ABSTRACT Intrapelvic hemangiosarcoma (IP HSA) is an uncommon primary location of canine HSA, and its presentation appears to be different than that of other intra-abdominal HSA locations. The objectives of this study were to report on the clinical presentation, diagnostic options, and clinical outcomes of dogs with IP HSA. Medical records from two veterinary teaching hospitals between 2000 and 2019 were reviewed for dogs diagnosed with IP HSA. Inclusion criteria consisted of a cytologic or histopathologic diagnosis of HSA from a mass arising from the muscles of the pelvic canal. Exclusion criteria included masses arising from organs within the pelvic canal, including the urethra, prostate, and urinary bladder. Twelve dogs were ultimately included in the study. Clinical presentation of IP HSA was varied, with clinical signs including hindlimb lameness, urethral obstruction, and evidence of hemoperitoneum. The median progression free survival was 92 days (range: 1–1057 days). The overall survival time was 165 days (range: 1–1170 days). IP HSA is an underreported location of canine HSA with similar biologic behavior and outcome compared with visceral HSA. It serves as an important differential in patients presenting with new-onset lameness and urethral obstruction.

Latar Belakang. Wilayah Jakarta Timur menduduki posisi lokasi fokus stunting, salah satunya di Kelurahan Tengah. Berdasarkan pengolahan data awal, diketahui prevalensi stunting pada enam posyandu di dua RW terpilih mencapai angka 25,9%.Tujuan. Mengetahui perbandingan pola asuh balita stunting dan tidak stunting di Kelurahan Tengah, Kecamatan Kramat Jati Metode. Penelitian kualitatif dengan pendekatan studi kasus melalui wawancara mendalam secara daring. Sampel dipilih secara purposive sampling sesuai kriteria inklusi dan eksklusi dengan 12 informan utama yang memiliki balita stunting dan tidak stunting usia 24-59 bulan, serta informan kunci terdiri dari, Tenaga Pelaksana Gizi dan kader posyandu.Hasil. Hasil penelitian terhadap informan utama dengan balita stunting menunjukkan bahwa sebagian besar ibu tidak memberikan ASI Eksklusif kepada anaknya, memberikan makan dengan frekuensi yang kurang, variasi makanan tidak beragam karena anak banyak diberikan jajanan. Selain itu, ibu dengan anak stunting juga mendapatkan dukungan psikososial yang rendah serta rendahnya partisipasi ke Posyandu.Kesimpulan. Terdapat perbedaan antara pola asuh ibu dengan balita stunting dan ibu dengan balita tidak stunting. ABSTRACTBackground. East Jakarta region has become the primary location of stunting, one of which is Tengah Village. Based on the weighing and preliminary data, the prevalence of stunting in 6 Posyandu in the two selected RWs reached 25.9%. Objective to find out the comparison of parenting patterns of stunting toddlers and non-stunting toddlers in Tengah Urban-Village, Kramat Jati Sub-District Method. This research is qualitative, with a case-study approach and collected through in-depth online interviews. The sample was selected by purposive sampling, based on the inclusion and exclusion criteria which 12 main informants had stunting and not stunting toddlers aged 24-59 months and key informants consisting of Nutrition Workers in the Puskesmas Tengah and Posyandu cadres. Results. The research results on key informants with stunting toddlers show that most mothers didn’t give exclusive breastfeeding to their children, provide food with less frequency, food variations are not varied because children are given a lot of snacks. Mothers with stunting toddlers also experienced low psychosocial support and low participation in Posyandu.Conclusion. There is a difference between mothers with stunting toddlers' parenting patterns and mothers with non-stunting toddlers.

