Comparison of Three Topical Fluorides Using Computer Imaging

The purpose of this in vitro study was to evaluate the short and long-term effectiveness of fluoride varnish and compare it with the two most commonly used topical fluorides, i.e., fluoride gel and foam.A second purpose of the study was to compare the effectiveness of these preparations on primary and permanent teeth. Enamel slabs with a thickness of 500 microns were obtained from caries free primary molars and premolars. They were divided into four groups: control, foam (F), gel (G) and varnish (V). Fluorides were applied to the enamel slabs according to the manufacturer's instructions and were placed in a 5.1 pH acidic gel for one week. The amount of demineralization from the enamel was measured by calculating the amount of light reflected from these surfaces. This was done by using a Charged Couple Device attached to a high resolution microscope with image processing software, Optima 5.22. The enamel slabs were placed in a freshly prepared acidic gel for a second week without application of fluorides. This was done to mimic a situation where fluoride is eventually brushed away from teeth. Reflective images were recorded under the previously described conditions. A two-way analysis of covariance was used to compare the treatments. The results showed no statistically significant difference (with Bonferroni correction) in the effectiveness of different fluoride preparations over the short-term (Week I comparison; p-values: F vs. G 0.079, F vs.V 0.030, G vs.V 0.44). However, the long-term protection provided by fluoride varnish was far more than fluoride gel and foam (Week II comparison; p-values: F vs. G 9X10-5 , F vs.V 7X10-8, G vs.V 1X10-4). Fluorides were equally effective for both primary and permanent enamels (p-value 0.24). The results of this study suggest that fluoride varnish is beneficial for use with white spot lesions, newly erupted permanent teeth and early decalcification in primary dentition.

Introduction: Furcation perforation is one of the complications that occur during endodontic treatment due to anatomical differences or iatrogenic causes. Repairing the perforation with a biocompatible material eliminates the connection with the gingival sulcus and positively affects the prognosis. The sealing ability of the repair materials is very important and is affected by the conditions of the environment in which they are applied. Aim: To compare the sealing efficiency of Mineral Trioxide Aggregate (MTA) (Angelus, Londrina, PR, Brazil) and Biodentine (Septodont, Saint Maur des Fosses, France) materials used in the repair of furcation perforations in acidic and neutral environments. Materials and Methods: This in-vitro experimental study was carried out in the Department of Endodontics at Dicle University Faculty of Dentistry, Diyarbakır, Türkiye and Dicle University Science and Technology Application and Research Center from 10th June 2020 to 22nd July 2020. A total of 96 mandibular molars were used in the study. The specimens were randomly divided into two equal groups (n=48). When the perforations were repaired with MTA and Biodentine materials, each group was divided into four subgroups (n=12). These subgroups were kept in Phosphate Buffered Saline (PBS) and Acetic Acid (AA) solutions for different periods of time from 4 or 34 days, and all groups were immersed in methylene blue solution. The dye penetration level of the divided sections was examined under a stereomicroscope (Leica, Wetzlar, Germany). Microleakage data of all groups were statistically analysed using Kolmogorov Smirnov, Shapiro Wilk’s and Mann-Whitney U Test. Statistical significance was set at 0.05, it was stated that there was a significant difference when p-value <0.05. Results: The short-term (4 days) microleakage level of biodentine material in the AA environment was significantly lower than that of MTA material (p-value=0.005). There was no statistically significant difference between the materials in terms of long-term (34 days) microleakage levels in the AA environment (p-value >0.05). The long-term microleakage level in PBS environment of biodentine material was significantly lower than that of the AA environment (p-value=0.008). Conclusion: Lower microleakage levels were observed in the neutral environment than the acid environment with both MTA and biodentine during both holding periods.

Bacground/Aim. Glass ionomer cements (GIC) belong to the group of polycarboxyl cements, and one of the principal characteristics of these materials is their anticariogenic potential of fluorine release into saliva and enamel-dentin substance. The aim of this study was to examine the content of released fluorine from GIC restorations (Fuji IX, GC, Japan) of young permanent teeth in the medium of artificial saliva and similar releases in the same medium by the restorations of these teeth treated with a low concentration fluoride solution. Methods. We examined 12 premolars exctracted from orthodontic reasons. The GIC restored teeth were divided into the group treated daily with low concentration fluoride solution (334 ppm) and the control, not treated group. The samples of artificial saliva were analyzed for fluorine ion content using an ion selective electrode. Results. Our comparative analysis of the mean values using the Student?s t-test demonstrated a statistically significant difference in fluorine ion concentration in artificial saliva of fluoridated and non-fluoridated teeth with GIC fillings after 14 and 21 days (p < 0.05), while the difference detected after 7 days was with no statistical significance. Conclusion. The results of this in vitro study indicated that low-concentration fluoride solutions could serve to refluoridate GIC fillings and contribute to an increased fluorine content in saliva. The process of refluoridation of GIC fillings should be advised 2-3 weeks after the restoration, since the release of fluorine from GIC fillings diminishes in time.

