ABSTRACT Objective: Raise the nursing diagnosis by NANDA, identify the nursing interventions by NIC, raise the expected by NOC. Methods: Study of quantiitative approach, kind case report. Results: Study was raised from the evaluation of a patient that found in post operative of intermediate cardiac surgery ICU admission of a university hospital. Found eight nursing diagnosis and as such, the strokes as intervention NIC and expected results by NOC. Discussion: Diagnosis patient profile, prepared based on your needs, provides a rationale for determination of nursing interventions. Conclusion: Identification of nursing diagnosis is intended to assist in planning based nursing care and adequate to the needs of each patient effective actions resulting in solving problems.Descriptors: Nursing, Thoracic surgery, Nursing care.RESUMOObjetivo: Levantar os diagnósticos de enfermagem, segundo a NANDA; identificar as intervenções de enfermagem, segundo a NIC; levantar os resultados esperados segundo a NOC. Resultados: O estudo foi levantado a partir da avaliação de um paciente que se encontrava em pós-operatório mediato de cirurgia cardíaca internado na UTI de um hospital universitário. Foram encontrados oito diagnósticos de enfermagem e a partir destes, traçados as intervenções segundo o NIC e os resultados esperados segundo o NOC. Discussão: O perfil diagnóstico do paciente, elaborado com base em suas necessidades, oferece uma fundamentação para determinação das intervenções de enfermagem. Conclusão: A identificação dos diagnósticos de enfermagem tem o propósito de auxiliar no planejamento dos cuidados de enfermagem fundamentados e adequadas às necessidades de cada paciente, resultando em ações eficazes para a resolução dos problemas. Resultados: El estudio fue levantado de la evaluación de un paciente que se encuentra en post-operatorio de intermedio cirugía cardiaca ingreso en la uci de un hospital universitario. Encontraron ocho diagnóstico de enfermería y, como tal, los golpes al NIC intervención y resultados esperados por NOC. Discusión: El diagnóstico del paciente perfil, preparado en base a sus necesidades, proporciona una base para determinación de las intervenciones de enfermería. Conclusión: Identificación de diagnóstico de enfermería es para ayudar en cuidados de enfermería basada en la planificación y adecuado a las necesidades de cada paciente que resulten acciones efectivas en solución de problemas. Objective: Identifying the prescribed nursing care for hospitalized patients at risk of falls and comparing them with the interventions of the Nursing Interventions Classifications (NIC). Method: A cross-sectional study carried out in a university hospital in southern Brazil. It was a retrospective data collection in the nursing records system. The sample consisted of 174 adult patients admitted to medical and surgical units with the Nursing Diagnosis of Risk for falls. The prescribed care were compared with the NIC interventions by the cross-mapping method. Results: The most prevalent care were the following: keeping the bed rails, guiding patients/family regarding the risks and prevention of falls, keeping the bell within reach of patients, and maintaining patients’ belongings nearby, mapped in the interventions Environmental Management: safety and Fall Prevention. Conclusion: The treatment prescribed in clinical practice was corroborated by the NIC reference.

The paper presents information models of the nursing process through a conceptual presentation, including assessments of the patient's overall condition (nursing interviews, physical assessment, measurement, documentation analysis), nursing diagnosis, nursing care planning, determination of expected results, nursing manipulation, care plan implementation and care results evaluation. Particular attention is drawn to the possibility of using international terminology in describing nursing practices. Analysis of solutions is carried out using the terminology of decision trees.

ABSTRACT Objective: to report the use experience of convergent healthcare research for developing a nursing care protocol. Method: convergent care research developed in university hospital, from July to December 2016, with 27 participants. Results: the stages of the research and its results are described in the steps: conception, instrumentation, screening and analysis. The end result was the nursing care protocol in day zero of hematopoietic stem cell transplantation. Conclusion: convergent care research was an appropriate method for developing the care protocol, and an important contribution to the approximation between theory and practice. The nursing care protocol was the result of this study and confirmed both the purpose of the research as a professional Master's in acquiring knowledge aimed at improving professional practice.