<p>The musical object occupies a strange place in music criticism. The new musicology schools influenced by post-structuralist continental thought have shied away from the object’s autonomous existence, exemplified by Christopher Small’s view of music as a cultural activity: “musicking.” Other theorists, such as Dennis Smalley, have created taxonomies of musical sound. Smalley’s spectromorphology defines sonic typologies that he claims to be based on an experiential understanding of sound, while simultaneously undertaking the technical project of a systematic cataloguing of sounds. Both views inhabit quite opposite positions in relation to the sound object – either a total rejection of its reality or a positivistic attempt at a catalogue of sound types. Both of these approaches suffer from distancing the sonic object through their respective discourse: by reducing the importance of the object for the sake of viewing music as a network of cultural relations, or by reducing it to an idealized and rationalized object, seeing it as just the product of a bundle of auditory qualities unified by perception. These views introduce a distance from auditory experience, which is at its core an object-oriented experience. In other words, neither meets the musical object on its own level, and because of this, they deny or caricature the musical object’s ontology.  Graham Harman’s philosophical study of Object-oriented Ontology is a radicalization of Heideggerian phenomenology. Through a new reading of Heidegger’s tool-analysis, Harman argues that objects – whether real, living, non-living, ideal or abstract – are the primary location of ontological investigation, and that objects exist both discretely and as a part of a wider network of possible relationships. By viewing the object this way, and by recognizing the multifaceted and multidimensional features of the musical object, we may be able to account for features of music that the trends above are unable to recognize or assess, such as the twentieth century aesthetic practices of György Ligeti, Salvatore Sciarrino, and the Spectral school of composition. It is possible to read these composer’s aesthetics as object-oriented because they are so strongly focused on examining sonic objects themselves –whether it is a physical event or modeling a natural process – instead of examining objects only through their affective potential towards human beings. This practice suggests that these qualities and processes are themselves areas for possible contemplation. Historically, this move away from an emphasis on the human-world binary goes against the nineteenth century aesthetic of Romanticism, which relies on an object’s affective potential. Also, an object-oriented position rejects formalism, because of its reduction of music to an intellectual activity. An object-oriented approach to music traverses the line between these two positions, acknowledging the subtle and shifting relationships between the affective and the analytic or, to locate this within Harman’s approach, between the sensual and real. The thesis will explore the implications of an object oriented approach to music, trace the history of its development in relation to music – chiefly that of the twentieth and twenty-first centuries – as well as make object oriented analyses of selected works, including my own compositions.</p>

Cancer of unknown primary (CUP), in which metastatic diseases exist without an identifiable primary location, accounts for about 3–5% of all cancer diagnoses. Successful diagnosis and treatment of such patients are difficult. This study aimed to assess the expression characteristics of 90 genes as a method of identifying the primary site from CUP samples. We validated a 90-gene expression assay and explored its potential diagnostic utility in 44 patients at Jiangsu Cancer Hospital. For each specimen, the expression of 90 tumor-specific genes in malignant tumors was analyzed, and similarity scores were obtained. The types of malignant tumors predicted were compared with the reference diagnosis to calculate the accuracy. In addition, we verified the consistency of the expression profiles of the 90 genes in CUP secondary malignancies and metastatic malignancies in The Cancer Genome Atlas. We also reported a detailed description of the next-generation coding sequences for CUP patients. For each clinical medical specimen collected, the type of malignant tumor predicted and analyzed by the 90-gene expression assay was compared with its reference diagnosis, and the overall accuracy was 95.4%. In addition, the 90-gene expression profile generally accurately classified CUP into the cluster of its primary tumor. Sequencing of the exome transcriptome containing 556 high-frequency gene mutation oncogenes was not significantly related to the 90 genes analysis. Our results demonstrate that the expression characteristics of these 90 genes can be used as a powerful tool to accurately identify the primary sites of CUP. In the future, the inclusion of the 90-gene expression assay in pathological diagnosis will help oncologists use precise treatments, thereby improving the care and outcomes of CUP patients.

AbstractTumor tissue of origin (TOO) is an important factor for guiding treatment decisions. However, TOO cannot be determined for ~3% of metastatic cancer patients and are categorized as cancers of unknown primary (CUP). As whole genome sequencing (WGS) of tumors is now transitioning from the research domain to diagnostic practice in order to address the increasing demand for biomarker detection, its use for detection of TOO in routine diagnostics also starts becoming within reach. While proof of concept for the use of genome-wide features has been demonstrated before, more complex WGS mutation features, including structural variant (SV) driver and passenger events, have never been integrated into TOO-classifiers even though they bear highly characteristic links with tumor TOO. Using a uniformly processed dataset containing 6820 whole-genome sequenced primary and metastatic tumors, we have developed Cancer of Unknown Primary Location Resolver (CUPLR), a random forest based TOO classifier that employs 502 features based on simple and complex somatic driver and passenger mutations. Our model is able to distinguish 33 cancer (sub)types with an overall accuracy of 91% and 89% based on cross-validation (n=6139) and hold out set (n=681) predictions respectively. We found that SV derived features increase the accuracy and utility of TOO classification for specific cancer types. To ensure that predictions are human-interpretable and suited for use in routine diagnostics, CUPLR reports the top contributing features and their values compared to cohort averages. The comprehensive output of CUPLR is complementary to existing histopathological procedures and may thus improve diagnostics for patients with CUP.

