The main treatment method of the internal endometriosis nowadays is still surgical method - hysterectomy by laparoscopic or laparotomic way. The histeroscopic endometrial resection (ablation) is an alternative with regard of hysterectomy in female patients with surface adenomyosis forms in absence of pain syndrome. According to available information on our investigation, the endometrial resection (ablation) is very effective in 72% of female patients with adenomyosis combined with recidive endometrial hyperplasia.

The objective: to study the prevalence of ovarian endometriosis in women of reproductive age, the features of clinical and ultrasound picture of endometriosis. Patients and methods. The study involved 22 patients with endometriomas, the control group – 50 women gynecological and somatically healthy. Results. The incidence of ovarian endometriomas in the structure of benign tumor-like formations of ovaries is 0.62%. In 72.73% of ovarian endometriomas occur on a background of concomitant gynecological pathology. Clinically, in 77.27% of cases there is a pain syndrome, in 59.09% – algomenorrhea, in 13.64% – infertility; 18.18% of cases endometriomas remain «dumb» and proceed with the erased clinical picture. Sonologically in patients with endometriomas adenomyosis, endometrial hyperplasia, changes in the contralateral ovary are determined. Conclusion. Without additional methods of diagnostic gynecological examination may identify the 3 cm tumor-like formation of the ovary with different characteristics, which does not allow to differentiate endometrioma from inflammation, functional and ovarian tumors. Ultrasound can diagnose pathological ovarian formation of 1 cm, detailing the nature of the cyst. Key words: endometriosis, reproductive age, retrospective analysis, ultrasound.

The experience of surgical treatment of 154 patients having chronic pancreatitis with preferential injury of the pancreas head which underwent different operative interventions: isolated resection of pancreatic head ( based upon Berger surgery - 24, Frey surgery - 39), pancreatoduodenal resection ( with gastric resection - 22, with preserved pylorus - 43) and drainage surgeries - 26 is presented in the article. The surgery of isolated resection of pancreatic head has less number of nearest unfavorable results compared with pancretoduodenal resection with preserved pylorus. Proximal resection of the pancreas the variants of which are different isolated resection of pancreatic head is superior upon surgeries with full or partly resection of the duodenum due to fast normalization of the motor-evacuation function, less rate of the intestinal reflux and portion character of duodenal evacuation. In the follow-up period after pancreatoduodenal resection, atrophic processes occur in distal areas of the pancreas which are followed by clinical manifestations of exo- and endocrinous insufficiency. The surgery of longitudinal pancreatic jejunostomy does not avoid pathologic changes in the organ head and pain syndrome.

Introduction. The aim of this study was to analyze and compare the histopathological findings in curettage and hysterectomy specimens, to evaluate the accuracy of histopathological diagnosis in curettage specimens, and to determine the frequency of coexisting endometrial carcinoma in patients with histopathological diagnosis of endometrial hyperplasia. Material and methods. Curettage and hysterectomy specimens of 135 female patients with initially diagnosed endometrial hyperplasia were retrospectively analyzed and compared. Results. Simple hyperplasia was found in 49 patients (36.3%), complex hyperplasia in 14 (10.4%), simple atypical hyperplasia in 24 (17.8%), and complex atypical hyperplasia in 48 (35.5%) patients. After hysterectomy, 59 (43.7%) patients were found to have simple hyperplasia, 12 (8.9%) complex hyperplasia, 15 (11.1%) simple atypical hyperplasia, 18 (20.7%) complex atypical hyperplasia, and 21 (15.5%) endometrial carcinoma. The accuracy of histopathological diagnosis of endometrial hyperplasia in curettage specimens was 82.2-89.6% and dependent on the types of hyperplasia. The frequency of coexisting endometrial carcinoma was significantly higher (p<0.001) in patients with atypical hyperplasia than in patients with hyperplasia without cytological atypia. After hysterectomy, coexisting endometrial carcinoma was found in 27.8% of patients with histopathological diagnosis of atypical hyperplasia in curettage specimens. In contrast to simple atypical hyperplasia, the frequency of coexisting endometrial carcinoma was significantly higher (p<0.05) in complex atypical hyperplasia. Conclusion. The frequency of coexisting endometrial carcinoma in hysterectomy specimens in patients with histopathological diagnosis of atypical hyperplasia in curettage specimens was relatively high and it should be taken into account when planning therapy. .

