Summary. Retro- and prospective trial was based on data about 30 patients, suffering entero-colocutaneous fistula, who were treated in the Shalimov National Institute of Surgery and Transplantology during 2016-2019.  There was revealed, that the most informative phenotypical markers of undifferentiated dysplasia of the connective tissue (UDCT) in patients, with entero-colocutaneous fistula, are visceral (83,3%), vascular (70%), arrhythmic (70%) syndromes. There was founded, that direct correlation between the level of biochemical markers of the collagen biodegradation and the UDCT degree may be applied for prognostication of development and course of complications in patients, suffering entero-colocutaneous fistula. Severe degree of UDCT in the patients, entero-colocutaneous fistula, constitutes unfavorable prognostic sign and enhances the mortality by 62.5%. The presence of UDCT in the patients winh entero-colocutaneous fistula is an aggravating comorbid factor that is difficult to be treated and accompanied by high rates of lethality, which must be taken into account, choosing the adequate surgical tactics and complex pathogenetically substantiated treatment.

After clinical and neurophysiological examinations of 22 patients with developmental anomalies of the rectum and anus, who suffered of fecal incontinence of type III and who were operated in the Vladimirsky Moscow Region Clinical Institute, a modern approach to the diagnostics of neuromuscular disorders has been proposed. A particular attention was paid to the sacral plexus and its branches. A detailed examination of the sexual nerve and its functions was made using a St. Mark disposable rectal electrode. The optimal evaluation of motor potentials from the thigh muscles under transrectal stimulation of the sacral plexus was made; findings of EMG diagnostics were compared too. The researchers backgrounded an optimal modality for treating this pathology: not only surgical tactics, but also patient’s reserve capabilities in restoring lost functions using EMG biological feedback.

Aspects of ultrasound diagnostics of extratubal forms of ectopic pregnancy are presented from the standpoint of choosing the optimal treatment tactics for patients, aimed primarily at minimiz-ing surgical procedures. The issues of diagnostics of cervical pregnancy, caesarean scar preg-nancy, ovarian pregnancy, as well as a rare indication of tubal pregnancy – interstitial pregnancy are detailed. In most cases, echography is not only decisive, but, in fact, the only method of pre-operative diagnosis. In this case, color Doppler mapping of blood flow in the vessels of the ec-topic trophoblast is of great importance, which makes it possible to assess the degree of its inva-sion and, thereby, to make adjustments to the surgical tactics. A literature review is devoted to the current classifications of liver failure that occurs in obstructive jaundice. Modern methods of diagnosis and criteria for determining the time of development and assessing the severity of this disease, model assessment and prognosis of acute liver failure are also considered. Attention is paid to assessing the severity and determining the timing of the development of liver failure in extrahepatic cholestasis. In addition, there is no common understanding of the role of liver compensatory mechanisms involved in the development of this condition. There are no generally accepted views on the surgical tactics of treatment of patients with obstructive jaundice complicated by liver failure. The limits of application of step-by-step treatment of patients with neoplastic obstructive jaundice have not been reliably determined. All these issues require further research, search for universal tools for assessing liver failure, predicting postoperative complications for choosing the optimal surgical treatment tactics.

Summary. Inflammatory lesion of the sternoclavicular joint (SCJ) is a rare disease that requires surgical treatment. Traditional radiography reveals osteolysis and osteosclerosis, however, at the onset of the disease, such changes are absent. Difficulties in diagnosing this disease require further study of the problem and the development of new therapeutic and diagnostic approaches. Aim of the work: to study the role of sonography in the diagnosis of purulent arthritis of SCJ and the choice of treatment tactics. Materials and methods. Prospective single-center study of 28 cases of acute nonspecific arthritis of SCJ. The patients were divided into 2 groups: the first included patients who received drug therapy, the second included patients subject to surgical treatment. Results and discussion. Ultrasound scanning of SCJ identified 3 sonographic variants of the lesion: exudative, destructive with synovial hyperperfusion, and destructive with synovial hypoperfusion. The duration of hospital stay in both groups did not differ statistically: 12 (12; 13.5) vs 14 (12; 14) days, p = 0.194. Analysis of long-term results in patients of the first group established the progression of the disease with the development of osteomyelitis or recurrence of arthritis in 3 cases (30 %). Among the patients of the second group, no complications or recurrence were detected during the indicated observation period. Conclusion. Sonography is diagnostically significant in the diagnosis of SCJ arthritis. It allows you to determine the group of patients subject to mandatory surgical correction. Intravenous drug addiction associated with bacteremia is a predictor of poor drug treatment outcomes and an indication for active surgical tactics.

Lung cancer is the most commonly diagnosed cancer in the world, and currently the mortality rate from this disease is one of the highest. The detection of EGFR mutation plays an important role in the choice of treatment in clinical practice. As already known, over the past decade, EGFR mutations have been a predictive factor for the prescription of tyrosine kinase inhibitors (EGFR TKIs) in patients with advanced non-small cell lung cancer (NSCLC). The role of EGFR mutation as a prognostic factor for patients undergoing surgery and the possibility of using surgery to overcome the resistance mechanisms that develop in response to EGFR TKIs remain unexplored. This literature review examines key aspects of the EGFR mutation as a factor that can influence the treatment and using the surgical method.

AbstractWhitehouse adapts insights from evolutionary anthropology to interpret extreme self-sacrifice through the concept of identity fusion. The model neglects the role of normative systems in shaping behaviors, especially in relation to violent extremism. In peaceful groups, increasing fusion will actually decrease extremism. Groups collectively appraise threats and opportunities, actively debate action options, and rarely choose violence toward self or others.
