Resumo: Objetivou-se avaliar a atuação do enfermeiro na consulta de pré-natal quanto às orientações sobre aleitamento materno. Pesquisa observacional não participante, quantitativa, realizada em três Centros de Saúde da Família de Fortaleza-CE, Brasil, durante atendimento a 54 gestantes, utilizando um checklist. Das 54(100%) consultas observadas, verificou-se percentual reduzido em relação às orientações: anatomia da mama 1(1,9%), posição para amamentar 5(9,3%) e pega adequada 18(33,3%); em contrapartida o preparo da mama 35(64,8%) e duração do aleitamento 40(74,1%), foram mais orientados. Identificaram-se lacunas que devem ser minimizadas por meio da corresponsabilidade dos profissionais referente à prática do aleitamento materno. De las 54 (100%) consultas observadas, hubo bajo porcentaje en las siguientes orientaciones: anatomía de la mama 1(1,9%), posición para amamantar 5 (9, 3%) y pega 18 (33,3%); en contradicción, preparación de la mama 35 (64,8%) y duración de la lactancia 40 (74,1%), fueron más orientados. Objetivo: Distinguir as necessidades de saúde priorizadas pelo enfermeiro na consulta pré-natal, e caracterizar a especificidade e abrangência das mesmas. Método: Pesquisa descritiva, qualitativa, realizada em quatro unidades da Estratégia Saúde da Família de Cuiabá-MT, mediante observação participante de 50 consultas de gestantes, análise de 79 registros de enfermagem e análise de conteúdo do tipo temática. Resultados: Nas consultas, os enfermeiros privilegiam a abordagem de necessidades físico-obstétricas. Eventualmente levantam aspectos sociais e psicoemocionais, mas não os abordam como necessidades a serem satisfeitas. Entretanto, as gestantes expressam necessidades sociais, psicoemocionais, de informação, de acesso a tecnologias e de participação familiar na consulta. Apesar do potencial mobilizador destas últimas necessidades, elas não chegam a modificar o que recebe atenção na consulta. Conclusão: É preciso investir na melhoria dos processos de trabalho da atenção básica, na ampliação de suas ações, e na formação e educação dos enfermeiros, tendo em perspectiva a abordagem de necessidades abrangentes no pré-natal.

Background: The objective of this study was to highlight obstetrical complications that occurred among adolescent girls who delivered at the ward and to identify factors associated with the occurrence of such complications.Methods: This was a prospective study of descriptive and analytical type extending over a period of one year from September 1, 2016 to August 31, 2017 carried out at the maternity ward of Ignace Deen National Hospital at Conakry Teaching Hospital (CHU). It covered a continuous series of 1034 deliveries among adolescent girls.Results: The frequency of childbirth among adolescent girls was 16.7%. The main complications identified were dystocia, severe preeclampsia, eclampsia, retroplacental hematoma, placenta previa, uterine rupture, severe anemia, postpartum hemorrhage and puerperal endometritis. These complications occurred among adolescent girls aged 18 to 19, christian, skin and pelvic bones secondary school or university students. Factors associated with such complications were the marital status (p=0.010), the gestational age (p=0.012), the number of prenatal consultations (p=0.001), the place of prenatal consultation (p=0.001), the reason for admission (p=0.000) and the mode of admission (p=0.000).Conclusions: Childbirth among adolescent girls is frequent in this context; complications are numerous but they are preventable in the vast majority of cases.

AbstractObjective:The aim of this single-center study was to identify factors that affect the short-term outcome of newborns delivered around the limits of viability.Methods:A group of 137 pregnant women who gave birth between 22+0/7 and 25+6/7 weeks of gestation was retrospectively studied. The center supports a proactive approach to infants around the limits of viability. Perinatal and neonatal characteristics were obtained and statistically evaluated.Results:A total of 166 live-born infants were enrolled during a 6-year period; 162 (97.6%) of them were admitted to the neonatal intensive care unit (ICU) and 119 (73.5%) survived until discharge. The decrease in neonatal mortality was associated with an advanced gestational age (P<0.001) and a completed course of corticosteroids (P=0.002). Neonatal morbidities were common among infants of all gestational ages. The incidence of severe intraventricular hemorrhage significantly depended on gestational age (P<0.001) and a completed course of corticosteroids (P=0.002). Survival without severe neonatal morbidities was 39.5% and occurred mostly after 24+0/7 weeks of gestation.Conclusion:The short-term outcome of newborns delivered around the limits of viability is mostly affected by gestational age and antenatal corticosteroid treatment. A consistently proactive approach improves the survival of infants at the limits of viability. This is most pronounced in cases where the delivery is delayed beyond 24 completed gestational weeks.
