ABSTRACT Breath-hold diving is a common practice as a part of military dive training. An association between prior lung injury and a propensity for lung barotrauma may have the potential to impact mission readiness for combat divers, Pararescue, Combat Controllers, Army Engineer divers, and various units in Naval Special Warfare and Special Operations. Barotrauma is a common complication of diving, typically occurring at depths greater than 30 m (98.4 ft). Individuals with abnormal lung anatomy or function may be at increased risk of barotrauma at shallower depths than those with healthy lungs, rendering these service members unfit for certain missions. We describe the case of a 25-year-old male, with a remote history of polytrauma and resultant pulmonary pleural adhesions, whose dive training was complicated by lung barotrauma at shallow depths. In missions or training utilizing breath-hold diving, the association with secondary alterations in lung or thoracic anatomy and function may limit which service members can safely participate.

The sinus of valsalva aneurysm (SVA) is a small dilatation caused by a separation between the aortic media and annulus fibrosus. It may be either acquired or congenital. The right coronary sinus is most frequently affected, with the most common complication being rupture of the sinus. Cardiac failure and sudden auscultation changes suggest the possibility of SVA rupture. Echocardiography is sufficient to diagnose SVA, its complications, repercussions and to determine surgical options. SVA, even if asymptomatic, has potential risks of expansion, rupture, cardiac failure, endocarditis, embolism and sudden death. This justifies surgical correction which has a low mortality rate in both the short and long-term.Bangladesh Med J. 2015 Sep; 44 (3): 175-177

BACKGROUND: Pain is the most common complication of fibroids in pregnancy and can be difficult to treat. The choices of pain relief in pregnancy are limited due to myriad risks including miscarriage, teratogenicity, premature birth, and low birth weight. CASE REPORT: This paper describes the analgesic challenges faced when managing severe pain in the antepartum period for a woman pregnant with twins who also suffered from uterine leiomyomas. Multiple analgesic regimens were trialled over the course of the pregnancy with large doses of opioids required for long periods. Ultimately the patient underwent a laparotomy and myomectomy at 25 weeks gestation in an attempt to alleviate her pain. CONCLUSION: There should be early discussions and planning around the choice of analgesic agents and their planned duration, with the risks and benefits weighed in each instance. A multidisciplinary approach with obstetricians, neonatologists, anesthetists, and pain specialists is likely to result in the most benefit while limiting the risk to the fetus

The Oxford (Oxford® partial knee; Biomet) mobile bearing medial unicondylar knee replacement (OUKR) is a preferred choice by surgeons due to minimal blood loss, reduced pain, and better range of motion. Commonly observed complications include aseptic loosening, polyethylene wear, bearing dislocation, and periprosthetic fractures. A bearing dislocation can be prevented by ensuring that there is correct tracking of bearing during the trial reduction as well as no loss of entrapment. We present a case report in a 50-year-old patient undergoing bilateral OUKR wherein it was observed that the meniscal bearing upon the flexion of the knee joint had the tendency to dislocate. Upon revising the vertical tibial recut according to the anterior superior iliac spine, the meniscus was found to have a normal excursion. Mobile bearing dislocation is a unique complication of mobile bearing OUKR. Surgical technique is of paramount importance in ensuring a successful OUKR. The vertical tibial cut is made to accommodate the vertical wall of the tibial component. Meniscal displacement is quite a common complication and can occur due to multiple causes. Inaccurate vertical tibial cut may be one such reason leading to tibial dislocation.

Tuberculous meningitis (TBM) has a variety of clinical manifestations and complications, and ischemic stroke is a common complication of TBM. However, there is no established prevention or treatment for stroke associated with TBM, and the safety and efficiency of thrombolysis in acute stroke caused by TBM remain unknown. Herein, we present a case of successful intravenous thrombolysis in ischemic stroke caused by TBM. A 50-year-old male patient with cerebral infarction had substantially improved neurological function after intravenous thrombolysis, and he was subsequently found to have TBM. Our findings suggest that intravenous thrombolysis might be an effective acute treatment method for infectious stroke.

Diverticular perforation is a common complication of diverticulitis and can lead to the creation of abscesses. The presence of such abscesses on the abdominal wall is rare and can lead to misdiagnosis. We present the case of a patient with abdominal pain and the formation of a large left lumbar abscess due to perforation of a diverticulum of the left colon and our surgical treatment of choice with favorable results.

Ectopic spleen is a rare splenic malformation. Apart from torsion of the vascular pedicle, a common complication, an ectopic spleen could be responsible of an acute intestinal obstruction. We report a case of an 8-year-old girl who presented an acute abdomen, following of the transverse colon strangulation by the torsion of the vascular pedicle of a wandering spleen. The patients benefited a surgical opening into the abdomen who has been confirmed the diagnosis and made a detorsion of the vascular pedicle of spleen with splenopexy.The interest of this case report lies on the rarity of a wandering spleen and on the unusual situation of transverse colon strangulation by the vascular pedicle of spleen.
