The presence of a foreign body in the periapical tissues can cause endodontic failure by triggering an inflammatory response and a subsequent foreign body reaction. This inflammatory response, which can occur to varying degrees, appears radiographically as a radiolucency that can remain asymptomatic for many years. A foreign object can reach the apical region by accident or iatrogenic procedures during dental procedures. The aim of the present case report is to describe the endodontic surgical treatment of an iatrogenic displacement of a foreign body (a metal fragment) into the periapical tissues and to describe its clinical and radiographic follow-up over a period of 52 months.

PURPOSE: To investigate the biocompatibility and biodegradability of a membrane made from porcine peritoneum. METHODS: The membrane (5x5 mm) was inserted in the subcutaneous tissue on the back of 15 mice, which were killed after 1, 3 and 9 weeks (ISO 10993-6). The cellular components of the inflammatory response and degradation of the membrane were analyzed in hematoxylin-eosin-stained histological sections. RESULTS: After one week, mononuclear cells were observed inside the membrane. After three weeks, the material was almost completely absorbed. After nine weeks, there was no presence of material and there were signs of tissue remodeling. There was neither a foreign body reaction nor signs of tissue necrosis. CONCLUSION: The collagen membrane derived from porcine peritoneum is biocompatible and bioabsorbable when implanted in the subcutaneous tissue of mice.

Pencil core granuloma is characterized by a delayed foreign-body reaction against retained fragments of pencil lead. Previous case reports presented pencil core granuloma resembling malignant melanoma, haemangioma, or soft tissue sarcoma. We present a case of pencil core granuloma arising from the palm 25 years after the initial injury. The patient presented a bluish nodule that had been present over 25 years before. The nodule initially measured 5 mm in diameter. However, five years before presentation, it suddenly enlarged to the size of 30 mm during six months. Computed tomography (CT) of the lesion revealed a linear radiopaque structure of 8 mm long with a mass on its distal end. Surgical resection revealed a bluish muddy mass and pencil lead. Histological examination revealed degenerative tissue with calcification surrounded by massive amounts of black granular material in the middle and lower dermis.
