BACKGROUND: Pain is the most common complication of fibroids in pregnancy and can be difficult to treat. The choices of pain relief in pregnancy are limited due to myriad risks including miscarriage, teratogenicity, premature birth, and low birth weight. CASE REPORT: This paper describes the analgesic challenges faced when managing severe pain in the antepartum period for a woman pregnant with twins who also suffered from uterine leiomyomas. Multiple analgesic regimens were trialled over the course of the pregnancy with large doses of opioids required for long periods. Ultimately the patient underwent a laparotomy and myomectomy at 25 weeks gestation in an attempt to alleviate her pain. CONCLUSION: There should be early discussions and planning around the choice of analgesic agents and their planned duration, with the risks and benefits weighed in each instance. A multidisciplinary approach with obstetricians, neonatologists, anesthetists, and pain specialists is likely to result in the most benefit while limiting the risk to the fetus

Abstract Introduction: Nasopharyngeal carcinoma in pregnancy is a rare case, a dilemma in its management. This condition has a significant negative effect on pregnancy, given risk of premature birth, fetal defects and abortion. Case Report: 23-year-old woman with 13 weeks gestation age comes with swelling in both necks accompanied by nosebleeds. On nasoendoscopic examination, there was a mass at nasopharynx. Histopathological examination results a nasopharyngeal squamous cell carcinoma. Conclusion: The management of nasopharyngeal carcinoma in pregnancy is very complex, need for extensive multidisciplinary approach with experienced doctors in their respective fields

Nephrotic syndrome is a spectrum of renal disorders in which proteinuria is a hallmark. When pregnancy is affected by nephrotic syndrome, the maternal and fetal outcome, as well as the required treatment, depends on the underlying cause and severity of the disease. Pregnant patients with nephrotic syndrome are more prone to develop pre-eclampsia, preterm birth, low birth weight babies and intrauterine fetal growth restriction. Here we are presenting a case report on pregnancy with nephrotic syndrome which was managed by multidisciplinary approach with successful outcome.

Primitive neuroectodermal tumor (PNET) is a small round tumor belonging to the PNET/Ewing's sarcoma family. We hereby report a case of PNET of the ovary, which was detected at the second trimester of pregnancy. Chemotherapy was administered and a healthy baby was delivered by cesarean section. After the pregnancy, the mother was found to have metastatic disease. Chemotherapy was continued, but she died due to progressive disease 13 months after the initial diagnosis. In this case report, we discuss chemotherapy options during pregnancy and the importance of multidisciplinary approach to unusual presentations of rare tumors

BACKGROUND: Some benign changes of the ovaries like hyper reaction luteinalis sometimes cannot be differentiated from malignant ones without histological examination. In these cases, surgical intervention sometimes cannot be avoided. Hyperreactio luteinalis is a condition that can occur only in pregnancy. It is characterised by bilateral benign multicystic ovarian enlargement. CASE REPORT: We present a case of misleading intraoperative findings during Cesarean section that ended with ovariectomy. CONCLUSION: During the Caesarean section, some benign masses of the ovaries, like hyper reaction luteinalis, are difficult to differentiate from malignant disease without histological examination, requiring surgical intervention.

AbstractObjectives:To report a case of life-threatening epistaxis in a pregnant patient, describe the links between pregnancy and epistaxis, and discuss the management of such cases. Life-threatening epistaxis in pregnancy is rare, and there are no specific evidence-based guidelines regarding the management of these patients.Case report:A 31-year-old primigravida presented with severe epistaxis in the second trimester of her pregnancy. Conservative measures failed, thereby necessitating surgical intervention.Conclusion:This case illustrates the importance of a multidisciplinary approach in the management of a pregnant patient presenting with severe epistaxis, and highlights the surgical challenges presented in such a situation.

Acute appendicitis is the most common acute surgical infection during pregnancy. Although usually pyogenic in origin, parasitic infections account for a small percentage of cases. Despite the relatively high prevalence of acute appendicitis in our environment, it is not commonly associated with schistosomiasis. We report here the association of pregnancy and appendicitis caused by Schistosoma haematobium. Schistosomiasis is very common complication of pregnancy in hyperendemic areas. Schistosome egg masses can lodge throughout the body and cause acute inflammation of the appendix, liver and spleen. Congestion of pelvic vessels during pregnancy facilitates passage of eggs into the villi and intervillous spaces, causing an inflammatory reaction. Tourism and immigration make this disease a potential challenge for practitioners everywhere.
