, it should be kept in mind that this condition may also develop in spontaneous pregnancies.

The ovary is a key tissue in the study of physiological neo-vascularisation in the adult and its study has highlighted important molecules involved in the regulation of angiogenesis in vivo. These include vascular endothelial growth factor, delta-like ligand 4, thrombospondin-1, prokineticin-1 and prostaglandin E2. Targeting these molecular pathways has therapeutic potential and their manipulation has an increasing preclinical and clinical role in the management of the pathological ovary. Targeting angiogenic pathways has utility in the promotion of ovarian angiogenesis to improve tissue and follicle survival and function as well as the prevention and management of ovarian hyperstimulation syndrome. There is a theoretical possibility that targeting angiogenesis may improve the function of the polycystic ovary and a real role for targeting angiogenesis in ovarian cancer.

Summary Background: Spontaneous ovarian hyperstimulation syndrome (sOHSS) can occur following hypothyroidism. Ultrasonography facilitates diagnosis and monitoring of this syndrome. We describe ovarian sonographic changes in a hypothyroid patient with sOHSS after treatment with levothyroxine (l-T4). Case presentation: A 15-year-old girl presented with abdominal pain and distension for a few months. On examination, she had classical features of hypothyroidism. Abdominal and pelvic ultrasound revealed enlarged ovaries with multiple thin-walled cysts and mild ascitic fluid. On follow-up, abdominal ultrasound showed significant reduction of ovary size after 6 weeks of initiation of l-T4. Normal ovary size with complete regression of ovarian cysts was seen after 4 months. Conclusion: Serial ultrasound in sOHSS associated with hypothyroidism showed regression of ovarian cysts and ovarian volume after 4 months whereas in other studies, it is reported to happen in various durations, presumably according to its etiology. Learning points OHSS can rarely occur due to hypothyroidism. This type of OHSS can be simply treated by l-T4 replacement, rather than conservative management or surgery in severe cases. Ultrasound follow-up shows significant regression of ovarian size and cysts within 6 weeks of initiation of l-T4. Ultrasound follow-up shows normal ovarian size with complete resolution of ovarian cysts 4 months after treatment.