The prognosis of malignant gliomas remains poor, with median survival fewer than 20 months and a 5-year survival rate merely 5%. Their primary location in the central nervous system (CNS) and its immunosuppressive environment with little T cell infiltration has rendered cancer therapies mostly ineffective, and breakthrough therapies such as immune checkpoint inhibitors (ICIs) have shown limited benefit. However, tumor immunotherapy is developing rapidly and can help overcome these obstacles. But for now, malignant gliomas remain fatal with short survival and limited therapeutic options. Oncolytic virotherapy (OVT) is a unique antitumor immunotherapy wherein viruses selectively or preferentially kill tumor cells, replicate and spread through tumors while inducing antitumor immune responses. OVTs can also recondition the tumor microenvironment and improve the efficacy of other immunotherapies by escalating the infiltration of immune cells into tumors. Some OVTs can penetrate the blood-brain barrier (BBB) and possess tropism for the CNS, enabling intravenous delivery. Despite the therapeutic potential displayed by oncolytic viruses (OVs), optimizing OVT has proved challenging in clinical development, and marketing approvals for OVTs have been rare. In June 2021 however, as a genetically engineered OV based on herpes simplex virus-1 (G47Δ), teserpaturev got conditional and time-limited approval for the treatment of malignant gliomas in Japan. In this review, we summarize the current state of OVT, the synergistic effect of OVT in combination with other immunotherapies as well as the hurdles to successful clinical use. We also provide some suggestions to overcome the challenges in treating of gliomas.

ABSTRACT: Parasitic or wandering leiomyoma are rare extra-uterine benign pedunculated sub-serous leiomyoma which after being detached from its primary location thrives in secondary location from blood supply from nearby feeding vessels. This case represents a multiparous woman, with one such parasitic leiomyoma encountered at laparotomy, proven by histopathological examination.

Abstract BackgroundIt is well recognized that risk stratification of gastrointestinal stromal tumors (GISTs) is closely related to tumor size, mitotic index (MI), and primary location. Among these three parameters, tumor size and primary location are easily established, while MI is subjective and its repeatability is poor. It is thus necessary to identify a biomarker to represent the true MI. Expression status and biological or prognostic significance of mitotic marker phosphohistone H3 (PHH3) and cell proliferation marker Ki67 in GIST have not been clearly understood until now. MethodsAn immunohistochemistry experiment was performed to detect the expression status of PHH3 and Ki67 in 125 paraffin-embedded GIST samples. All of the patients were followed up until September 30, 2019. ResultsThe MI determined using stained hematoxylin and eosin (H&E) sections (MI-H&E) and immunohistochemically positive PHH3 index (PHH3-IHC) was compared among groups of different genders, ages, primary locations, and histological subtypes, showing that the difference was not statistically significant (P>0.05). MI-H&E and the immunohistochemically positive Ki67 index were positively correlated (r=0.273, P=0.001), but the correlation was lower than that with the PHH3-positive index (r=0.705, P=0.000). The PHH3-positive index was also positively correlated with the Ki67 index (r=0.224, P=0.006). MI-H&E were positively correlated with MI quantified using immunohistochemically stained PHH3 sections (MI-PHH3) (P<0.05). After using PHH3 to perform MI quantification, the risk stratification of five GIST cases was changed, where two cases were given a higher risk grade and three cases were given a lower risk grade. Follow-up data were obtained from 98 cases (98/125, 78.4%), including two cases of metastasis and one death. Both metastatic and death cases had high MI-H&E. One metastatic case and one death case had high PHH3-positive indexes, while the remaining metastatic case had a low PHH3-positive index. ConclusionImmunohistochemical PHH3 labeling is a potentially useful tool for risk stratification and prognostic analysis in GIST. Using immunohistochemical PHH3 labeling makes it more convenient for pathologists to determine the MI for GIST. MI quantification with immunohistochemical PHH3 sections can be used as an adjunct tool for risk stratification and prognostic analysis of GIST, but cannot completely replace MI quantification using stained H&E sections. The Ki67 index is positively correlated with MI-H&E, although the efficiency of tumor risk stratification is lower than that of PHH3.

Abstract The indoor environment is the primary location for the transmission of severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2), the causative agent of coronavirus disease 2019 (COVID-19), largely driven by respiratory particle accumulation in the air and increased connectivity between the individuals occupying indoor spaces. In this study, we aimed to track a cohort of subjects as they occupied a COVID-19 isolation dormitory to better understand the impact of subject and environmental viral load over time, symptoms, and room ventilation on the detectable viral load within a single room. We find that subject samples demonstrate a decrease in overall viral load over time, symptoms significantly impact environmental viral load, and we provide the first real-world evidence for decreased aerosol SARS-CoV-2 load with increasing ventilation, both from mechanical and window sources. These results may guide environmental viral surveillance strategies and be used to better control the spread of SARS-CoV-2 within built environments and better protect those caring for individuals with COVID-19.
