Of 431 patients with gastric cancer observed in our Istitution, 23 (5.3 %) had early gastric cancer (EGC). Macroscopic presentation, histology, depth of invasion, and lymph node involvement were evaluated in all the cases. All patients underwent surgery and an intensive follow-up was performed. Five of the 23 patients progressed, and the risk factors were examined. Histology seemed to be the main prognostic factor in our study, since intestinal type of EGC was associated to a significantly better prognosis. Total gastrectomy is indicated in the proximal localization of EGC, and should perhaps be performed also in cases presenting undifferentiated histology.

e15068 Background: Despite radical oncologic resection with extended lymph node removal, patients with adenocarcinoma of the gastro-esophageal junction or stomach in UICC stage I show only a 5-year survival of 60-80% (Hölscher et al, 2009; Siewert et al. 1998). The aim of this retrospective study was to analyze the long-term survival of caucasian patients with early stage gastric cancer as for this population exist only sparse data. Patients with lymph-node involvement were not included as this parameter is a well-known negative prognostic marker. Methods: Tissue specimens and clinical data from patients with gastric cancer treated in the years 1993 to 2010 at the Charité, Berlin were collected retrospectively. Patients with stage T1 and T2 pN0M0 gastric cancer treated only by surgery including D1- and D2-lymphnode dissection were included in this study. Patients without relapse were followed-up for a minimum period of 24 months. Results: 97 patients (w = 36, m = 61, age 29-90 years) with a follow-up time from 6 to 208 months were identified. The 5-year survival was 94.85% (for details, see Table). Conclusions: The present data indicate a much better prognosis (5-year survival of 95%) of UICC I patients than previously described (60-80%). In harmony with other studies, our data demonstrate that R1, L1 or V1 resection seem to be a risk factor for recurrence whereas signet-ring differentiation was not found as a risk factor in our patient cohort. Ongoing work involves a broad panel of immunohistochemical markers to select prognostic expression profiles which help to identify patients with early gastric cancer at higher risk. Objective: Inflammatory markers are of prognostic importance in many malignancies. This study aimed to examine the effects of neutrophil-lymphocyte ratio (NLR), platelet-lymphocyte ratio (PLR), and pathological parameters on survival in preoperative complete blood counts in patients with operated gastric cancer. Material and Methods: Between 2012 and 2017, 281 patients were analyzed after total/subtotal gastrectomy. According to the ROC curve, we determined the cut-off values for NLR as 2.5 and PLR as 158. Overall survival (OS) was calculated from surgery to the last interview or to death. Results: In univariate analysis age ≥ 55 (p = 0.028), non-adeno hystology (p = 0.003), lenfovascular invasion (LVI) positivity (p = 0.003), perineural invasion (PNI) positivity (p < 0.001), T 3-4 stage (p = 0.006), lymph node involvement (LN) 2-3 (p < 0.001), metastatic stage (p <  0.001), NLR ≥ 2.5 ( p<0.001) and PLR ≥ 158 (p < 0.001) were statistically significant for OS. In multivariate analysis age (HR 0.652, 95% CI: 0.475-0.895; p = 0.008), PNI positivity (HR 0.493, 95% CI: 0.337-0.720; p<0.001), more lymph node involvement (HR: 0.608, 95% CI:0.412-0.896, p = 0.012), metastatic stage (HR 0.377, 95% CI: 0.265-0.537; p < 0.001) and PLR ≥ 158  (HR: 0.610; 95% CI: 0.433-0.859; p = 0.005) were found to be independent prognostic factors affecting OS. Conclusion: Age ≥ 55, PNI positivity, more lymph node involvement, metastatic stage, and PLR ≥ 158  are independent prognostic factors for shorter overall survival. Given the high morbidity and mortality of gastric cancer, besides classical known prognostic factors, parameters such as preoperative PLR may have benefits for forecast the prognosis of gastric cancer.

Endoscopic resection (ER) is the treatment of choice for early gastric cancer (T1) without lymph node involvement. An additional gastrectomy with D2 lymphadenectomy is recommended if ER is considered as non-curative. Here, we present a case of a robot-assisted sentinel lymph node procedure performed with the use of duel-tracer, including ICG fluorescence and technetium-99, after a non-curative ESD for an early gastric tumor. Five “hot” lymph nodes were resected, one of which was positive for metastasis. A subtotal gastrectomy with D2 lymphadenectomy was performed additionally during the same procedure. This case presentation indicates the feasibility of a robot-assisted sentinel lymph node procedure in early gastric cancer.