The aim of this study was to evaluate the apical microleakage of Biodentine and MTA orthograde apical plugs and to compare the effect of thickness of these biomaterials on their sealing ability. A total of eighty maxillary anterior teeth were used. The apices were removed by cutting with a diamond disc (Jota, Germany) 2 mm from the apical root end in an attempt to standardize the working length of all specimens to 15 ± 1 mm. Both materials were placed in 1–4 mm thickness as apical plugs root canal. Root canal leakage was evaluated by the fluid filtration technique. One-way ANOVA was used in order to determine normality of dispersal distribution of parameters; thereafter, results were analyzed by Kolmogorov-Smirnov test. Overall, between microleakage values of MTA and Biodentine regardless of apical plug thickness, no difference was observed. In terms of plug thickness, a statistically significant difference was observed between the subgroups of MTA and Biodentine (p<0.05). The apical sealing ability of Biodentine was comparable to MTA at any apical plug thickness.

Introduction: Microleakage is a major setback to restorative dentistry. Leakage at the tooth/restoration interface may lead to numerous problems like postoperative sensitivity, recurrent caries, marginal sealing and later total failure of restoration. Aim: To determine the sealing ability (microleakage) of a new hydraulic calcium silicate cement (Biodentine) as a base material and a bulk filling material. Materials and Methods: An in-vitro study was conducted with ten freshly extracted maxillary molars from March 2019 to June 2019. Standardised Class II cavities were prepared on mesial and distal proximal surfaces. The twenty prepared samples were randomly divided into two groups (n=10)- G1 (BiodentineTM group) and G2 (BiodentineTM + Tetric N-Ceram group). The samples were subjected to thermocycling, followed by 1% methylene blue dye immersion. They were subsequently sectioned both buccolingually and mesiodistally in order to separate to proximal restorations (mesial and distal) and appreciate the amount of dye penetration at tooth/restoration interface respectively. Microleakage was evaluated by observing the samples under a stereomicroscope (30x). The data obtained was analysed using one-way ANOVA test and compared using Post-hoc test. p-value <0.05 to be considered significant. Results: Biodentine showed no significant difference between the sealing ability as a bulk filling and a base material. Group G2 (0.35) indicated comparatively less microleakage as compared to Group G1 (0.6) but the difference in microleakages in both groups was insignificant (p-value 0.083467). Conclusion: BiodentineTM exhibits microleakage when used as a bulk filling material and as a dentin substitute in Class II cavities since there is no significant difference in both the groups.

Background: Prolonged retention of provisional restoration is depends upon the long term affinity between restoration and teeth structures which depend upon the use of good mechanical characteristics, low solubility, and superior adhesion  quality of luting cement which also resist bacterial and molecular penetration. Hence, assessment of marginal leakage of provisional restorative materials sealed with provisional cements using the standardized procedures is essential. Aim: This study aimed at in vitro study of microleakage of 4 provisional cements, a cavity base compound and a zinc-phosphate luting cement in provisional acrylic resin crowns fixed on extracted human teeth. Material & Methods:  The teeth with acrylic restorations were randomly divided into 6 groups of 6 specimens each. Each group received different types of temporary cement. Acrylic resin crowns were made and fitted on intact human premolars with the 4 cements. All restorations were applied in a standardized manner. Specimen were submerged in a 2% methylene blue solution, then sectioned and observed under a stereomicroscope for the detection of marginal microleakage. Obtained data were subjected to ANNOVA and Chi –square test to know statistical significant difference between different groups.  P value less than 0.05 was considered significant. Results and observation: Mean frequency of microleakage was statistically significant among all the groups under study with Pvalue<0.017. Conclusion: Among all the temporary cement, Zinc-phosphate cements has the best property with reduced microleakage. Even it is used as cavity base but can be used as good temporary cement as far as microleakage is concerned. Key words, Microleakage, provisional cement, restorations, crowns, in vitro

