Is people’s trust in doctors affected by their ethnicity or gender? It is often assumed that highly skilled migrants are protected from the worst effects of discrimination. Yet, they also report high degrees of discrimination. This study examines whether ethnic and gendered stereotypes influence trust in general practitioners in Norway. The question is investigated using a survey experiment. Respondents receive a brief resume of a general practitioner where the ethnicity and gender of the physician are randomly varied. Thereafter, the study focuses on investigating the interaction between the respondents' and fictive GPs' ethnicity and gender. The results show that while the majority population is indifferent to the ethnicity of the physicians, they prefer females. Non-Western minorities have a bias in favour of Pakistani-named GPs but are indifferent to the GP's gender.

<em>Background</em>. Our aim was to investigate the pattern of self reported symptoms and utilisation of health care services in Norway. <em>Design and methods.</em> With data from the cross-sectional Tromsø Study (2007-8), we estimated population proportions reporting symptoms and use of seven different health services. By logistic regression we estimated differences according to age and gender. <em>Results</em>. 12,982 persons aged 30-87 years participated, 65.7% of those invited. More than 900/1000 reported symptoms or health problems in a year as well as in a month, and 214/1000 and 816/1000 visited a general practitioner once or more in a month and a year, respectively. The corresponding figures were 91/1000 and 421/1000 for specialist outpatient visits, and 14/1000 and 116/1000 for hospitalisations. Physiotherapists were visited by 210/1000, chiropractors by 76/1000, complementary and alternative medical providers by 127/1000, and dentists by 692/1000 in a year. Women used most health care services more than men, but genders used hospitalisations and chiropractors equally. Utilisation of all services increased with age, except chiropractors, dentists and complementary and alternative medical providers. <em>Conclusions</em>. Almost the entire population reported health related problems during the previous year, and most residents visited a general practitioner. Yet there were high rates of inpatient and outpatient specialist utilisation. We suggest that wide use of general practitioners may not necessarily keep patients out of specialist care and hospitals.

AbstractResearchers claim that data in electronic patient records can be used for a variety of purposes including individual patient care, management, and resource planning for scientific research. Our objective in the project Integrated Primary Care Information (IPCI) was to assess whether the electronic patient records of Dutch general practitioners contain sufficient data to perform studies in the area of postmarketing surveillance studies. We determined the data requirements for postmarketing surveil-lance studies, implemented additional software in the electronic patient records of the general practitioner, developed an organization to monitor the use of data, and performed validation studies to test the quality of the data. Analysis of the data requirements showed that additional software had to be installed to collect data that is not recorded in routine practice. To avoid having to obtain informed consent from each enrolled patient, we developed IPCI as a semianonymous system: both patients and participating general practitioners are anonymous for the researchers. Under specific circumstances, the researcher can contact indirectly (through a trusted third party) the physician that made the data available. Only the treating general practitioner is able to decode the identity of his patients. A Board of Supervisors predominantly consisting of participating general practitioners monitors the use of data. Validation studies show the data can be used for postmarketing surveillance. With additional software to collect data not normally recorded in routine practice, data from electronic patient record of general practitioners can be used for postmarketing surveillance.

AbstractThis paper explores the relationship between migration and integration policies in the Netherlands, diaspora policies in India, and the transnational practices of Indian highly skilled migrants to the Netherlands. We employ anthropological transnational migration theories (e.g., Ong 1999; Levitt and Jaworsky 2007) to frame the dynamic interaction between a sending and a receiving country on the lives of migrants. This paper makes a unique contribution to migration literature by exploring the policies of both sending and receiving country in relation to ethnographic data on migrants. The international battle for brains has motivated states like the Netherlands and India to design flexible migration and citizenship policies for socially and economically desirable migrants. Flexible citizenship policies in the Netherlands are primarily concerned with individual and corporate rights and privileges, whereas Indian diaspora policies have been established around the premise of national identity.

A common belief is that African Americans are hyposensitive to pain compared to Whites, but African Americans show increased pain sensitivity in clinical and laboratory settings. The neurobiological mechanisms underlying these differences are unknown. We studied an ethnicity- and gender-balanced sample of African Americans, Hispanics and non-Hispanic Whites using functional magnetic resonance imaging during thermal pain. Higher pain report in African Americans was mediated by discrimination and increased frontostriatal circuit activations associated with pain rating, discrimination, experimenter trust and extranociceptive aspects of pain elsewhere. In contrast, the neurologic pain signature, a neuromarker sensitive and specific to nociceptive pain, mediated painful heat effects on pain report largely similarly in African American and other groups. Findings identify a brain basis for higher pain in African Americans related to interpersonal context and extranociceptive central pain mechanisms and suggest that nociceptive pain processing may be similar across ethnicities.

This book follows the transformations of the goddess Baijie, a deity worshiped in the Dali region of southwest China’s Yunnan Province, to understand how local identities developed in a Chinese frontier region from the twelfth century to the twenty-first. Dali, a region where the cultures of China, India, Tibet, and Southeast Asia converge, has long served as a nexus of religious interaction even as its status has changed. Once the center of independent kingdoms, it was absorbed into the Chinese imperial sphere with the Mongol conquest and remained there ever since. Goddess on the Frontier examines how people in Dali developed regional religious identities through the lens of the local goddess Baijie, whose shifting identities over this span of time reflect shifting identities in Dali. She first appears as a Buddhist figure in the twelfth century, then becomes known as the mother of a regional ruler, next takes on the role of an eighth-century widow martyr, and finally is worshiped as a tutelary village deity. Each of her forms illustrates how people in Dali represented local identities through gendered religious symbols. Taken together, they demonstrate how regional religious identities in Dali developed as a gendered process as well as an ethno-cultural process. This book applies interdisciplinary methodology to a wide variety of newly discovered and unstudied materials to show how religion, ethnicity, and gender intersect in a frontier region.
