Patients with myocardial infarction (MI) and atrial fibrillation (AF), the number of which is progressively increasing every year, make up a high-risk group for both recurrent cardiovascular events and bleeding; they require special attention from clinicians. The literature review provides data on features of pathogenesis and clinical manifestations of MI in patients with AF. The analysis of data on AF effect observational studies on short-term and long-term prognosis in patients with myocardial infarction was carried out. Mechanisms of occurrence, clinical features and prognostic value of postinfarction AF are considered. From the standpoint of modern clinical guidelines, information is presented on features of MI invasive treatment in combination with AF. Algorithms of anticoagulant and antiarrhythmic therapy in patients of this category are considered.

Some of the indications for anticoagulant therapy remain controversial, but they are widely used for the prevention and treatment of thrombosis and embolism after surgery1 and after myocardial infarction,2 in patients with atrial fibrillation, and in those with transient cerebral ischaemia.3 This article discusses the drugs available for long-term use. Those suitable for immediate and short-term use, heparin and arvin, will be discussed in the next issue.

Rising health care insurance premiums have forced many businesses and institutions to examine cost-cutting measures to lower health care costs. Managed care is one measure that has become widespread throughout the United States. The problem was to learn how managed care administrators view the role and purpose of vocational rehabilitation services in their organizations and if these services will lower short-term and long-term use. A survey was sent to the administrators of fifty managed care organizations in the states of Arizona, Colorado, Oregon, and Washington. Twenty of the fifty administrators responded to the survey. Results show that only one of the managed care organizations offered vocational rehabilitation services to their enrollees. The results also show that three responded that short-term use and ten responded that long-term use of managed care programs would be reduced if vocational rehabilitation services were offered.
