Aim. To estimate the relationships between the serum concentrations of acute-phase proteins (APPs) and adipocytokines, body composition (BC), and blood glucose (BG) fluctuations in women with type 2 diabetes mellitus (T2DM). Subjects and methods. A total of 165 women with T2DM and 22 with a normal body mass index (BMI) at the age of 40 to 70 years were examined. The concentrations of high-sensitivity C-reactive protein (hs-CRP) and acid α1-glycoprotein (α1-AGP) were determined by ELISA. The levels of interleukins 6, 8, and 18 (IL-6, IL-8, IL-18), tumor necrosis factor-α (TNF-α), and plasminogen activator inhibitor type 1 (PAI-1) were measured by a multiplex analysis. Dual energy X-ray absorptiometry was used to estimate BC parameters. BG fluctuations were estimated via continuous glucose monitoring. Results. The levels of hs-CRP, α1-AGP, IL-6, IL-8, IL-18, TNF-α, and PAI-1 were significantly higher in the obese women with T2DM than those in the control group. In the diabetic normal weight women, only hs-CRP, α1-AGP, and IL-8 concentrations exceeded those in the controls. The level of hs-CRP (other than α1-AGP) correlated positively with BMI, the mass of adipose tissue, body trunk (android), and gynoid fats. A multivariate regression analysis showed that adipose tissue mass and trunk fat proportion were independent predictors of hs-CRP levels. The concentrations of IL-6, IL-8, IL-18, PAI-1, and TNF-α correlated positively with waist-to-hip ratio, but demonstrated no associations with BMI and BC. Only the serum α1-AGP level showed a positive association with mean BG and its variability parameters. Conclusion. In the women with T2DM, the serum concentrations of APPs and adipocytokines correlate differently with the mass of adipose tissue, its distribution, and BG fluctuations. The findings indicate the multifactorial genesis of chronic inflammation in these patients.

We aimed to compare the level of agreement between leg-to-leg bioelectrical impedance analysis (LBIA) and dual-energy X-ray absorptiometry (DXA) for assessing changes in body composition following exercise intervention among individuals with Type 2 diabetes mellitus (T2DM). Forty-four adults with T2DM, age53.2±9.1years; BMI30.8±5.9 kg/m2participated in a 6-month exercise program with pre and post intervention assessments of body composition. Fat free mass (FFM), % body fat (%FM) and fat mass (FM) were measured by LBIA (TBF-300A) and DXA. LBIA assessments of changes in %FM and FM post intervention showed good relative agreements with DXA variables (P<0.001). However, Bland-Altman plot(s) indicated that there were systematic errors in the assessment of the changes in body composition using LBIA compared to DXA such that, the greater the changes in participant body composition, the greater the disparity in body composition data obtained via LBIA versus DXA data (FFM,P=0.013; %FM,P<0.001; FM,P<0.001). In conclusion, assessment of pre and post intervention body composition implies that LBIA is a good tool for assessment qualitative change in body composition (gain or loss) among people with T2DM but is not sufficiently sensitive to track quantitative changes in an individual’s body composition.

Background. Type 2 diabetes mellitus is characterized by a violation of the incretin effect, in particular a decrease in the secretion of glucagon-like peptide-1 (GLP-1) by intestinal endothelial cells. In recent decades, the intestinal microbiota has been shown to play a key role in the regulation of various metabolic pathways, immune system activity, and intestinal permeability. It has been shown that the composition of bacterial genera in the intestine can unfluence the effectiveness of antidiabetic drugs (eg metformin and GLP-1 receptor agonists), which may be reduced in dysbiosis. Therefore, it is of interest to study the mechanisms that mediate the effect of microbiota on the incretin secretion. The purpose was to establish the relationship between the effects of probiotic therapy, incretin therapy and the level of endogenous GLP-1 in the serum of patients with type 2 diabetes mellitus, taking into account anthropometry and body composition. Materials and methods. We examined 23 patients with type 2 diabetes mellitus (11 women and 12 men), their average age was 56.4 ± 10.5 years (M ± SD). At the beginning of the study, the mean HbA1c level was 7.7 ± 1.5 %; all patients took metformin at an average dose of 1,500 mg/day. Patients were randomized into 2 groups: group 1 (n = 11) received a probiotic, group 2 (n = 12) — a long-acting GLP-1 receptor agonist. The concentration of GLP-1 in the blood serum was determined by the enzyme-linked immunosorbent assay, anthropometry parameters and body composition were assessed using the Tanita analyzer. Results. In the group of patients who took the probiotic, a significant increase in GLP-1 was observed, but less pronounced compared to an increase in GLP-1 level in the group of patients who took GLP-1 receptor agonists. In group 2, on the background of taking GLP-1 receptor agonists, a significant decrease in body weight, total and abdominal fat content, and a decrease in dehydration were revealed. Conclusions. An increase in the concentration of endogenous GLP-1 against the background of probiotic therapy indicates a possible positive effect of normalization of the intestinal microbiota on the secretion of endogenous incretins. The results obtained suggest that the use of a combination of probiotic and GLP-1 receptor agonists may have an additive effect on the hormonal and metabolic profile in patients with type 2 diabetes mellitus.
