Visceral malignancies often present as cutaneous lesion and are an important clue towards the diagnosis. Authors report a case of a 64 years old male, who presented to us with right hemiparesis and multiple purple-red skin lesions. Systematic evaluation, thorough history and histological examination of the skin lesion showing the characteristic lesion lead to a final diagnosis of Renal Cell Carcinoma. Although in the history and physical examination there was no sign of Renal Cell Carcinoma, but eventually the case turned out to be an extensively spread cancer. This kind of Cutaneous lesions in a case of RCC is a rare phenomenon and has not been frequently mentioned in the previous literature, thus this case reminds us about the importance of a detailed clinical look out in each and every patient and various clues to make a correct and timely diagnosis of systemic diseases. The list of visceral malignancies causing cutaneous metastasis is quite long and requires a battery of test to diagnose the same.

Renal cell carcinoma is the most common kidney tumor in adults. Cutaneous metastasis is a rare first symptom of the disease. This paper describes the diagnosis of a renal cell carcinoma that was indicated by cutaneous metastasis in the head and neck region, and considers the etiopathogenesis of such cases. A careful skin examination is important to detect cutaneous metastasis associated with renal cell carcinomas. Metastatic skin lesions in the head and neck region must be taken into consideration during a differential diagnosis.

Metastatic tumours of the distal extremities, also known as acrometastases, are rare. The majority of the acrometastases involve bones—involvement of the soft tissues of the feet and hands is extremely rare. We report a case of clear cell renal cell carcinoma metastasised to the soft tissues of the foot. The patient presented with pain and swelling in his right foot. Diagnosis of acrometastases frequently gets delayed due to the rarity of this condition and resultant low clinical suspicion. Possibility of metastatic disease should be entertained as an important differential diagnosis when patients with a history of cancer present with musculoskeletal symptoms. A systematic evaluation incorporating thorough clinical assessment, advanced imaging techniques like MRI and pathological examination is critical to establish the diagnosis.

The clinico-radiologic, gross, microscopic and ultrastructural findings in 2 cases of renal oncocytoma are reported. The diagnosis of renal oncocytoma has important clinical and prognostic implications, since the neoplasia is usually benign despite its often large size. The differential diagnosis with other renal neoplasms, in particular renal cell carcinoma, is discussed. The problem of the pre-operatory diagnosis of renal oncocytoma is explored on the basis of clinico-radiologic findings and/or histologic examination of pre- or intraoperatory biopsy. In view of the tumor's benign nature, this diagnosis implies tumorectomy, but its close resemblance to renal cell carcinoma at present counsels nephrectomy. A final diagnosis of oncocytoma may be formulated only after histologic examination of several specimens and ultrastructural confirmation.
