Background: Early diagnosis of lung cancer can reduce its mortality and morbidity. Minimally invasive image guided percutaneous core needle biopsy can obtain tissue sample for diagnosis and staging of lung cancer, which is crucial for correct management of lung lesions. Common complications of lung biopsy include pneumothorax, parenchymal haemorrhage and haemoptysis. The study was aimed to determine diagnostic yield and complications of the percutaneous computed tomography (CT) guided core needle biopsy of lung lesion in tertiary hospital.Methods: Hospital based prospective study was performed in 40 patients in Tribhuvan University Teaching Hospital. CT guided biopsy of lung lesions was performed with 18-gauge semi-automated biopsy instrument. The complications following the biopsy were recorded and correlated with different factors using chi-square test. Histopathology report were obtained to measure the diagnostic yield.Results: Among 40 patients who underwent guided lung biopsy, histopathology showed definitive diagnosis in 37 patients; 31 malignant and 6 benign lesions. Parenchymal haemorrhage, pneumothorax and haemoptysis were seen in 13, 8 and 5 respectively; however, none required active intervention. Emphysema in traversing lung and numbers of pleural punctures used were predictive factors of complication (p value <0.05).Conclusions: The study showed percutaneous image guided core needle biopsy has high diagnostic yield with fewer complication rates and is thus recommended for routine biopsies of lung lesions.

Objectives: The aim of this retrospective study was to investigate the relationship between lung lesion lobar distribution, lesion size, and lung biopsy diagnostic yield. Material and Methods: This retrospective study was performed between January 1, 2013, and April 30, 2019, on CT-guided percutaneous transthoracic needle biopsies of 1522 lung lesions, median size 3.65 cm (range: 0.5– 15.5 cm). Lung lesions were localized as follows: upper lobes, right middle lobe and lingual, lower lobes superior segments, and lower lobes basal segments. Biopsies were classified as either diagnostic or non-diagnostic based on final cytology and/or pathology reports. Results were considered diagnostic if malignancy or a specific benign diagnosis was established, whereas atypical cells, non-specific benignity, or insufficient specimen were considered non-diagnostic. Results: The positive predictive value (PPV) of a diagnostic yield was 85%, regardless of lobar distribution. Because all PPVs were relatively high across locations (84–87%), we failed to find statistically significant difference in PPV between locations (P = 0.79). Furthermore, for every 1 cm increase in target size, the odds of a diagnostic yield increased by 1.42-fold or 42% above 85%. Although target size increased the diagnostic yield differently by location (between 1.4- and 1.8-fold across locations), these differences failed to be statistically significant, P = 0.55. Conclusion: Percutaneous transthoracic needle biopsy of lung lesions achieved high diagnostic yield (PPV: 84– 87%) across all lobes. A 42% odds increase in yield was achieved for every 1 cm increase in target size. However, this increase in size failed to be statistically significant between lobes.

PURPOSE In an initial evaluation of 1,500 computed tomography (CT)-guided core-needle biopsies performed at our institute during the period from 1989 to 1994, we encountered 100 patients with the diagnosis of lymphoma. Here, we review the clinical impact of 109 image-guided needle biopsies in these 100 patients with non-Hodgkin's lymphoma (NHL) and Hodgkin's disease (HD). PATIENTS AND METHODS NHL was diagnosed in 71 patients, and 29 had HD. Among the NHL patients, 17 (24%) had proven lymphoma diagnosed before the biopsy was performed; in 54 (76%) core-needle biopsy was performed as the first diagnostic procedure. Of 29 HD patients, nine (31%) were already established cases of HD, and in 20 (69%) core-needle biopsy was the first diagnostic procedure attempted. Most of the biopsies were performed under CT control using a 20- or 18-gauge Turner biopsy needle. RESULTS Eighty-six patients received therapy based on the results of the needle biopsy alone. Fourteen patients received therapy after undergoing surgical biopsy for a suspected diagnosis of lymphoma, which could not be established with certainty on the basis of an earlier core-needle biopsy alone. In 78% of the patients, the needle biopsy saved a further surgical procedure that may have been difficult to perform because of the primary location of the tumor. CONCLUSION From our experience in this study, image-guided core-needle biopsies provide sufficient information for the diagnosis of and subsequent therapeutic decision to treat most cases of lymphoma.

