Orbital hemorrhage as a primary manifestation of disseminated intravascular coagulation (DIC) associated with intrauterine fetal death and placental abruption

AbstractDisseminated intravascular coagulation (DIC) is a serious complication of obstetric emergencies, and its clinical manifestation occurs in various organs and tissues. Ocular and orbital involvement has been reported only rarely.A 15-year-old primigravida complained about loss of vision in the right eye for 3 days. Magnetic resonance imaging showed a retrobulbar hemorrhage. A first diagnosis of pregnancy (estimated gestational age of 23 weeks) was made, and intrauterine fetal death was diagnosed by ultrasound examination. Laboratory workup revealed the diagnosis of DIC. Due to massive vaginal bleeding a cesarean section was performed, and placental abruption was diagnosed intraoperatively.The concomitance of intrauterine fetal death and other obstetric complications such as placental abruption might induce a fulminant coagulopathy with severe consequences even with uncommon organ localization.

Abstract Massive subchorionic thrombosis (MST) is defined as a massive thrombosis under the chorionic plate, and is responsible for obstetric complications. We encountered two cases of MST. Placental abruption was misdiagnosed by ultrasound examination in Case 1, and a cesarean section was performed at 33 weeks of gestation. Magnetic resonance imaging (MRI) at 21 weeks of gestation suggested MST in Case 2. A cesarean section was performed at 26 weeks’ gestation because of pregnancy-induced hypertension, fetal growth restriction and non-reassuring fetal status.

A Rudimentary horn is a rare form of congenital uterine anomaly, resulting from an arrest in the development of one of the Mullerian ducts with inappropriate fusion with the other side. In most of the cases no direct communicating channel exists between the two parts of the uterus. Pregnancy in the rudimentary horn represents a rare form of ectopic gestation and is thought to result from transperitoneal migration of spermatozoa or the fertilized ovum. Most patients with rudimentary horn pregnancy present in second trimester with hemorrhagic shock due to rupture of horn. Diagnosis of the rudimentary horn may be done with magnetic resonance imaging or 3D, 4D ultrasonography but confirmation of diagnosis is usually surgical at laparoscopy or laparotomy. We report a case with provisional diagnosis of a case of primigravida with 40+ weeks pregnancy with transverse lie along with placenta praevia and intrauterine fetal death. But at laparotomy it revealed a ruptured pregnant non-communicating horn of uterus.Bangladesh J Obstet Gynaecol, 2015; Vol. 30(1) : 53-55

A 31-year-old female was in the 13th week of pregnancy when an abdominal ultrasound examination revealed a large retroperitoneal tumour. Magnetic resonance imaging was carried out and the imaging described a 10-cm mass in diameter extending from the right kidney. Given that the patient was in her first trimester and that there was a suspicion of malignancy, further surgical exploration of the tumour was warranted. During the operation, the tumour was removed, but nephrectomy was not necessary. Histologic analysis of the resected tumour showed a mucinous cystic adenoma, and no signs of malignancy were present. Following the surgery, the pregnancy was otherwise uneventful and further complications did not occur. This case illustrates that surgery is recommended in patients with a retroperitoneal tumour early during a pregnancy, when a malignancy cannot be excluded.
