Peculiarities of hemostasis in transurethral resection of prostatic gland using fybrinolysis inhibitor

Tissue plasminogen activator being the most important component part of fybrinolysis is producing and accumulating in the prostatic gland. In surgical treatment of prostate an increased bleeding often occurs during the surgical operations on the prostate during as well as in post-operative period. General and local fybrino- lysis increase is in case among the reasons of haemor-ragy as a result of mechanical action on prostatic gland tissue in the process of an operation. It is suggested the use of Tranexam a tranexamic acid drug to neutralize an increasing fybrinolysis. In 83 patients with benign prostate hyperplasia, which underwent by transurethral resection of prostate, significantly positive effect of Tranexam was found, prescribed in therapeutic doses before operation, during operation and in early postoperative period.

Abstract Background:The correlation between modified bladder outlet obstruction index (MBOOI) and surgical efficacy still remains unknown. The purpose of the study was to investigate the clinical value of the MBOOI and its use in predicting surgical efficacy in men receiving transurethral resection of the prostate (TURP).Methods :A total of 403 patients with benign prostate hyperplasia (BPH) were included in this study. The International Prostate Symptom Score (IPSS), quality of life (QoL) index, transrectal ultrasonography, and pressure flow study (PFS) were conducted for all patients. The bladder outlet obstruction index (BOOI) (PdetQmax -2Qmax) and MBOOI (Pves-2Qmax) were calculated. All patients underwent TURP, and surgical efficacy was accessed by the improvements in IPSS, QoL, and Qmax 6 months after surgery. The association between surgical efficacy and baseline factors was statistically analyzed.Results:A comparison of effective and ineffective groups based on the overall efficacy showed that significant differences were observed in PSA, Pves, PdetQmax, Pabd, BOOI, MBOOI, TZV, TZI, IPSS-t, IPSS-v, IPSS-s, Qmax, and PVR at baseline (P<0.05). Compared with BOOI, the results of Pearson’s correlation analysis showed that there was a more optimal correlation of MBOOI with preoperative variables, as well as with the changes postoperative(P<0.05). Binary logistic regression analysis suggested that MBOOI was the only baseline parameter correlated with the improvements in IPSS, QoL, Qmax, and the overall efficacy. Additionally, the ROC analysis further verified that MBOOI was more optimal than BOOI, TZV and TZI in predicting the surgical efficacy.Conclusion:Compared to BOOI, MBOOI may be a more useful factor that can be used to predict the surgical efficacy of TURP.Trial registration: retrospectively registered.

Background: Bipolar Transurethral Resection of Prostate (TURP) and monopolar TURP has been widely used for surgical management of bladder outlet obstruction due to benign prostatic hyperplasia. The aim of this study is to prove the difference of haemoglobin (Hb), sodium levels (Na⁺), application of urethral catheter traction between two groups.Methods: Prospective cohort study was conducted for 82 patients as eligible sample which divided into two groups, bipolar TURP and Monopolar TURP from September 2018 to August 2019. The patient’s demographics, preoperative, postoperative data, and application of urethral catheter traction were recorded.Results: The result shows basic characteristic data seem comparable. The decline levels of Hb before surgery 14.2(2.6) and after surgery 13.6(2.6) in monopolar group was significant (p=0.01), and also in bipolar group 13.7(2.1) vs 13.4(2.1) was significant (p=0.033), despite decreasing Hb levels between two groups were insignificant (p=0.639) but decline levels of Na between two groups were significant (p=0.013) Na⁺ level in bipolar 0(3) and monopolar 1(4). The application of urethral catheter traction in bipolar (19.5%) lower than monopolar (80.5%) were statistically significant (p<0.001).Conclusions: Based on the results of this study, bipolar TURP surgery can be an optional recommendation in treating Benign Prostate Hyperplasia (BPH) in terms of better preserving blood sodium during surgery and low use of traction without significant bleeding complications.

Objectives: To evaluate urinary symptoms and quality of life in patient with BPH before and after TURP. To determine the impact of TURP on the urinary symptoms (IPSS) and peak urinary flow rate. Methods: This study is prospective study carried out between 2010 and 2011 in the department of Urology, National Institute of Kidney Diseases & Urology. Total 102 cases were selected purposively according to selection criteria. Each patient was observed and followed up at 8 weeks (1st visit), 16 weeks (2nd visit) 24 weeks (3rd visit) after transurethral resection of prostate (TURP). IPSS score, QOL score also recorded and uroflowmetry was done to see the peak urinary flow rate (Qmax) of urine and voiding time. USG was done to see post voidal residual urine volume and DRE also done in selected cases. Data was complied and statistical analysis were done using computer based software, Statistical Package for Social Science (SPSS), using paired ‘t’ test. A P value <0.05 was taken as significance. Results: Before TURP, IPSS range 17-25 and mean 21.61+2.43, after TURP, range 0-7 and mean 4.27+1.71). Hence a significant improvement of IPSS was found from 2 months to 6 months follow up after TURP. The change was tested using “paired student ‘t’ test”. Before TURP Qmax range 7-12.2 and mean was 9.96+1.69, which became range 18-25 and mean was 22.61+2.28 after TURP and therefore change of mean Qmax was 12.64+2.69. The change was tested using “paired student ‘t’ test”. The change was found significant (P<0.001). Conclusion: Transurethral resection of prostate resolves obstructive symptoms, rapid improvement of urinary flow rate Bangladesh Journal of Urology, Vol. 16, No. 1, Jan 2013 p.11-15
