ABSTRACT Objective: Raise the nursing diagnosis by NANDA, identify the nursing interventions by NIC, raise the expected by NOC. Methods: Study of quantiitative approach, kind case report. Results: Study was raised from the evaluation of a patient that found in post operative of intermediate cardiac surgery ICU admission of a university hospital. Found eight nursing diagnosis and as such, the strokes as intervention NIC and expected results by NOC. Discussion: Diagnosis patient profile, prepared based on your needs, provides a rationale for determination of nursing interventions. Conclusion: Identification of nursing diagnosis is intended to assist in planning based nursing care and adequate to the needs of each patient effective actions resulting in solving problems.Descriptors: Nursing, Thoracic surgery, Nursing care.RESUMOObjetivo: Levantar os diagnósticos de enfermagem, segundo a NANDA; identificar as intervenções de enfermagem, segundo a NIC; levantar os resultados esperados segundo a NOC. Resultados: O estudo foi levantado a partir da avaliação de um paciente que se encontrava em pós-operatório mediato de cirurgia cardíaca internado na UTI de um hospital universitário. Foram encontrados oito diagnósticos de enfermagem e a partir destes, traçados as intervenções segundo o NIC e os resultados esperados segundo o NOC. Discussão: O perfil diagnóstico do paciente, elaborado com base em suas necessidades, oferece uma fundamentação para determinação das intervenções de enfermagem. Conclusão: A identificação dos diagnósticos de enfermagem tem o propósito de auxiliar no planejamento dos cuidados de enfermagem fundamentados e adequadas às necessidades de cada paciente, resultando em ações eficazes para a resolução dos problemas. Discusión: El diagnóstico del paciente perfil, preparado en base a sus necesidades, proporciona una base para determinación de las intervenciones de enfermería. Conclusión: Identificación de diagnóstico de enfermería es para ayudar en cuidados de enfermería basada en la planificación y adecuado a las necesidades de cada paciente que resulten acciones efectivas en solución de problemas. A healthy and beautiful smile can affect appearance and confidence. One of the aesthetic problems in dentistry that is often complained of by patients is excessive gingival display (gummy smile). The excessive gingival display can be caused by several factors, one of which is altered passive eruption (APE). One of the treatments to correct gummy smile related to APE is crown lengthening. Crown lengthening can be with bone reduction (gingivectomy with bone reduction) or without bone reduction (gingivectomy). Crown Lengthening with bone reduction is a surgical procedure that aims to maintain the dentogingival complex and to improve smile aesthetics. The purpose of this case report is to determine the crown lengthening with bone reduction (gingivectomy with bone reduction) procedure as a gummy smile treatment related to APE .A23-year-old female patient, came to Dental Hospital of Universitas Airlangga with complaints of her upper gum which not in the same length and the teeth looked short, she considered her smile was less aesthetic. After conducting analyses relating to aesthetics and periodontal tissue, crown lengthening with bone reduction was chosen for this patient treatment. The treatment results are quite good, visible gingival margins that matched the gingival zenith and improved patient's smile profile. APE as the etiology of patient's gummy smile can be corrected. There are no post-surgical complications such as excessive pain and infection. A proper diagnosis, treatment plan, and good techniques can produce a harmonious smile on the patient.

Abstract Background Lymphocytic esophagitis is a new and rare clinicopathological entity. It is a histological pattern characterized by lymphocytic infiltrate without granulocytes. Its etiology and clinical significance remains unclear. The clinical manifestations are typically mild, with reflux and dysphagia the most commonly reported symptoms. Aims We describe a case report of spontaneous esophageal perforation associated with lymphocytic esophagitis. Methods Case report Results A previously well 31-year-old male presented to the emergency department with acute food impaction. His antecedent symptoms were acute chest discomfort and continuous odynophagia following his most recent meal, with persistent globus sensation. The patient had no reported history of allergies, atopy, rhinitis, or asthma. A previous history of non-progressive dysphagia was noted after resuscitation. Emergent endoscopy revealed no food bolus, but a deep 6 cm mucosal tear in the upper-mid esophagus extending 24 to 30 cm from the incisors. Chest computed tomography observed small volume pneumoperitoneum consistent with esophageal perforation. The patient’s recovery was uneventful; he was managed conservatively with broad-spectrum antibiotics, proton pump inhibitor therapy, and a soft-textured diet. Endoscopy was repeated 48 hours later and revealed considerable healing with only a residual 3-4cm linear laceration. Histology of biopsies taken from the mid and distal esophagus demonstrated marked infiltration of intraepithelial lymphocytes. There were no eosinophils or neutrophils identified, consistent with a diagnosis of lymphocytic esophagitis. Autoimmune indices including anti-nuclear antibodies and immunoglobulins were normal, ruling out a contributory autoimmune or connective tissue process. The patient was maintained on a proton pump inhibitor (pantoprazole 40 mg once daily) following discharge. Nearly six months following his presentation, the patient had a recurrence of symptoms prompting representation to the emergency department. He described acute onset chest discomfort while eating turkey. Computed tomography of the chest redemonstrated circumferential intramural gas in the distal esophagus and proximal stomach. Conclusions Esophageal perforation is a potentially life-threatening manifestation of what had been considered and described as a relatively benign condition. From isolated dysphagia to transmural perforation, this case significantly expands our current understanding of the clinical spectrum of lymphocytic esophagitis. Funding Agencies None

Background: Erector spinae plane block (ESPB) has been described as an effective regional anesthesia technique in thoracic parenchymal surgery. Evidence highlighting the use of this technique continuously via perifascial catheter is lacking. Case presentation: In this case report, we present the case of a patient scheduled for robotic-assisted thoracic surgery for a pulmonary neoformation in the lower right lobe. We decided to manage this patient with a multimodal approach in order to have an opioid-sparing effect. This is the first reported case of continuous ESPB in robot-assisted thoracic surgery. Conclusions: Anesthesiologists should consider this method in surgery that is slower than conventional surgery, such as robot-assisted, and less invasive than thoracotomy, which does not warrant the use of neuroaxial or paravertebral techniques that increase the risk of iatrogenic complications.
