SummaryPost-infusion hepatitis is known to occur very frequently in haemophiliacs after treatment with unheated commercial clotting factor concentrates, obtained from large plasma donation pool. On the contrary, single-donor cryoprecipitate is likely to carry a lower risk of transmitting hepatitis.To evaluate this hypothesis, we retrospectively reviewed the medical records of 25 first infused haemophiliacs (from 1981 to 1984) treated with unheated commercial clotting factor concentrates (n = 19) or cryoprecipitate (n = 6).The hepatitis-free interval after the beginning of therapy was expressed as exposure days. The end point of each patient, i.e. the hepatitis occurrence, was defined as an increase of aminotransferases (ALT and AST) and/or the seroconversion of HBV-markers, which were checked every three months.The life-table method and log-rank test showed that cryo-precipitates had a significantly longer hepatitis-free interval (p = 0.0131, log-rank test) and a lower risk of transmitting hepatitis (p = 0.01-0.05, life-table method) than the commercial concentrates. However, the safety of cryoprecipitate therapy was shown to cover only a few exposure days, and so the real advantage of this product depends on the bleeding frequency of the patient concerned.We believe that these methods and our findings may be useful to assess and compare the safety of the new “heat-treated” clotting factor concentrates.

Spatial information technology has been widely used for vehicles in general and for fleet management. Many studies have focused on improving vehicle positioning accuracy, although few studies have focused on efficiency improvements for managing large truck fleets in the context of the current complex network of roads. Therefore, this paper proposes a multilayer-based map matching algorithm with different spatial data structures to deal rapidly with large amounts of coordinate data. Using the dimension reduction technique, the geodesic coordinates can be transformed into plane coordinates. This study provides multiple layer grouping combinations to deal with complex road networks. We integrated these techniques and employed a puncture method to process the geometric computation with spatial data-mining approaches. We constructed a spatial division index and combined this with the puncture method, which improves the efficiency of the system and can enhance data retrieval efficiency for large truck fleet dispatching. This paper also used a multilayer-based map matching algorithm with raster data structures. Comparing the results revealed that the look-up table method offers the best outcome. The proposed multilayer-based map matching algorithm using the look-up table method is suited to obtaining competitive performance in identifying efficiency improvements for large truck fleet dispatching.

Abstract Purpose Osteotomies represent well-established treatment-options for the redistribution of loads and forces within and around the knee-joint. Effects of these osteotomies on the remaining planes and adjacent joints are not fully understood. The aim of this study was to determine the influence of a distal-femoral-rotation-osteotomy on the coronal alignment of the ankle. It was hypothesized that supracondylar-external-rotation-osteotomy of the distal femur leads to a change in the coronal orientation of the ankle joint. Methods Long-leg standing radiographs and CT-based torsional measurements of 27 patients undergoing supracondylar-rotational-osteotomy of the femur between 2012 and 2019 were obtained and utilized for the purpose of this study. Postoperative radiographs were obtained after union at the osteotomy-site. The hip-knee-ankle-angle (HKA), the mechanical-lateral-distal-femur-angle (mLDFA), and Tibia-Plafond-Horizontal-Orientation-Angle (TPHA) around the ankle were measured. Comparison between means was performed using the Wilcoxon-Mann–Whitney test. Results Twenty-seven patients with high femoral antetorsion (31.3° ± 4.0°) underwent supracondylar-external-rotation-osteotomy. The osteotomy led to a reduced antetorsion (17.4 ± 5.1; p < 0.001) and to a valgisation of the overall limb-alignment. The HKA decreased by 2.4° ± 1.4° (p < 0.001). The TPHA decreased by 2.6° (p < 0.001). Conclusions Supracondylar external rotation osteotomy of the femur leads to lateralization of the weight bearing line at the knee and ankle due to valgisation of the coronal limb alignment. The mobile subtalar joint has to compensate (inversion) for the resulting valgus orientation of the ankle to ensure contact between the foot and the floor. When planning a rotational osteotomy of the lower limb, this should be appreciated – especially in patients with a preexisting valgus alignment of the lower extremities or restricted mobility in the subtalar joint.

The present study explores CFD analysis of a supercritical carbon dioxide (SCO2) radial-inflow turbine generating 100kW from a concentrated solar resource of 560oC with a pressure ratio of 2.2. Two methods of real gas property estimations including real gas equation of estate and real gas property (RGP) file - generating a required table from NIST REFPROP - were used. Comparing the numerical results and time consumption of both methods, it was shown that equation of states could insert a significant error in thermodynamic property prediction. Implementing the RGP table method indicated a very good agreement with NIST REFPROP while it had slightly more computational cost compared to the RGP table method.
