The goal of this article was to review the most common temporomandibular disorders according to the Diagnostic Criteria for Temporomandibular disorders (DC/TMD) emphasizing in the criteria for headache attributed to TMD. By presenting a clinical case that shows the characteristics of a patient with masticatory myofascial pain, arthralgia of the temporomandibular joint and headache attributed to TMD. A case of a 34-year-old female patient is presented, the diagnostic process and its approach: self-care, modification of parafunctional habits and behaviors, physical therapy and pharmacological support.

AIM: Conducting a systematic review of randomized clinical trials focusing on the efficacy of LLLT on pain control in patients with TMD, diagnosed by the Research Diagnostic Criteria for Temporomandibular Disorders (RDC/TMD). MATERIAL AND METHODS: Search was performed at PubMed/MEDLINE database with the terms: (1) “Laser AND temporomandibular disorders”; (2) “Laser AND temporomandibular disorders AND RDC/TMD”; (3) “Low-level laser therapy AND temporomandibular disorders”; (4) “Low-level laser therapy AND temporomandibular disorders AND RDC/TMD”; (5) “Low-level laser therapy AND temporomandibular joint dysfuntion syndrome”; (6) “Low-level laser therapy AND temporomandibular joint dysfuntion syndrome AND RDC/TMD”; (7) “Laser AND temporomandibular joint dysfuntion syndrome” (8) “Laser AND RDC/TMD”; (9) “Low-level laser therapy AND RDC/TMD”. Inclusion criteria: articles need to be randomized clinical trial performed in humans; evaluate the effect of LLLT in the treatment of TMD diagnosed by the use of RDC/TMD; published in English or Portuguese in the last 10 years. Protocol studies and pilot studies were excluded. RESULTS: Ten studies were included. The type of laser used was Gallium Aluminum Arsenide (GaAlAs) diode, with exception of 1 paper, which used super pulsed Gallium Arsenide laser. Eight studies reported decreased in pain levels, in two articles there was no statistically significant difference between test and placebo groups. CONCLUSION: In most studies, LLLT was effective in pain remission, but there is no standardization in parameters like wavelength, output power and frequency. Studies with more complex experimental designs, standardized diagnostic criteria for TMD and defined protocols for the use of LLLT are needed to determine its efficacy in the treatment of TMD.

AbstractTemporomandibular dysfunction (TMD) is complex and multifactorial. Its etiology involves various factors, such as malocclusion, psychological patterns, parafunctional habits, and the temporomandibular joint anatomy. Symptoms include myofascial pain, joint clicking, mouth opening limitation, headaches, earaches, and neck pain, comprising one of the greatest causes of nondental pain. Acupuncture has been used to treat these conditions by acting locally both in muscle relaxation and pain management as well as reaching physical, mental, and emotional balance. The aim of this study is to present a brief literature review reporting the efficacy of acupuncture on TMDs and a case report in which the painful symptomatology of TMD was treated with a systemic protocol, based on scientific evidence and concepts of traditional Chinese medicine. Acupuncture may be a complementary treatment for TMD, and in this case report, it eliminated the patient’s painful symptomatology.

This article describes a case of Caroli’s disease in a 53-year-old female patient who complained nonspecificabdominal pain without cholestasis or cholangitis. Ultrasound and hepatic magnetic resonanceimaging showed segmental saccular dilations connected to intrahepatic bile ducts without hepaticfibrosis. This clinical case shows the possibility of having oligosymptomatic forms in the diffuse formsof Caroli disease, therefore the interest to consider this diagnosis in case of non-specific abdominalsigns and to request a hepatic ultrasound. The boundaries of simulation and dissociative amnesia are thin and not rarely unclear. Diagnostic criteria and a thorough clinical history are sometimes not enough to shed light on the nature and essence of one's will and conduct. Throughout history, psychiatry has been challenged to testify before society over patients’ truth and deceit. Do they share common grounds or must we accept that uncertainty is the inevitable price of truth?The authors present a clinical case of a young man coming to the emergency services with multiple lacerations in the upper abdomen and limbs, referring “amnesia” for the last five years of his life. The patient had no previous psychiatric record or any known family or social dysfunction. Approach, management and diagnosis are discussed.

Introduction. Nowadays, the total hip arthroplasty is a very frequent surgical intervention. In some cases, vascular and nerve injuries may happen around the hip with total hip arthroplasty. Although they are very rare, they may be very dangerous for the patient in some cases. This paper presents a case of a female patient, in whom the nervous fibularis lesion was detected after the total hip arthroplasty, and the occlusion of the iliac femoral artery was revealed later during physical therapy. Case Report. We described a case of a 32-year-old female patient, in whom the nervous fibularis lesion was detected after the total hip arthroplasty. The patient was referred to a ward for physical therapy. On the 19th postoperative day, she felt a vigorous ache and numbness on the left operated leg during stimulation of the paretic fibular musculature. Clinically weak inguinal arterial pulse was detected. After the examination, iliac-femoral occlusion was diagnosed. The patient was referred to the vascular surgeon. In the next few months, she was treated conservatively and eventually underwent surgery. The revascularization was achieved with a satisfactory effect. A year after the total hip replacement, the patient continued with rehabilitation and physical treatment, which lasted one and a half month and had an incomplete functional result - the patient walked with a walking stick and had weak fibular musculature of a severe degree. The vascular status of the leg was good. Conclusion. In this case, neurovascular lesions led to an incomplete functional recovery of the patient and compromised the expected treatment outcome. According to the scoring system used to assess the functionality, the result was marked as poor.

BACKGROUND: Postoperative lymphatic complications are not common, and lymphatic leakage complication post appendectomy (LLCPC) is even rarer. However, the number of this operation is high so LLCPC can occur. CASE REPORT: Here, we report a female patient post appendectomy with severe chylous ascites. This patient underwent six operations. A leakage point at the right iliac-fossa, which was embolized successfully after two sessions, was spotted during intranodal lymphangiography. After 6 months, the ascites were significantly reduced while some lymphatic aneurysms still existed in the lumbar-retroperitoneal region. CONCLUSIONS: Basing the knowledge of this clinical case and literature, we have concluded that lymphatic leakage can be diagnosed and embolized by percutaneous intervention.