ABSTRACT Aim The aim of this review is to summarize and discuss the available information concerning the connection between the tooth and implant in a fixed partial denture. Background Although implant-supported prosthesis (ISP) has substantial biological and biomechanical advantages, due to the extensive use of implant in partially edentulous patients, encounter to cases of obligatory connection between the tooth and implant is not rare. However, because of their differences, especially in support mechanism, long-term prognosis of this treatment method is a special debate in dental literature. Materials and methods The literature published over the past 25 years was searched through PubMed, Medline, Google and indexed journals (search terms: tooth implant connection, tooth implant-supported fixed partial denture, tooth implant splinting, implant and tooth). The most valuable and relevant articles were selected and analyzed. Results and discussion The existing studies reveal that there are certain conditions in which this method is applicable. The main advantage of the method based on literature reviewed is reducing the need to the removable prosthesis in patients that otherwise require it. The disadvantages of this connection, mainly due to different movement range of components under loading, reportedly will be minimized if some guidelines are followed. Clinical significance According to available studies, this literature review supports tooth-implant connection technique where indicated, with complete attention to prudent guidelines. Further researches preferably concentrating on new methods and also long-term longitudinal studies are certainly needed before this approach can be widely used. Importance. Exfoliative cheilitis is an inflammatory disease accompanied by damage to the red border of the lips. With dry exfoliative cheilitis, scales form on the lips, which are densely soldered to the underlying tissue in the center, and along the periphery, on the contrary, are raised. At the same time, patients indicate a feeling of dryness, burning. When the exudative form of cheilitis is observed puffiness, sore lips. Diagnosis of exfoliative cheilitis is reduced to the collection of complaints, the preparation of anamnesis of the disease, physical examination. For the treatment of exfoliative cheilitis, Bucca rays, moisturizing creams, acupuncture techniques, as well as agents that increase body resistance are used. As it turned out, in practice, all existing methods of treatment are ineffective, and the disease is manifested by frequent relapses. Pathology is more common in women. The dry form of the disease is mainly detected at the age of 16-40 years. Exudative cheilitis is diagnosed in patients aged 16-65 years. Objectives ― evaluate the effectiveness of lip balm based on lamb fat, and make a conclusion based on the clinical data and patient's subjective sensations. Methods. Twenty patients aged from 18 to 45 years with a diagnosis of exfoliative cheilitis were selected for work. All patients were divided into two groups: control (10 people) and main (10 people). Local therapy in both groups began according to the generally accepted scheme: sanation of the oral cavity, treatment of periodontal diseases according to indications, selective grinding off of the sharp edges of the teeth. In the main group, patients received treatment in the form of an application with a cool furatsilina solution, and applying the dosage form in the form of lip balm., Consisting of red-hot lamb fat and filtered through gauze, lanolin, pumpkin oil, ylang ylang oil, geranium oil, and also vitamins oil A and E and medical paraffin, this dosage form was prepared in paraffin and sterilized, after complete cooling it was applied to the lips with a spatula, and the patients easily used it at home as a balm for K in the morning and evening. Patients of the control group received the traditional treatment of cheilitis, anestezin applications, lip treatment with an antiseptic solution and keratoplasty applications - vitamin A and E oil. The results of the treatment were evaluated according to the clinical examination of patients, as well as taking into account the subjective feelings of the patients. Results. Based on the schemes of complex treatment of exfoliative cheilitis in patients of the main group, a positive result was obtained after the 3rd visit. A decrease in pain syndrome, a decrease in lesion elements (flakes), epithelization of cracks, a decrease in pain syndrome, a decrease in complaints of dryness of the red border of the lips were revealed. Conclusions. The development of this algorithm for treating ecfoliative cheilitis, including a mutton fat lip balm, has reduced the treatment time for this pathology.

Objective — to highlight the issue of difficulties in the diagnosis and treatment of patients with pyoderma gangrenosum (PG). Materials and methods. A literature review and analysis of the results of the clinical examination of patients with PG was performed. Pyoderma gangrenosum is most commonly observed in young and middle-aged people, more frequently in women. The pathogenesis of PG has not been fully studied. It is believed to be related to genetic mutations, neutrophil dysfunction and impaired immunity/inflammation regulation. Maverakis and others developed diagnostic criteria for PG. First-line treatment is aimed at optimizing the local wound care. For more severe diseases, systemic therapy is required. Oral corticosteroids (0.5—1 mg/kg/day) are the basis of treatment and used for control of PG. Cyclosporine can be used either alone or in combination with corticosteroids. At present, there is an increasing evidence of effectiveness of biological therapy as treatment method for a number of cytokines. Results and discussion. This article presents a clinical case of a patient diagnosed with PG. Based on recent guidelines, systemic corticosteroid therapy (Medrol starting at 48 mg/d), systemic anti-inflammatory and vascular drugs, topical therapy and treatment results were presented. Conclusions. Without treatment, the disease lasts for months and years. Ulcers sometimes grow very fast, reaching huge sizes in a matter of days. After the healing of some ulcers new ones often emerge. Pain syndrome, the addition of a secondary infection and the continuous progression of the pathological process lead to a decrease in the patient’s quality of life, as well as to a deterioration in his social adaptation. Therefore, timely adequate diagnosis, treatment and full compliance of the patient are the key to success.
