Сurrent status of the surgical treatment of liver echinococcosis

The literature review presents the world and russian experience in the surgical treatment of patients with hepatic echinococcosis over the last two decades. The most important aspects of the epidemiology and biology of echinococcus for the surgeon have been considered. Special attention is paid to the modern diagnosis of the disease, to the choice of volume and method of surgical treatment, according to WHO classification. The approaches to minimally invasive, laparoscopic and robotic operations of liver echinococcosis had been reviewed.

AbstractObjective:We report two cases in which dysphagia and aspiration, caused by anterior cervical osteophytes, were so severe that surgical resection was performed.Method:Case reports and a review of the world literature concerning dysphagia caused by anterior cervical osteophytes, in regard to pathogenesis, diagnosis and treatment.Results:Two patients, aged 71 and 70 years, had long-standing, slowly progressive dysphagia and aspiration; one patient had recurrent episodes of aspiration pneumonia as a result. Both patients were diagnosed on videofluoroscopy with large bony anterior cervical osteophytes. Immediate relief of symptoms was obtained after resection of the osteophytes via an anterolateral, extrapharyngeal approach. Anterior cervical osteophytes are relatively common in the elderly, although not frequently diagnosed, and are mostly seen in cases of diffuse idiopathic skeletal hyperostosis. If therapy is indicated it is mainly conservative; resection is rarely needed.Conclusion:In patients with anterior cervical osteophytes, surgical treatment is indicated only for selected cases with large, bony osteophytes and severe symptoms.

The aim of the current study is to present the possibilities for minimally invasive percutaneous procedures for surgical treatment of osteoporotic compression fractures of the vertebrae (OVCF), expressed in vertebroplasty and balloon kyphoplasty. We compare their therapeutic effect with that of conservative treatment and present conclusions from the world literature regarding the safety of these procedures. Material and methods: We used data from our surgical experience for the last 10 years. We took into account the relevant literature from the global PubMed database and Google Scholar to present randomized clinical trials and meta-analyzes comparing OVCF treatment methods. We analyzed those who assessed the quality of life, pain, and complications of treatment. Results: The world experience shows an excellent result in the short term from the minimally invasive percutaneous procedures. In the long term some authors note a significant overlap of the results with the conservative methods. All studies show a low risk profile in the surgical treatment, as well as comparable results between the two main procedures - vertebroplasty and balloon kyphoplasty. Conclusion: Both vertebroplasty and balloon kyphoplasty have a statistically significant advantage in terms of final clinical indicators compared to conservative treatment at low and clinically justified risks. No statistically significant differences were found between the surgical techniques on most of the studied criteria, which would justify the significantly higher cost of kyphoplasty.

Objective: To evaluate the results of using minimally invasive technologies in the surgical treatment of hepatic echinococcosis (HE) and its complications. Methods: The results of examination and surgical treatment of 165 patients with HE and its complications using minimally invasive technologies are presented. Results: 77 patients with uncomplicated HE underwent: open echinococcectomy (EE) from various mini-accesses (n=37), laparoscopic EE (n=30), PAIR technology (n=6), hepatic resection using the da Vinci S robotic complex (n=4). In patients with suppurated hydatid cysts (n=49) a combination of percutaneous cyst puncture with laparoscopic (4) and open (5) EE; PAIR technology (5) and laparoscopic EE (35) were performed. All 39 patients with intrabiliary rupture of hydatid cysts underwent two-stage interventions: percutaneous transhepatic choledochostomy followed by laparoscopic (7) and traditional (4) EE; endoscopic papillosphincterotomy (EPST) followed by open EE (9); laparoscopic choledochostomy followed by open EE (3); EPST followed by laparoscopic total (2) and subtotal (14) pericystectomy. Postoperative complications were noted in 29 (17.5%), relapses of the disease – in 12 (10.9%), lethal outcome – in 1 (0.6%) cases. Based on the analysis of the study, an algorithm for choosing the method of surgical treatment for HE and its complications was developed. Conclusions: The use of minimally invasive technology for hepatic echinococcosis can improve surgical treatment outcomes. In the last two decades all over the world, endoscopic surgery is increasingly used in the treatment of cancer. Since the first publications at the end of the last century, many studies have been performed that have proven that the minimally invasive approach during the treatment of lung cancer is safe and effective from an oncological point of view and has a number of advantages over traditional traditional thoracotomy. Based on the literature review, the main purpose of this article is to discuss the extent to which endoscopic anatomical lung resections are used in the surgical treatment of lung cancer. The form presents the evolutionary aspects of the endoscopic method, data on the radicality of these interventions and their immediate results.