Abstract Background: Accurate working length (WL) determination is necessary in achieving optimal healing by non-surgical root canal therapy in teeth with inflammatory apical root resorption. Electronic apex locators (EALs) are one of the mainstays in determination of WL of teeth. Aim: This study evaluated the accuracy of three EALs [RootZX (third generation), iPex (fourth generation) and Raypex 6 (modification of a fifth generation)] in determining the WL of teeth with simulated apical root resorption in permanent teeth. Materials and methods: Forty freshly extracted maxillary anterior teeth were collected and a 45° oblique cut was made at the root apex with a disc to simulate apical root resorption. Actual working length (AWL) was determined by direct visual method and was used as a control. Electronic working length (EWL) values were measured by three different apex locators that are RootZX (Group I), iPex (Group II) and Raypex 6 (Group III) at apex, 0.5 mm and 1 mm from apex. All values obtained were tabulated and statistical evaluation was carried out. Results: At apex, EWL obtained using iPex (p=0.05) showed a statistically significant difference from AWL. At 0.5 mm and 1.0 mm tolerance, iPex showed non-acceptability for WL measurement in 67.5% and 17.5% of samples compared to Root ZX (12.5% and 2.5%) and Raypex (7.5% and none) respectively. Conclusion: Within the limitation of this study, it can be concluded that Raypex 6 and RootZX show statistically significant accuracy in WL measurement compared with iPex in teeth with apical root resorption.

AbstractThe objective of this in vitro study was to evaluate the influence of the Er,Cr:YSGG laser associated or not to a desensitizing agent in the treatment of erosive lesions. Forty specimens with dimensions of 4mm x 4mm and 3mm thickness were divided into 4 groups (n = 10): G1- no treatment; G2- 5% fluoride varnish; G3- Er,Cr: YSGG; G4 - fluoride varnish + laser. The specimens were immersed in erosive drink 3 times a day for 1 minute with an average interval of 2 hours between cycles for a period of 10 days. The treatments were performed according to the groups and the surface roughness and the wear profile were analyzed by scanning confocal microscopy. The normality (Kolmogorov-Smirnov) and homogeneity (Levene's) of the tests were evaluated. After these analyzes, the surface roughness data were submitted to the statistical analysis of Variance Analysis (ANOVA). All tests adopted a significance level of 5% (α = 0.05). At the representative images of the wear profile, the morphology of bovine dental enamel in its control and eroded areas were compared and qualitatively discussed. As regard surface roughness, there was no statistically significant difference between the groups. The qualitative analysis of the loss of volume showed that all experimental groups showed significant superficial morphology differences. Considering the limitations of an in vitro study, it can be concluded that the treatments performed were not able to treat dental erosion satisfactorily, indicating the need for more treatment sessions. Keywords: Tooth Erosion. YSGG Laser. ResumoO objetivo desse estudo in vitro foi avaliar a influência do laser Er,Cr:YSGG associado ou não a um agente dessensibilizante no tratamento de lesões erosivas. Foram confeccionados 40 espécimes com dimensões de 4mm x 4mm e 3mm de espessura, divididos em 4 grupos (n=10): G1- nenhum tratamento; G2- verniz fluoretado 5%; G3- aplicação do laser Er,Cr:YSGG; G4- verniz fluoretado + laser. Os espécimes foram imersos em bebida erosiva, 3 vezes por dia, durante 1 minuto, com intervalo médio de 2 horas entre os ciclos, por um período de 10 dias. Os tratamentos foram realizados de acordo com os grupos e que foram analisados a rugosidade superficial e o perfil de desgaste por meio de microscopia confocal de varredura. Avaliou-se a normalidade (Kolmogorov-Smirnov) e homogeneidade (Levene’s) dos dados. Após estas análises, os dados de rugosidade superficial foram submetidos ao teste estatístico de Análise de Variância (ANOVA). Todos os testes adotaram nível de significância de 5% (α=0,05). Através da obtenção das imagens representativas do perfil desgaste, a morfologia do esmalte dental bovino em suas áreas controle e erodida foram comparadas e qualitativamente discutidas. Quanto à rugosidade superficial, não houve diferença estatisticamente significativa entre os grupos. A análise qualitativa da perda de volume mostrou que todos os grupos experimentais apresentaram diferenças significativas na morfologia superficial. Considerando as limitações de um estudo in vitro pode-se concluir que os tratamentos realizados não foram capazes de tratar a erosão dentária de forma satisfatória, indicando a necessidade de mais sessões de tratamento. Palavras-chave: Erosão dentária. Introduction: Thorough, chemo-mechanical preparation of the root canal is required to completely eliminate microorganisms from the dentinal tubules. Faulty obturation in the apical area is the pathway from which bacteria and their toxins pass on to the periapical area and this leakage can be studied by sealability studies. Aim: To evaluate the apical sealing ability of three sealers i.e., AH Plus, Roekoseal and Adseal using passive dye penetration test and a dye extraction test with Methylene blue dye to evaluate the apical leakage. Materials and Methods: The present study was an experimental in-vitro study, carried out at MGM Dental College and Hospital, Navi Mumbai, Maharashtra, India over a period of 10 months (from August 2013 to May 2014). Eighty freshly extracted human permanent maxillary central incisor teeth were obtained. The crowns of the teeth were sectioned at/below cemento-enamel junction and divided into five groups: Groups I, II and III were obturated with gutta-percha and three different sealers, and groups IV and V were the two control groups. Passive dye penetration test was performed to evaluate penetration of the dye by means of capillarity followed by dye extraction test. At the end of dye extraction procedure, the supernatant solution obtained was analysed using an Ultraviolet spectrophotometer. Significance was set at 5%. Statistical analysis was performed by using IBM Statistical Package for the Social Sciences (SPSS) statistical software version 20 (Armonk, New York, United States). The normality of data in each group was tested using Shapiro-Wilk test. The mean absorbance values of the five groups were compared using one-way ANOVA. Further to compare the level of absorbance of dye among various groups, Tukey’s post-hoc test was applied. Results: The mean values of dye absorbance of all groups were as follows, AH Plus (0.02638), Roekoseal (0.06713), Adseal (0.03575), positive control group (1.36250) and negative control group (0.01894). One-way ANOVA showed p<0.001, thus indicating there was a significant difference between the dye absorbance values of all the groups. Tukey’s post-hoc test showed statistically significant difference in dye absorbance of the experimental groups when compared to positive control group. There was no significant difference in dye absorbance when experimental groups were compared to each other. When AH Plus was compared with Roekoseal, the p-value was 0.568 and 0.997 with Adseal whereas when Roekoseal was compared with Adseal the p-value was 0.778. Conclusion: The mean dye absorbance value of AH Plus was found to be least and that of Roekoseal was highest in the experimental groups. AH Plus gave the best results, as least dye was extracted from this group, indicating least dye penetration and minimum microleakage followed by Adseal and Roekoseal.

