Service innovations: The role of a consultant in old age psychiatry

There is increased recognition that the role and function of a consultant psychiatrist is ill-defined and associated with excessive workloads, low job satisfaction, high levels of stress and high rates of premature retirement (Kennedy & Griffiths, 2001). This has led to an examination and debate about how consultants in general psychiatry could adapt models of working to address these difficulties, and also face the agenda of change facing the NHS as a whole and the mental health services in particular (Kennedy & Griffiths, 2001; De Silva & Sutcliffe, 2003). These challenges are not, of course, unique to general psychiatry, but as yet, there has been little debate about how consultants in other specialities, including old age psychiatry, could begin to try and address these difficulties. This article aims to stimulate debate, by describing an adapted model of working adopted by two consultants in old age psychiatry within the Hull and East Riding Community Health NHS Trust.

The current National Health Service (NHS) approach to commissioning health services is in flux. The purchasing of care from providers by general practitioner fundholders (GPFHs) and health authorities has changed with the new White Papers. GPFHs no longer exist and the commissioning role is being handed over from health authorities to primary care groups (PCGs). An understanding of the reasons for change and current arrangements will aid the consultant psychiatrist in influencing this process.

IHC is one form of community-sourced health efforts (UKBM) managed from, by, for and with the community, to empower the community and provide facilities for the community to obtain basic health services. Efforts to improve the role and function of IHC are not merely the responsibility of the government, but all components in the community, including cadres. The role of cadres in the implementation of IHC is very large because in addition to being a provider of health information to the community to come to the IHC and carry out clean and healthy life behaviors. Linggasari is one of the villages in Ciamis Sub-District, Ciamis Regency, which has good potential about the existence of IHC. The number of IHC in Linggasari Village are 8 units. Cempaka Putih is one of the IHC in Linggasari Village. The IHC is still in progress, which requires assistance. Based on the situation, it is necessary to have assistance for the development of it by conducting community service, namely through fostering and counseling cadres on the management of IHC that is carried out from November 27, 2017 to January 03, 2018.

In the currant climate of the implications of the Community Care Act and health service reforms, defining the role and function of the psychogeriatric day hospital is crucial, particularly its distinction from day centre care. This paper describes this role and key issues in day hospital planning. It was presented to the Wessex Specialists in Old Age Psychiatry and accepted by them as a consensus document.

This chapter describes an interview with Dr Nori Graham, a key figure in older people’s mental health, former Chairman of the Alzheimer’s Society and of Alzheimer’s Disease International, and an inspiration to generations of doctors. This reflective interview provides a first-person account of the development of mental health services for older people over the last 40 years, paying tribute to some of the pioneers and architects of the astonishing growth of old age psychiatry services during this period, and comparing the challenges then and now. Nori Graham also provides insights and advice on how to develop a successful career, and on dealing with the challenges for women doctors with children. Finally, she describes her work in helping to build up the Alzheimer’s Society and Alzheimer’s Disease International, both of which expanded considerably during her period as Chairman.

The introduction of mental health services into a juvenile correctional facility should be conducted by a well-trained child psychiatrist with personal qualities that will enable him to get actively involved in a dynamic correctional program. He should have a series of preliminary discussions with the institutional administration to clarify mutual expectations and to dispel mis conceptions and apprehensions on the part of the administra tion, the institution staff, and the pyschiatrist. Other matters to be covered in initial meetings are the psychiatrist's contract concerning hours, salary, lines of communication, role, and function. While the psychiatrist will be called upon to diagnose, treat, and commit certain cases, there seems to be a growing demand for him to function as consultant and educator.

ABSTRAKPelabuhan berperan sebagai fasilitas penunjang pusat pertumbuhan regional dalam proses pembangunan ekonomi wilayah. Pelabuhan Bebas Sabang diarahkan sebagai pusat pertumbuhan ekonomi regional dan diharapkan dapat meningkatkan perekonomian Kawasan Sabang. Permasalahan yang dihadapi Pelabuhan Bebas Sabang adalah belum optimalnya peran dan fungsi Pelabuhan Bebas Sabang dalam menunjang perekonomian wilayah. Penelitian ini bertujuan untuk mengetahui peran Pelabuhan Bebas Sabang dalam mendorong perkembangan perekonomian Kawasan Sabang. Lingkup materi yang dibahas mencakup peran-peran Pelabuhan Bebas Sabang, menentukan potensi dan masalah serta upaya-upaya peningkatan peran Pelabuhan Bebas Sabang. Metode analisis yang dilakukan adalah analisis deskriptif dengan pendekatan analisis data kualitatif dan kuantitatif. Alat analisis yang digunakan adalah analisis SWOT IFAS-EFAS. Hasil analisis menunjukkan dalam kurun waktu 4 (empat) tahun terakhir dari tahun 2010-2013, Pelabuhan Bebas Sabang belum optimal dalam menjalankan perannya, sehingga membutuhkan strategi pengembangan dengan pendekatan Agressive Maintenance Strategy (strategi perbaikan agresif), yaitu strategi konsolidasi internal dengan memperbaiki faktor-faktor kelemahan untuk memaksimalkan pemanfaatan peluang.Kata kunci: Pengelolaan, SWOT IFAS-EFAS, WilayahABSTRACTPort was supporting facility of regional growth center in the process of regional economic development. Sabang free port was directed as the center of regional economic growth and expected to raise the economy of sabang. Problems faced by sabang free port was yet optimal role and function in supporting the economy of the region. This study aimed to determine the role of sabang free port in supporting the economic development of sabang. The covered material scope included roles of sabang free port, determining the potentials and problems and efforts of increasing the role of sabang free port. The method of analysis was descriptive analysis with qualitative and quantitative approach. The analytical tool used was the swot ifas-efas analysis. The analysis results showed in the period of 4 (four) years from 2010 until 2013, sabang free port was not optimal in carrying out its role yet, so it requires development strategies with agressive maintenance strategy approach, which is internal consolidation strategy by improving vulnerability factors to maximize the utilization of opportunities.Keywords:, Management, Regional, SWOT IFAS-EFAS

This chapter discusses contemporary debates regarding the use of artificial intelligence as a vehicle for criminal justice reform. It closely examines two general approaches to what has been widely branded as “algorithmic fairness” in criminal law: the development of formal fairness criteria and accuracy measures that illustrate the trade-offs of different algorithmic interventions; and the development of “best practices” and managerialist standards for maintaining a baseline of accuracy, transparency, and validity in these systems. Attempts to render AI-branded tools more accurate by addressing narrow notions of bias miss the deeper methodological and epistemological issues regarding the fairness of these tools. The key question is whether predictive tools reflect and reinforce punitive practices that drive disparate outcomes, and how data regimes interact with the penal ideology to naturalize these practices. The chapter then calls for a radically different understanding of the role and function of the carceral state, as a starting place for re-imagining the role of “AI” as a transformative force in the criminal legal system.