The article reviews the literature devoted to modern methods of treating children with flat-valgus deformity of the feet. The basic concept, etiology, pathogenesis of the disease, methods of conservative and surgical treatment of feet deformities in children are given, conclusions on the topic are presented, special emphasis has been placed on minimally invasive intervention. Currently one of the most popular methods of surgical correction of flat-valgus deformity of the feet in children is subtalar arthroeresis.

Despite the rapid development of thoracic surgery, the problem of the surgical treatment of non-specific chronic pleural empyema remains relevant. The variability of the proposed treatment methods: puncture and drainage interventions aimed at pleural cavity sanitation, minimally invasive videothoracoscopic surgeries and traditional lung decortication, dictates the need for case-by-case surgical tactics and determining the stages and duration of the above methods. Treatment outcomes are significantly worsened by the presence of bronchial fistulas, which either cause the development of chronic pleural empyema or develop as a result of its presence and late treatment. Some answers to these key questions are provided in this paper, which aims to identify them in the Ukrainian and foreign literature.

Retrocecal hernia is a rare cause of mechanical ileus and requires surgical treatment in emergency. Laparoscopy is not a frequent approach in this type of pathology. The diagnosis preoperative is not easy despite the TC can provide information to that effect. We present the case of a 52-year-old male patient with no comorbidity and never operated on the abdomen previously, which arrives in the emergency room with symptoms of acute abdomen. Practice direct Rx abdomen and TC abdomen that highlight an ileal stop without signs of decompensation. It is implemented, first instance, a conservative therapy, naso-jejunal tube and gastrographin intake by the way oral. Due to the persistence of occlusive symptoms, he underwent surgery after 24 hours. In laparoscopy we highlight a retrocecocolic hernia that strangles the last ileal loops. Proceed to section of the throttling track and to the opening of the entire laterocolic shower on the right with the resolution of the occlusive frame. Laparotomy has been avoided; laparoscopy made it possible to diagnose and guarantee a minimally invasive treatment with rapid recovery of the patient. We did not consider it appropriate to close the hernial orifice as described by several authors in the literature. The technique is described laparoscopy comparing it with other authors.

It has been known that both in the world and in Turkey a continuous change has been experienced in the provision of health services in recent years. In this sense by adopting the customer(client) focused approach of either public or private sector hospitals; it has been seen that they are in the struggle for presenting a right, fast, trustuble, comfy service. The purpose of this research is to measure the satisfaction degree, expectations and perceptions of the patients in Karabük State Hospital through comparison. In this context, the patient satisfaction scale which has been developed as a result of literature review has been used and by this scale it has been tried to measure the satisfaction levels of the patients in terms of material and human factors which are the two main factors of the service that was presented. In the study, with the scales of Servqual and 0-100 Points together, in the part of the analysis MANOVA have been used. The expectations and the perceptions of the patient has been compared first by generally and then by separating to different groups according to the various criterias and in thisway it has been tried to be measured their satisfaction levels. According to the results that were obtained, although, the satisfaction levels of the patients who have taken service from Karabük State Hospital are high in terms of thedoctors and the nurses; it has been reached to the result that their satisfaction levels are low in terms of the materials that have been used at the presenting of the service and the management.
