Pain is a subjective and multidimensional phenomenon. Diagnosis, assessment, and evaluation of pain are all complicated in children by the range of diagnoses and developmental levels, and by cultural influences. This chapter summarizes definitions and classifications of pain, including total pain. It looks at ways that children express pain, and measurement of pain severity. Pharmacological treatment of pain is considered, alongside the World Health Organization pain guidelines, which are considered to be the basis of managing pain in palliative care.

En el 2016, la OMS publicó sus directrices unificadas sobre el uso de los antirretrovirales (ARV) en el tratamiento y la prevención de la infección por el VIH. Como parte de esta revisión, la OMS reconoce que, a medida que se amplíe la escala del tratamiento antirretroviral (TAR) y que los países adopten la política de “tratar a todos”, será necesario diferenciar los servicios de TAR para proporcionar conjuntos de intervenciones de atención de salud adaptados a las personas con infección por el VIH con necesidades clínicas diversas. Con este fin, se definen cuatro grupos de personas: 1) las que acuden por primera vez o regresan en busca de atención presentando una infección avanzada por el VIH; 2) las que acuden por primera vez o regresan en busca de atención encontrándose bien desde el punto de vista clínico; 3) las que se encuentran clínicamente estables mientras siguen el TAR; y 4) las que reciben un esquema de TAR que no está siendo eficaz...El público destinatario de estas directrices está constituido principalmente por directores de programas nacionales del VIH, que son los responsables de adaptar estas nuevas recomendaciones a nivel del país. Las directrices son también pertinentes para los médicos clínicos y otros interesados directos, como las personas con infección por el VIH, organizaciones nacionales de la sociedad civil, asociados para la implementación, organizaciones no gubernamentales y financiadores nacionales e internacionales de los programas del VIH. Cicely Saunders married late in life and had no children. Her closest personal relationships were complicated. Her steely look and assured manner masked years of vulnerability, poor self-image, and struggles with her femininity. She was an unlikely pioneer of an improbable movement. Stripping away the hagiography, there is no doubt that Cicely shaped a new field of medicine which was gaining significant ground by the time of her death, and one which made further progress in the decade following it. A whole generation of palliative-care professionals was trained at St Christopher’s, many of whom spread their knowledge and expertise in other places. The hospice ideal transferred and translated around the world — and eventually led to universal support through the encouragement of the World Health Organization. This chapter concludes the book with an assessment of her legacy — and the complex and demanding life that shaped it.

Abstract Baseline CD4 testing rates declined from 73% to 21% between 2013 and 2018 with adoption of “Treat All” in Uganda. Advanced human immunodeficiency virus (HIV) disease (CD4 count < 200 cells/µL) remained common (24% of those tested in 2018, 83% of whom had World Health Organization stage I/II disease). Despite frequent presentation with advanced HIV disease, CD4 testing has declined dramatically.

It has been reported that pain relief for patients with cancer is suboptimal in Japan. This has been mainly attributed to inadequate dissemination of the World Health Organization (WHO) guidelines for cancer pain management. To better understand this problem, we reviewed how 6 hospital palliative care teams (HPCTs) used the WHO guidelines for unrelieved pain in a 1-year audit that included 534 patients. The HPCT interventions were classified according to the contents of the WHO guidelines. In our study, HPCT interventions involved opioid prescriptions in >80% of referred patients, and “For the Individual” and “Attention to Detail” were the 2 most important principles. Our study indicates which parts of the WHO guidelines should be most heavily emphasized, when disseminating them in Japan.
