Objective: to characterize and determine the polypharmacy prevalence in patients with chronic diseases and to identify the factors associated, in order to improvement of pharmaceutical care focused on patient safety. Methods: cross-sectional study included 558 patients, covered by primary health care, using a household and structured questionnaire. We analyzed the data on polypharmacy and its clinical and socioeconomic factors. Poisson regression analysis with robust variance was applied, with results expressed in prevalence ratio. Results: the results showed that polypharmacy (consumption of four or more drugs) was of 37.6%. The prevalence ratio analyses identified independent variables associated with polypharmacy: age (3.05), economic strata (0.33), way of medication acquisition through a combination of out-of-pocket and Brazilian public health system (1.44), diabetes and hypertension (2.11), comorbidities (coronary artery disease 2.26) and hospital admission (1.73). In the analyses, inappropriate medication use of the 278 patients (≥ 65 years) was associated with polypharmacy (prevalence ratio 4.04). Conclusion: polypharmacy study becomes an opportunity to guide the strategies for the patient safety to promote the medication without harm in chronic diseases.

Objective: to evaluate the patient safety culture in a primary care health unit. Methods: cross-sectional study conducted with 51 professionals linked to Primary Health Care. Data were collected using the self-administered instrument Medical Office Survey on Patient Safety Culture, translated, adapted and validated for use in Brazil. Descriptive statistical analysis was performed. Nurses, physicians, Community Health Agents, among other professionals participated. Results: the dimensions of the patient safety culture in Primary Health Care indicated opportunities for improvement or weaknesses in the service. The general assessment of patient safety and the global assessment of the quality of care provided in Primary Health Care were appointed as good or fair. Conclusion: the safety culture in Primary Care did not identify strong dimensions; there were problems with equipment, performance and availability of test results and exchange of information with imaging centers/laboratories in the care network.

ABSTRACT Objective: to characterize patient safety according to the perception of primary health care workers in a large city in Paraná. Method: cross-sectional study, in which information was collected using the "Research on Patient Safety Culture for Primary Care" instrument, which was self-answered by health workers working in primary care, from April to June 2017. The Statistical Package for the Social Sciences program was used for data analysis. Results: it was found that as for the frequency of problems related to patient safety and quality of care, the “medical record/patient record being unavailable when needed” and “patient not getting an appointment within 48 hours was considered as a serious or acute problem”. Regarding the support of managers/administrators/leaders, the overall frequency of positive responses was only 38.4%. The section on global safety assessment showed the highest overall percentage of positive responses (79.0%). Workers in the southern region of the municipality under study had a worse assessment of the work process, communication, monitoring and manager support (45.5%, 61.6% and 29.3% of positive responses, respectively). Conclusion: it is concluded that there is a need to strengthen the culture of patient safety at this level of care, strengthening and valuing the performance of all workers, with the intensification of permanent education and the use of soft technologies.

Nutrition and its awareness play an important role in medical education for the management, as well as in the treatment of various chronic illnesses. Moreover, it always has been a concern about the knowledge and awareness of nutrition among physicians. Therefore, we aimed to study the nutritional knowledge, as well as awareness of primary health care physicians (PHC) working in Hail City, Saudi Arabia. A questionnaire-based cross-sectional survey was conducted among PHC physicians working in Hail, Saudi Arabia from September 2019 to December 2019. The questionnaire was distributed to PHC physicians and responses were recorded and data were statistically analyzed by applying Chi-square tests using SPSS 21.0. Questionnaires were circulated to 124 physicians, participated in this study. However, out of 124 physicians only 41 physicians filled out the complete questionnaire, indicating a very poor response rate (33%). The mean percentage of correct answers was 71.56%. Physicians who described their knowledge as “good” were found to be 75%. Furthermore, physicians working in PHC centers were aware of the basic nutritional knowledge regarding food content, pregnancy, lactation, diabetes, hypertension, etc. Our assessment revealed that physicians working in PHC of Hail Region had average nutritional knowledge. Moreover, it was also found that addition of various nutrition related educational programs in medical schools and hospital premises would improve the knowledge and awareness among the physicians.

Method: A cross-sectional study, developed with professionals from the multiprofessional team, who work in basic and family health units of a municipality in the northwest of the State of Rio Grande do Sul. For data collection from June to December 2016, the Safety Attitudes Questionnaire - Outpatient version. Responses follow the Likert scale, and scores were considered positive when ≥7.5. Results: 172 professionals from the multiprofessional team participated in the study. The domains that showed positive results were patient safety and unit management. The lowest scores occurred in the error and working conditions domains. Abstract Background The Frail-VIG frailty index has been developed recently. It is an instrument with a multidimensional approach and a pragmatic purpose that allows rapid and efficient assessment of the degree of frailty in the context of clinical practice. Our aim was to investigate the convergent and discriminative validity of the Frail-VIG frailty index with regard to EQ-5D-3L value. Methods We carried out a cross-sectional study in two Primary Health Care (PHC) centres of the Catalan Institute of Health (Institut Català de la Salut), Barcelona (Spain) from February 2017 to January 2019. Participants in the study were all people included under a home care programme during the study period. No exclusion criteria were applied. We used the EQ-5D-3L to measure Health-Related Quality of Life (HRQoL) and the Frail-VIG index to measure frailty. Trained PHC nurses administered both instruments during face-to-face assessments in a participant’s home during usual care. The relationships between both instruments were examined using Pearson’s correlation coefficient and multiple linear regression analyses. Results Four hundred and twelve participants were included in this study. Frail-VIG score and EQ-5D-3L value were negatively correlated (r = − 0.510; P < 0.001). Non-frail people reported a substantially better HRQoL than people with moderate and severe frailty. EQ-5D-3L value declined significantly as the Frail-VIG index score increased. Conclusions Frail-VIG index demonstrated a convergent validity with the EQ-5D-3L value. Its discriminative validity was optimal, as their scores showed an excellent capacity to differentiate between people with better and worse HRQoL. These findings provide additional pieces of evidence for construct validity of the Frail-VIG index.