ABSTRACT Aim The aim of this study was to compare the effectiveness of nano-hydroxyapatite (nano-HAP) paste and fluoride varnish in remineralizing initial enamel lesion in young permanent teeth and their ability to resist secondary caries under dynamic pH cycling quantitatively and qualitatively. Materials and methods Initial caries-like lesions were artificially developed on 45 specimens. Specimens were divided into three groups: (1) Control (without treatment), (2) fluoride varnish (3M ESPE), and (3) nano-HAP paste (Desensibilize Nano P). The nano-HAP paste was applied twice separated by one pH cycle, and the varnish was applied only once followed by 7 days of pH cycling. All specimens were examined using DIAGNOdent® pen (KaVo, Germany), and a representative specimen was randomly selected from each group for qualitative evaluation using scanning electron microscope (SEM) at four stages: Baseline, after lesion formation, immediately after remineralization, and after pH cycling. Data were statistically analyzed with Statistical Package for the Social Sciences (SPSS), version 20. Results The degree of demineralization was significantly elevated in control group; however, no significant difference was found between fluoride varnish group and nano-HAP paste group (p < 0.001). Conclusion Nano-HAP paste showed promising long-term protective effect in terms of surface depositions and maintaining a smooth surface compared with fluoride varnish. Clinical significance Based on the findings of this study, nano-HAP paste might be recommended as alternative remineralizing agent with lower fluoride concentration than fluoride varnish that could be beneficial for children, pregnant females, and those who are at high risk of dental fluorosis. Background: The using of devices that emitted electromagnetic radiation has beenlimited in many healthcare centers to prevent interference with medical devices.The purpose of this in vitro study was to evaluate the effect of Wi-Fi Router(Tenda FH456) on working length determination using electronic apex locator(NSK-iPex II) in the presence of network connection with tablet (GT-N800) ornot.Materials and methods: Twenty-five intact teeth with a single root were sectioned atthe cementoenamel junction. Visually, roots were examined understereomicroscope (X20) and the working length was determined using a size 15K-file. The effect of Tenda Wi-Fi router on accuracy of electronic working lengthmeasurement of electronic apex locator (iPex II) was determined in this study inclosed room under 5 conditions (without router and tablet, with router in differentdistances 1 meter and 3 meters from electronic apex locator, with router andconnected with tablet in different distances 1 meter and 3 meters from electronicapex locator). The electronic working length was measured 3 times per canalunder each condition. The data were compared using analysis of variance.Results: The mean of working length measured under microscope was (13.30), andthe mean of working length by electronic apex locator was (13.14) while the meanvalues of working length in different conditions was fixed (13.12). Analysis ofvariance test (ANOVA) showed non-significant difference among working lengthmeasurements in different conditions (P-value ≥ 0.05).Conclusion: Within the limitations of the present study, stability and reliability ofiPex II apex locator in measurement of electronic working length where notinfluenced when placed in (1M, 3M) from Wi-Fi router and active Tablet(connected to network by Wi-Fi and streaming online videos). During root canaltherapy, active Tablet can be used in the dental operatory without the fear ofcausing electromagnetic interference in electronic working length determination.
