Objective: A new technique of direct phonoplasty under local anaesthetic is described.Background: Vocal-fold medialization is an accepted method to improve voice in patients with a paralysed vocal fold. Various materials have been used to inject into the paralysed vocal fold, resulting in medialization of the fold and improved phonation. The two established methods are direct injection under general anaesthetic and transcutaneous injection under local anaesthetic; both have advantages and disadvantages. Method: A Pentax 80K series flexible endoscope is used to directly inject collagen into the paralysed vocal fold via a reusable 23G endoscopic needle under local anaesthetic. Conclusion: New technology and innovation have allowed development of the new technique of direct phonoplasty under local anaesthetic. This combines the advantages of transcutaneous injection under local anaesthetic and direct injection under general anaesthetic, without their disadvantages.

The actively prestressed strengthening technique of CFRP plate is a kind of strengthening technology with great application in the future. After a comparison and conclusion of the advantages and disadvantages of various prestressed tensile techniques for CFRP plate from home and abroad, based on the WSGG anchor, this paper puts forward the hinged anchor and a new technique where the hinged anchor tensions in vertical direction, the biggest characteristic of this technique is to ensure force uniformity of CFRP plate, consequently, high strength feature of CFRP plate can be fully developed. In addition, a series of improvement have been achieved, such as on-site baiting, fast clamping and rapid tensioning, etc. of CFRP plate. Hinged anchor and its prestressed tensile technique are firstly applied for the reinforced projects of Chongqing Xiaojia River Bridge and Chongqing Lianhua River Bridge, which promote rapid strengthening for the old bridges successfully.

AbstractObjectives:To study the effect of a new technique for closed reduction of dislocated arytenoids.Methods:The study included 21 females (72.4 per cent) and eight males (27.6 per cent) with a diagnosis of arytenoid dislocation. There were 18 cases of left arytenoid dislocation and 11 of right arytenoid dislocation.Twenty-eight cases had anteromedial dislocation and one had posterolateral dislocation. All patients were treated with closed reduction using custom-made metal rods and an operating microscope, under general anaesthesia.Results:Post-operatively, 21 patients’ voices returned completely to normal (including full vocal fold mobility), five had an improved voice and three failed to show any improvement. There were no post-operative complications.Conclusion:This new closed reduction technique is a safe procedure for patients with dislocated arytenoids. It is repeatable and the outcome is reliable and effective. Closed reduction can be a useful treatment for arytenoid dislocation.

Piezosurgery is a relatively a new technique of bone surgery, that is recently gaining popularity in implantology, periodontics and oral surgery. Piezoelectric ultrasonic vibrations are utilized to perform precise and safe osteotomies. Becauseof its highly selective and accurate nature, with its cutting effect exclusively targeting hard tissue, its use may be extended to more complex oral surgical procedures, as well as to other interdisciplinary problems. It can be used for selective cutting of bone depending on bone mineralization, without damaging the adjacent soft tissue (e.g. vessels, nerves or mucosa), providing a clear visibility in the operating field, and cutting with sensitivity without the generation of heat. So this review discusses the equipment, mechanism of action biological effects on bone, indications, contraindications, advantages and disadvantages of this new technology.

Insertion of J stent is a very common procedure performed by urologists. This procedure is often performed to relieve ureteral obstruction due to stones, tumours, ureteral kink, or stricture on urgent basis. An increasing number of urologists are using a flexible cystoscope and local anaesthetic for J stent insertion. Occasionally, the guide wire curls in the bladder, making it impossible to insert the J stent. Here, we describe a simple method to overcome this problem using modified ureteral dilator, which is usually available in the cystoscopy suite.

AbstractBlom—Singer valve prosthesis is an increasingly popular technique for voice rehabilitation in alaryngeal patients. Although primary voice puncture is being practised, the creation of the tracheo-oesophageal fistula is performed in the majority of patients as a secondary procedure.We describe a technique of secondary tracheo-oesophageal puncture using the flexible endoscope which can be performed under local anaesthetic and sedation. The technique overcomes the difficulty of passing a rigid oesophagoscope or forceps to the level of the tracheostome in a scarred and irradiated neck. The new technique also obviates the need for general anaesthesia.We have successfully used the technique in three patients.

Desde hace unos pocos años se ha comenzado a hablar de vídeo digital. Para algunos es sólo un proceso técnico que se aplica a un medio conocido, para otros es algo más. Este artículo trata de profundizar en algunas ideas clave en relación al vídeo digital así como mostrar algunas aplicaciones que muestran su potencial de uso educativo.