TÓM TẮT Mục tiêu: Xác định độ chính xác và tỉ lệ tai biến của phương pháp sinh thiết u phổi xuyên thành ngực bằng kim dưới hướng dẫn CTscan. Phương pháp và đối tượng: Hồi cứu, mô tả các bệnh nhân được sinh thiết u phổi xuyên thành ngực bằng kim dưới hướng dẫn CTscan tại bệnh viện Bình Dân từ 3/2019 đến hết 12/2019. Kết quả: Tổng số mẫu là 57 bệnh nhân, nam gần gấp đôi nữ, tuổi trung bình 60,7. Phần lớn bệnh nhân tình cờ phát hiện u phổi (78,9%). U phổi bên phải nhiều hơn bên trái. Kết quả sinh thiết: Ung thư 57,9%; viêm 35,1% và lao 7%. Các trường hợp kết quả sinh thiết là lao: phù hợp với bệnh cảnh lâm sàng, hình ảnh học và được chẩn đoán xác định bởi bác sĩ chuyên khoa lao - Bệnh viện Phạm Ngọc Thạch. Các trường hợp kết quả sinh thiết là viêm: kết quả phẫu thuật là lao 50%, ung thư 20%, viêm 20%. Các trường hợp kết quả sinh thiết là ung thư: so sánh với kết quả phẫu thuật chúng tôi nhận thấy độ nhạy trong phát hiện ung thư phổi của sinh thiết là 85,7%; độ đặc hiệu 100%; giá trị tiên đoán dương 100% và giá trị tiên đoán âm 80%. Tỉ lệ tai biến 21,1% gồm tràn khí màng phổi 12,3%, ho ra máu 7% và tụ máu nhu mô phổi 1,8%. Tỉ lệ tràn khí màng phổi cần dẫn lưu màng phổi cấp cứu là thấp (3,5%). Kết luận: Phương pháp sinh thiết u phổi xuyên thành ngực bằng kim dưới hướng dẫn CTscan khả thi, hiệu quả cao và tương đối an toàn, vì vậy có vai trò quan trọng trong chẩn đoán u phổi. ABSTRACT EVALUATE THE EFFECTIVENESS OF COMPUTED TOMOGRAPHY - GUIDED TRANSTHORACIC CORE NEEDLE BIOPSY OF PULMONARY TUMORS Objectives: The present study aims to determine the diagnostic accuracy of computed tomography (CT) - guided transthoracic core needle biopsy of pulmonary tumorsand the complications of the procedure. Methods: A retrospective descriptive studywas carried out in a series of patients with pulmonary tumors diagnosed by CT - guided transthoracic core needle biopsy at Binh Dan Hospital between 3/2019 and 12/2019. Results: The total sample was 57 patients with an average age was 60.7, male/female = 1.85. Most of the patients were detected lung tumors incidentally (78.9%), and right-side tumors were more than the left - side tumors. Biopsy results included cancer, inflammation, and tuberculosis, with incidence rates were 57.9%, 35.1%, and 7%, respectively. The cases, which were confirmed tuberculosis on biopsy results, were determined tuberculosis by a specialist who worked in Pham Ngoc Thach Hospital. The patients had inflammatory results and were operated on, which had surgical outcomes were tuberculosis (50%), cancer (20%), and inflammation (20%). The cases, which were confirmed cancer on biopsy results after comparing with surgical outcome, found that the biopsy of detecting lung cancer had 85.7% sensitivity, 100% specificity, 100% positive predictive value, and 80% negative predictive value. The rate of complications was 21.1%, including pneumothorax (12.3%), hemoptysis (7%), and pulmonary parenchymal hematoma (1.8%). The rate of pneumothorax requiring emergency pleural drainage was low (3.5%). Conclusions: CT - guided transthoracic core needle biopsy is feasible, highly effective, and relatively safe; therefore, it plays an important role in diagnosing lung tumors.
