Penile strangulation is a very rare but most serious clinical condition. It usually results from positioning of any foreign object at base of penis for sexual stimulation. If urgent intervention is not done this may lead to serious complications even death? In this study usual common method were unsuccessful therefore partial penectomy was done.

Penile strangulation is an infrequent clinical condition that has widely been reported. It usually results following placement of a constriction device to enhance sexual stimulation. Early treatment is essential to avoid potential complications, including ischemic necrosis and autoamputation. We describe the use of a Large Orthopedic Pin Cutter to remove a metal penile constriction device in the Emergency Department (ED). This case report describes the relatively safe technique of using an instrument available in many hospitals that can be added to the physician’s arsenal in the removal of metal constriction devices.

Dry gangrene of the penis is a critical clinical condition provoked by vascular compromise that can lead to severe complications. Although usually caused by diabetes mellitus or due to end-stage renal disease, in these last years, there has been an increase of penile gangrene incidents induced by penile strangulation due to constricting devices for sexual stimulation purposes. We present such a case and discuss the correct approach for treatment of this unusual condition.

Penile incarceration is a rare but serious problem. Rapid treatment is necessary to prevent penile infarction. Glass bottles, metal rings, steel nuts and bull rings have been reported as constricting agents. Depending on the type of material, various tools such as diamond drills, bolt cutters and glass saws have been used to remove the objects. Penile strangulation by metal ring is a rare surgical emergency situation which requires urgent intervention to avoid vascular and urethral injury. It is usually associated with an attempt to improve sexual act and/or to prolong erection, psychosexual disorder. It is difficult to cut a metal ring as the edema was out of proportion and metal ring had made pressure ulcer at pen scrotal junction.  The treatment of penile strangulation is decompression of the constricted penis to facilitate free blood flow and micturition. It requires no particular skill but does require resourcefulness to perform the removal simply and effectively, and with as little discomfort for the patient as possible. Here, author presented a case of penile strangulation in which metal ring was removed by silk thread.

Strangulation of penis is a rare clinical entity, which requires urgent urological management to prevent its devastating outcomes. The treatment of penile strangulation is immediate decompression of the constricted penis to facilitate free blood flow. Many different techniques have been described in literature to remove the constricting penile foreign bodies, but there is no universally accepted technique. Each case needs individualized handling in removing the foreign body. The procedure should be done with as little discomfort to the patient as possible and under anesthesia. Here we present to you a case report on a rare case of a metallic ring penile foreign body causing penile strangulation.

Penile strangulation caused by foreign bodies mostly occurs in adolescents and adult male. A man may place metal object for erotic or autoerotic purposes, for masturbation or increasing penile erection, and due to psychological disturbances which lead to penile strangulation. Penile strangulation results in reduced blood flow distal to the injury, leading to edema, ischemia, and sometimes gangrene. These injuries are divided into five grades and their treatment options are divided into four groups. Penile strangulation is considered a rare andrological emergency that may lead to a wide range of complications. Penile strangulation may be related to mental retardation in children or hypersexuality, abnormal sexual preferences in adults. This case report shows a 23-year-old male who presented to the emergency room with agonizing pain related to a metallic ring placed at the base of the penis for more than 8 hours during an attempt of masturbation. Removing this metallic ring and saving the patient’s penis from gangrene without causing injury were very challenging. Patient was successfully managed and had an uneventful recovery.

The use of metallic and non-metallic objects on the penis for erotic pleasure is not rare. Sometimes penile strangulation can occur, but prompt attention and careful management can lead to full recovery. Penile strangulation, if unrelieved, may lead to penile engorgement, ulceration, necrosis of the spongiosum and cavernous tissue, urinary extravation and even gangrene. We report on a case of a young man who presented with a metal ring jammed around the penis. Metallic objects cause less severe injuries than non-metallic ones, but they are difficult to remove. In these cases a metal cutter, saw, heavy drill hammer and chisel can be used. It is easier to remove non-metallic objects but injuries are severe. The choice of method for removing an object depends upon its type, size, how long it has been in place and availability of equipment.

Placement of constricting devices around the penis for autoerotic purposes or increasing of sexual performance represents a well-known challenge for urologists. Penile incarceration is a urologic emergency with potentially severe clinical consequences. In many cases a rapid intervention and a sudden removal of the foreign body it is enough so that patients need no further intervention. We report three different cases of strangulating objects (metallic ring, metal bearing and plumbing pipe) presented at our emergency department and three different methods of devices extraction practiced. Remove these devices can be challenging and often requires resourcefulness and multidisciplinary approach.