ABSTRACTObjective: to conduct a survey of the studies regarding the main postoperative complications in cardiac surgery and main nursing diagnoses identified. Methods: this is a descriptive study that to select the papers, the following databases were used: SCIELO, LILACS and MEDLINE. The descriptors were: postoperative, cardiac surgery, nursing diagnoses. The following inclusion criteria have been considered: papers published in Brazil from 1997 to 2007, papers on postoperative complications in cardiac surgery in adults, papers using the diagnoses standardized by NANDA. The selected papers were distributed into categories. Results: the following categories have been defined: category I – Complications in the postoperative in cardiac surgery; and category II – Nursing diagnoses in postoperative in cardiac surgery. The relation among the main postoperative complications has been made – physiology and/or semiology of the complication – nursing diagnoses – nursing interventions, which has been presented through synoptic. Later, a nursing intervention plan has been proposed. Conclusion: in despite of the complexity of the development of a nursing plan, it is highlighted the assistance rendered, the necessity of clinical studies regarding complications and the postoperative scenario, and the logical thinking focused on scientific information contributing for knowledge construction and nursing improvement. Os artigos selecionados foram distribuídos em categorias. Resultados: foram definidas as seguintes categorias: categoria I - Complicações no pós-operatório em cirurgia cardíaca; e categoria II - Os diagnósticos de enfermagem no pós-operatório em cirurgia cardíaca. Fez-se a relação principais complicações pós-operatórias – fisiologia e/ou semiologia da complicação – diagnósticos de enfermagem – intervenções de enfermagem, a qual foi apresentada em quadros sinópticos. A seguir, propôs um plano de intervenções de enfermagem. Conclusão: Apesar da complexidade do desenvolvimento do plano de enfermagem, ressalta-se a qualidade da assistência prestada, a necessidade de estudos clínicos referentes às complicações e ao cenário pós-operatório e o raciocínio lógico centrado em informações científicas, contribuindo para a construção do conhecimento e engrandecimento da enfermagem. The purpose of this study was to evaluate a 16-hour intervention designed to build clinician competency in the use of North American Nursing Diagnosis Association (NANDA), Nursing Outcome Classification (NOC), and Nursing Interventions Classification (NIC) (hereinafter: N3) among nurses with limited N3 knowledge. Each of 19 pairs of nurses independently selected N3 terms and rated the outcomes applicable to an actual patient for a specified time. A pair-through discussion then created a single consensus patient profile of the applicable terms. Before discussion, pairs agreed on 46% of the NANDA diagnoses, 30% of the NOC outcomes, and 20% of the NIC interventions selected. Eighty-nine percent of NOC label pair ratings were within 1 point. Building competency in N3 requires consistent use in written and oral communication with peers across time. Inter-rater reliabilities (IRRs) for NOC label ratings support previous findings.

This cross-sectional study was carried out at a university hospital to describe the nursing interventions most frequently performed in the clinical practice of an intensive care unit, based on nursing care prescriptions, and to investigate their similarity to the Nursing Interventions Classification (NIC). The sample consisted of 991 hospitalizations of patients. Data were retrospectively collected from the computer database and analyzed through descriptive statistics and cross-mapping. A total of 57 different NIC interventions frequently used in the unit were identified; most of them in the complex (42%) and basic physiological (37%) domains, in the classes ‘respiratory management’ and ‘self-care facilitation’. Similarity between the nursing care prescribed and nursing interventions/NIC was found in 97.2% of the cases. The conclusion is that the interventions/NIC used in the clinical practice of this intensive care unit reflects the level of complexity of nursing care, which is mainly directed at the regulation of the body’s physical and homeostatic functioning.

Objective: to apply the nursing process fundament on the Theory of Orem’ Self-care, through the conduct of a report of clinical case, with a patient submitted to bariatric surgery. Methods: this is about a descriptive study, from qualitative approach, report of clinical case type, performed in a patient with morbid obesity, included in Bariatric Surgery Group of a teaching hospital in João Pessoa-PB city. For data collection was used a script adapted to Theory of Orem’ Self-care, which led to the identification of deficits of self-care and therefore to nursing diagnoses. It was then developed the plan of nursing care, with the determination of goals, objectives, method of assistance, type of system and nursing interventions. This study has been approved by the Research Ethics Committee of the Hospital of the Federal University of Paraiba (054/07). Results: from the identification of nursing diagnoses was established and implemented the plan of nursing care with the aim of restoring the patient to prevent postoperative complications, promote recovery and prepare you for the self-care. Conclusion: it is considered that the application of the nursing process, based on Theory of Orem’ Self-care, enabled the provision of assistance and qualified individual, encouraging the patient to participate actively in their treatment, but also to increase their responsibility in the outcome of care. Non-pharmacological actions that can prevent and treat constipation are abdominal massage and therapy to drink 500 cc of warm water in the morning.  Doing massage can help encourage faeces and reduce abdominal muscle tension.  The purpose of this study is the application of nursing care in stroke cases.  This research is a type of case study research by applying comprehensive nursing care to patients.  The results showed data of patients who have not defecated for 4 days, the stomach feels full, there is a desire to defecate but it is difficult to get out, bowel sounds 6 times / minute.  The goal of nursing care is to prevent and overcome constipation, from moderate to mild constipation.  Nursing diagnosis of constipation is related to decreased gastrointestinal.  Nursing interventions Management constipation / impication.  Implementation of nursing doing abdominal massage and therapy to drink 500 cc of warm water in the morning.  Evaluation after 5 times giving patient intervention can defecate.  Conclusion: The application of abdominal massage and 500 cc warm water drinking therapy can prevent and overcome constipation in stroke patients in Poso District General Hospital.

Objetivo: analisar a produção científica sobre a validação de intervenções de enfermagem. Método: trata-se de um estudo bibliográfico, descritivo, tipo revisão integrativa de literatura, no recorte temporal de 2007 a 2016, na MEDLINE, CINAHL e Bibliotecas Virtual de Saúde e COCHRANE. Organizaram-se os estudos, categorizando-os e os analisando de modo descritivo. Resultados: encontraram-se 147 artigos e, após a leitura dos resumos e dos textos na íntegra, foram incluídos oito artigos. Conclusão: identificou-se, por meio da revisão integrativa, a produção científica referente à validação de intervenções de enfermagem na literatura, evidenciando um aumento da produção desses estudos, o que contribui para a prática de Enfermagem no Brasil.
